Choose Well San Diego: A county-funded website that can help you compare details on more than 600 licensed assisted living and memory care facilities side-by-side. The facility finder is a searchable database. Contact: (619) 795-2165; choosewellsandiego.org
ElderHelp: A non-profit providing services and information geared toward helping seniors remain in their own homes, offering care coordination, a Seniors A Go Go transportation program, caregiver education, a unique HomeShare program and an extensive local resource guide. Jewish Family Service: Helping San Diegans of all faiths, JFS programs include the On the Go transportation service; free minor home safety repairs and modifications with JFS Fix-It Service; a day program for those with early-stage dementia at the Balboa Avenue Older Adult Center, hot meals for homebound adults from the Foodmobile and geriatric care management. Long-Term Care Ombudsman: County program helps residents of skilled nursing homes, assisted living facilities and other long-term care facilities, with complaints related to care or personal rights, will investigate and resolve them. Meals on Wheels San Diego County: This meal delivery program distributes food six days a week (Sunday meals are delivered Saturday) to residents countywide.
