Statement of Work SPLIT RIM TIRE REPAIR

1. Purpose

This statement of work describes the maintenance and services for the Government-owned vehicles located at the Exercise Support Division (ESD), Marine Corps Air Ground Combat Center (MCAGCC), Twenty-nine Palms, CA 92278. The contractor shall provide all labor, tools, equipment, management and supervision, to perform the repair, mounting and dismounting of split rim tires. The contractor shall comply with all applicable Federal (including MCAGCC), State and Local regulations, laws and requirements, in performance of this contract.

2. Task Management The Contractor shall be responsible for complying with all applicable Federal Safety Commission Codes in respect to the service provided.

3. Service Locations ESD Compound on MCAGCC site.

4. Marine Corps Designate Representative a. The Government representative tasked to coordinate with the contractor is:

5. Permits No permits are required for this contract.

6. Contract Authority Notwithstanding any other provision of this contract the Contracting Officer shall be the only individual authorized to redirect the effort or otherwise amend any of the terms of this contract.

7. Work Hours The scheduled work hours are Monday through Friday 0800-1600, excluding legal holidays.

8. Legal Holidays Equipment will not operate during legal holidays except for emergency job repairs.

9. Invoices and Payment Wide Area Workflow

10. Specific Requirements

The vehicles to be serviced include the Container handler, RT Kalmar Ind., (tire size 29.5R35). The contractor shall remove the flat tire from the wheel while the wheel remains on the vehicle. The task requires that the contractor raise the vehicle using a 40-50 ton (or equivalent) jack and remove the flat tire with the proper tools. The contractor shall install a new tire, which will be provided by the Government while the wheel remains on the vehicle. These tasks will be performed at the ESD vehicle parking lot, in an area prescribed by the Government. The old tire shall become the property of the contractor, who shall remove it from the Combat Center and be responsible for it thereafter. Should the contractor recommend that the wheel be removed from the vehicle for any reason, work shall stop and the vendor shall consult with the Government as to how or if to proceed. The vendor will not be required to remove the wheel from the vehicle.

Other vehicles include the Tractor, Articulated street, MR Tram, (tire size 20.5-25), the Grader, Road, Motorized, (tire size 14.00-24TG), MAC 50 (tire size 445/95R25), M105A2 and M1492 Trailer tires (tire size 9.00-20), and M353 trailer tires (tire size 11.00-20). The Government shall remove the flat tire and the split-rim wheel from the vehicle. The contractor will pick-up and transport the tire/wheel assembly and a new tire, which will be provided by the government from the ESD vehicle parking lot, to its facility, or the Contractor, may change the tire in an area prescribed by the Government but only with Government approval. The contractor will dismount the flat tire, and install the new Government furnished tire onto the Government supplied split-rim wheel. It shall be the responsibility of the vendor to insure that the Government furnished equipment is returned to the Government. The Government shall put in place a process to insure that equipment provided to the vendor is validated upon return as Government property. In the event that the contractor determines that the Government supplied wheel is not acceptable for reuse, the contractor will return the wheel to the government for evaluation. If the Government agrees that the wheel is unacceptable for reuse, the government will provide a replacement wheel to the contractor. The removed tires shall become the property of the contractor, for reuse or disposal.

When ESD requires the contractor to perform these services, it shall notify the contractor of the requirement. The contractor shall change the tire on the Container Handler within 48 Hours (not including weekends), after receiving notification from ESD. The contractor shall return the used wheels and new tires in operable condition, to ESD within 72 hours (not including weekends) of picking them up from ESD. ESD will be responsible for establishing a system that tracks the type of wheel repaired and the number of times repairs are done on a monthly basis. This system will be used to reconcile the Contractor invoices that will be submitted on a monthly, semi-monthly, or weekly basis.

11. Period of Performance: Oct 1, 2014 to Sept 30, 2015 plus Four (4), one (1) year option periods

12. Security Requirements

a. The Contractor will call to inform of service dates and times to be coordinated through Thomas Burke at 830-3927.

b. All contractor personnel performing work under this contract who require access to military installations shall obtain personal entry and vehicle passes from the Provost Marshall's Office (PMO) Bldg. 901 at main gate, phone number 760-830-6693.

c. The Commanding General of each base has broad authority to remove or exclude any person in fulfilling his responsibility to protect personnel and property, to maintain good order and discipline, and to ensure the successful and uninterrupted performance of the Marine Corps mission. In the exercise of this authority, the Commanding General may refuse to grant personal entry passes or may bar contractor employees, including employees who have been granted a personal entry pass. Refusal to grant an employee a personal entry pass or barment of an employee does not relieve the contractor of the responsibility to continue performance under this contract.

d. The contractor's employees shall observe and comply with all base rules and regulations applicable to contract personnel, including those applicable to the safe operation of vehicles, and shall not be present in locations not required for the proper performance of this contract.

e. Contractor personnel performing work under this contract shall wear uniforms or other clothing that distinguishes them as an employee of the contractor.

f. Contractor personnel and equipment entering a military installation are subject to 100% security checks. Contractor personnel shall follow any direction given by Military Police or other security of safety personnel. In compliance with the above, the contractor must be prepared to provide proof of the following: - A valid driver's license - Proof of Insurance - Current Registration - Knowledge of the contract # they are performing under

g. The contractor shall not employ any individual for the performance of this contraction in violation of DOD Joint Ethics Regulations (DOD Directive 5500.7-r) or if such employment would create a conflict of interest or an appearance of a conflict of interest.
