Wisconsin releases enforcement actions for December
The Wisconsin Office of the Commissioner of Insurance recently released a list of administrative actions for December.
The actions are issued by OCI when licensing applicants or licensed individuals or companies have violated Wisconsin insurance statutes or regulations. In some cases, the individual or company denied the stated allegations but consented to the action taken.
Any forfeitures paid are deposited in Wisconsin’s Common School Fund administered by the Board of Commissioners of Public Lands. Earnings from the Fund are distributed to all public K-12 schools in the state and are used by school libraries to purchase materials such as books, newspapers, and computers.
Allegations and Actions Against Agents
Dawn M. Buntrock, W8304 Basswood Rd., Shawano, WI 54166, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Wayne Coleman Jr., 2413 Freshford Dr., Waunakee, WI 53597, had his insurance intermediary license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
James Hardill, 7501 W. Oakland Park Blvd., Ste. 306, Tamarac, FL 33319, had his application for an insurance intermediary license denied. This action was taken based on allegations of having his license permanently revoked and violating an order of the insurance commissioner.
Alicia Hart, 3538 Fairburn Rd., Apt. 2202, Douglasville, GA 30135, had her application for an insurance intermediary license denied. This action was taken based on allegations of failing to pay state income taxes.
Charles Hughes, 3504 SW 4th St., Apt. B, Deerfield Beach, FL 33442, had his application for an insurance intermediary license denied. This action was taken based on allegations of failing to disclose administrative actions taken by other states, having had a license revoked by another state, and violating insurance laws in another state.
Mercedes Leininger, 2319 Layard Ave., Apt. 17, Racine, WI 53404, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Christopher D. Lyons, 4723 Kappus Dr., Apt. 3, Eau Claire, WI 54701, had his application for an insurance intermediary license denied. This action was taken based on allegations of having a criminal conviction substantially related to the circumstances of holding an insurance license.
Ian Martin, 760 S. Brevard Ave., #312, Cocoa Beach, FL 32931, was issued an insurance intermediary license pursuant to a stipulated agreement. This action was taken based on allegations of having a history of administrative actions and a lawsuit involving misrepresentation.
Luis Martinez, 2445 S. 30th St., Milwaukee, WI 53215, had his application for an insurance intermediary license denied. This action was taken based on allegations of failing to disclose aliases and administrative actions taken by other states, and having a criminal conviction substantially related to the circumstances of holding an insurance license.
Fabio Muhameti, 310 E. South Blvd., Troy, MI 48085, had his insurance license summarily suspended. This action was taken based on allegations of making misrepresentations relating to the business of insurance and committing fraudulent insurance acts.
Hannyher Parra Molinos, 5375 N.W. 7th St., Apt. 520, Miami, FL 33126, agreed to the revocation of his insurance intermediary license. This action was taken based on allegations of failing to respond to inquiries by OCI.
Paul H. Pulido, 580 W. Riverwood Dr., Apt. 310, Oak Creek, WI 53154, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Saedallah Salem, 3100 W. American Dr., Greenfield, WI 53221, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Brad Sauer, 2263 Samantha St., Apt. 22, De Pere, WI 54115, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Sierra Secker, 3652A E. Barnard Ave., Cudahy, WI 53110, agreed to the issuance of a one-year probationary insurance license. This action was taken based on allegations of failing to disclose a criminal charge.
Sarah L. Shipp Silva, 22127 Altona Dr., Boca Raton, FL 33428, had her application for an insurance intermediary license denied. This action was taken based on allegations of failing to disclose administrative actions taken by another state.
Laura J. Spiering, 1763 Ballinamore Pl., De Pere, WI 54115, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Rashad J. Stewart, 8565 S. Eastern Ave., Ste. 150, Las Vegas, NV 89123, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Shanotta Thomas, 3418 Victoria Brook Ln., Charlotte, NC 28208-4256, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely report two (2) administrative actions taken by another state and violating a previously issued warning letter.
Linda Todd-Ferguson, 934 Brimwood Cir., Cantonment, FL 32533-9514, had her insurance license denied. This action is based on allegations of having criminal convictions substantially related to insurance.
Michael Van Horn, 6211 Greenleigh Ave., Ste. 200, Baltimore, MD 21220-2074, was ordered to pay a forfeiture of $1,000.00. This action was taken based on allegations of allowing another agent to use his NPN inappropriately.
Tonuary Williams Robinson, 11086 County Rd. 167, Unit 3, Tyler, TX 75703, had her application for an insurance intermediary license denied for 90 days. This action was taken based on allegations of having a regulatory action taken with regard to an occupational license relating to financial irresponsibility in the conduct of business.
Jerry Yang, 459 S. Military Rd., Fond Du Lac, WI, 54935, had his insurance license denied. This action was based on allegations of failing to disclose previous criminal convictions on an insurance license application.
Anthony Zito, 1341 3rd Ct., Vero Beach, FL 32960-5803, had his insurance license denied. This action was taken based on allegations of failing to disclose administrative actions on an insurance license application, coercive business practices, and a previous Wisconsin license suspension.
Allegations and Actions Against Companies
A. A. Murrell & Associates, LLC, 5030 King Arthur Dr. NW, Rochester, MN 55901, stipulated to the voluntary surrender of its Wisconsin license and agreed to a 5-year bar on reapplication. This action was taken based on allegations of failing to pay a previous forfeiture when due.
Great Life Solutions, 24547 Creekside Dr., Farmington Hills, MI 48336, had their insurance license revoked. This action was taken based on allegations of failure to pay a previous forfeiture when due, and other insurance violations.
Watermark Insurance Agency Inc., 9360 Oak Ave., Waconia, MN 55387, had their insurance license revoked. This action was taken based on allegations of failure to reply and appear, and failure to pay a previous forfeiture.
Allegations and Actions Against Other Entities
Martha Moyer of Black River Falls, WI, was ordered to pay a forfeiture of $100.00. This action was taken based on evidence that she committed a fraudulent insurance act by knowingly presenting a false claim for payment of a loss.
