Nuevo programa combina Medicare y Medi-Cal/New Program Combines Medicare and Medi-Cal

El condado de Santa Clara es uno de siete condados en California (including San Diego) que participará en Cal MediConnect, que busca ayudar a personas de edad avanzada y/o con discapacidades, que son elegibles para los programas de cobertura de salud Medicare y Medi-Cal, a encontrar el mejor servicio de salud que mejor se adapte a sus respectivas necesidades.

El programa tendrá una duración inicial de tres años, y tendrá la misión de coordinar los servicios sociales y de salud para quienes califican para ambos programas mencionados- los profesionales se refieren a estas personas como duales, o de doble elegibilidad. Es decir, CCI combinaría los beneficios ofrecidos tanto por Medicare y Medi-Cal, en un solo programa.

Dificultades dentro del sistema actual

"El sistema actual resulta muy complicado para que las personas lo naveguen; no hay datos suficientes para entender los servicios; hay duplicidades; la gente no está recibiendo los servicios que necesitan", dijo Sarah Steenhausen, Asesora principal de las políticas de la Fundación SCAN, [www.thescanfoundation.org] una organización sin fines de lucro que tiene la misión de ayudar a las personas de edad avanzada a conservar su dignidad e independencia.

Añadió, "Este programa intenta promover un sistema coordinado para proveer servicios a los individuos de doble elegibilidad, al mejorar la coordinación de cuidados, mejorar la calidad de estos, y ayudar a las personas a mantenerse con salud y en casa lo más que se pueda".

Los duales ya han comenzado a recibir unos sobres azules de parte del Departamento de Servicios de Salud de California, a principios del 2014. Estos sobres contienen información importante sobre como inscribirse al programa, elegibilidad disponible, y otras respuestas a las preguntas que los duales podrían tener sobre el programa. El estado ha enviado estas noticias 90, 60 y 30 días antes del cumpleaños de la persona que califica para el programa.

El recibir un sobre azul fácilmente podría perderse en el correo, o confundirse con el correo chatarra. Esto es uno de varios obstáculos que enfrenta el programa, cómo hacer llegar a la población la información necesaria para explicar la complejidad del programa.

"La gente recibe un sobre azul y lo primero que piensan es que han perdido su cobertura", dijo Marty Lynch, Director Ejecutivo de Lifelong Medical Care, una organización sin fines de lucro que sirve en su mayoría a las personas de edad avanzada del condado de Alameda. "Es difícil comprender lo que está pasando. Nuestros consumidores nos traen esta correspondencia y nos preguntan, '¿Qué está sucediendo' 'Así que no tiren a la basura su sobre azul".

Las personas pueden buscar información sobre el programa en sitios de internet, comowww.calduals.org, en 13 idiomas diferentes (incluyendo español), o pueden llamar al 1-844-580-7272.

"Estamos conscientes de que el idioma es un gran problema", dijo Sonali Pamami, representante de la Cooperativa de Servicios para la Edad Avanzada de Santa Clara.

La cooperativa está compuesta por más de 80 organizaciones, entre ellas, On Lok Lifeways, quienes intentan conectar a potenciales consumidores con los proveedores de servicios sociales.

"Estamos tratando de identificar qué grupos necesitan esta información", dijo Pamami, "porque sabemos que existe gran diversidad en nuestro condado y las necesidades en cuanto al lenguaje son muy distintas; así que en nuestros eventos con la comunidad intentamos tener representantes que hablan los diversos idiomas para responder a las preguntas de la gente".

Los planes de salud: sus opciones

Cal MediConnect incluye planes públicos, sin fines de lucros y de compañías privadas, lo cual resulta ser otra dificultad del programa.

Lynch, quien enseña salud pública en UC Berkeley, explicó también que el programa podría enfrentar problemas para sobreponerse a las críticas negativas que rodean los planes de salud existentes.

"Los doctores han lidiado durante mucho tiempo con los planes de Medicare de organizaciones de mantenimiento de la salud (HMO por sus siglas en inglés), algunos buenos, algunos terribles", dijo Lynch. "Por lo tanto, tienen desconfianza, especialmente a los planes de salud privados, debido a que pueden realizar un buen trabajo, pero al final su principal objetivo es el de satisfacer a sus accionistas". Además, añadió, algunos doctores privados también temen de que sean obligados a cobrar las bajas tarifas que ofrece el programa estatal de Medi-Cal,

"Pero eso no es verdad necesariamente", continuó Lynch, "Otro problema (para los proveedores de salud) es ¿dónde se llevarán a cabo los servicios de cuidado?" Una lección que Lifelong Medical Care ha aprendido, explicó, es asegurar que su personal tome decisiones en la cercanía del lugar donde se lleven a cabo los cuidados de salud. Si los coordinadores no se encuentran cercanos al lugar donde los duales recibirán sus cuidados, entonces no será posible que exista una coordinación verdadera de los cuidados.

Los pacientes de doble elegibilidad, o duales, ahora tendrán sus cuidados coordinados bajo una sola entidad de cuidados administrados. "La gente tiene la opción de elegir diferentes planes de cuidados administrados dependiendo del condado donde viven, pero se inscribirán en uno de esos planes que ofrecen un varios servicios", dijo Steenhausen.

Coordinando Medicare y Medi-Cal

Las beneficiarios con doble elegibilidad podrán rechazar el programa. No obstante, de no hacerle saber al estado que lo rechazan, serán inscritos automáticamente. Las personas que reciban los sobres azules podrán unirse inicialmente y salirse del programa más tarde, para regresar al programa médico dentro de sus beneficios federales de Medicare al cual ya están inscritos.

Medicare es un programa de seguro de salud federal para personas de 65 años o más, y para personas que no pueden trabajar debido a sus discapacidades, sin importar su nivel de ingresos. Sin embargo, Medicare no lo cubre todo.

New Program Combines Medicare and Medi-Cal

Santa Clara County is one of seven California counties (including San Diego county) participating in Cal MediConnect, a three-year pilot program that aims to help seniors and people with disabilities, who are eligible for both Medicare and Medicaid (Medi-Cal in California) medical programs, choose the health care that best fits their needs.

The main goal of Cal MediConnect's is to coordinate the health and social care services that dual eligible (or "duals" as professionals call them) need. (For more information about Cal MediConnect.)

Existing System Difficult

"The existing system is extremely difficult to navigate for most people; there is lack of data to understand services; there is duplication; people are not getting the services they need," said Sarah Steenhausen, Senior Policy Advisor of The SCAN Foundation.

She added, "This program is trying to promote a coordinated service delivery system for dual-eligible individuals, by improving care coordination, driving higher-quality care, and helping people stay healthy in their homes for as long as possible."

They started receiving blue envelopes from the California Department of Health Services back earlier in 2014. The blue envelopes contains important information about how to enroll, eligibility, and other questions dual eligibles may have about the program. The state is sending these notices 90, 60 and 30 days before the birthday month of each eligible person.

One of the challenges the program is facing is reaching out to its eligible population, and helping them understand the complex issues about how this program works. Many could easily mistake the blue envelopes with junk mail and discarded them.

"People receive an envelope and they think they have lost their coverage," said Marty Lynch, Executive Director at Lifelong Medical Care based in Berkeley. "It is difficult to understand what's going on. Our consumers bring these letters in and ask us, 'What is going on?' So don't throw the blue envelope away."

Health-Plan Choices

Cal MediConnect includes both nonprofit and for-profit health care plans, depending on the county, which poses another challenge for the program.

Lynch, whose nonprofit Lifelong Medical Care serves mainly lower-income elders in Alameda county and who teaches public health at UC Berkeley, said Cal MediConnect may have problems overcoming mixed reviews for existing manage care plans.

"Doctors have gone through a long history of dealing with Medicare Advantage [HMO] Plans, some of which are good, some of which are terrible," said Lynch. "So they are suspicious, especially of for-profit health plans, because they may do a good job, but their main goal is to take care of the stockholders." Also, he said, some private doctors are a suspicious that they will be forced into receiving low payment rates from the state's Medi-Cal program for the poor.

"That is not necessarily true," said Lynch. "Another issue for [health care providers], is where is the care going to happen?" One lesson Lifelong Medical Care has learned, he explained, is to ensure then staff make decisions close to where the health care will actually be delivered to patients. If care coordinators aren't close enough to where duals will receive care, he said, real care coordination won't happen.

Dual eligibles will now have their plans coordinated under a single managed care entity. "People have different managed care plans to choose from depending on the county they live in, but they will enroll into one of those health plans with a range of services," said Steenhausen.

Coordinating Medicare and Medi-Cal

Dual-eligible beneficiaries can decline to sign up for Cal MediConnect, but they have to let the state know, or they will be automatically enrolled in the Cal MediConnect plan. People who get the blue envelopes can join initially and opt out of the program later, such as to go back to their existing medical program under traditional federal Medicare benefits.

In either case, though, duals who are not in Cal MediConnect must still be in an approved Medi-Cal managed care program under California's new Coordinated Care Initiative.

Medicare is the federal health insurance program for people 65 or older and those unable to work due to disability, regardless of their income level. However, Medicare does not cover everything.

Cal MediConnect will coordinate Medicare's health services with Medi-Cal's longterm services and supports that can be non-medical in nature. It aids people in personal care, mobility needs, transportation, shopping assistance, "anything you need to function independently and remain in the community, or you can receive in an institutional setting," said Steenhausen.

Cal MediConnect will have different enrollment dates for each county.

Duals in San Diego County can get free assistance in understanding the decisions they must make in Cal MediConnect by contacting the Health Insurance Counseling and Advocacy Program (HICAP), run by the nonprofit "Sourcewise."
