Oak Street Health Brings Value-Based Primary Care to Medicare Recipients in Youngstown

PR Web

YOUNGSTOWN, Ohio (PRWEB) November 19, 2019

Oak Street Health, a network of primary care centers that delivers value-based care to adults on Medicare, opened a new center in Youngstown, its fourth center in Ohio. Committed to rebuilding healthcare as it should be, Oak Street Health's model focuses on quality of care rather than quantity of services. Its proactive approach to integrated and preventive services improves outcomes for its patients.

The center will serve Youngstown residents at 3551 Belmont Ave. Patients will have a healthcare experience they may not have encountered before, including extended time with their provider and individualized treatment plans, community-centered support for social wellness, and access to transportation to and from the center for eligible patients. Supplementary services, such as behavioral health support and Medicare education classes, will also be offered to help older adults meet their unique needs in one convenient location.

"Throughout the nation, there is a growing need for high-quality, preventative care options that are individualized to each patient," said Oak Street Health Regional Vice President, Gina Chia. "We are eager to meet this need with our approach that specializes in a broad spectrum of healthcare services for older adults with complex medical needs. Our model is dedicated to treating the whole person, and we're thrilled to bring our services to help Youngstown residents live a healthy life."

"Expanding our reach in the Youngstown area is an important milestone for Oak Street Health, as we've already seen how our model has made a difference in our neighboring centers," added Oak Street Health CEO, Mike Pykosz. "Our innovative community-based approach can help local seniors live healthy lives, and we're excited to offer our services to residents in Youngtown with this new center."

The care teams at Oak Street Health are deeply embedded in each of the neighborhoods they serve with the majority of providers living near the local centers, emphasizing the importance of community support and relationship building. Ohio is already home to three other Oak Street Health centers in Cleveland, located in Glenville, Lee Harvard and Westown. Oak Street Health accepts multiple health plans at all locations.
