AMARILLO, TEXAS - GREASE TRAP PUMPING/CLEANING

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpar 12.6, as supplemented with additional information in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Award will be made to the lowest price technically acceptable that meets all technical requirements of this combined synopsis/solicitation.

Request for Quote (RFQ) VA-258-10-RQ-0216 is a combined synopsis/solicitation for commercial items. This combined synopsis/solicitation is issued as a RFQ. Submit written offers only, oral offers will not be accepted. All firms or individuals responding must be registered with the Central Contractor Registration (CCR).

This combined synopsis/solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-44. This requirement is set aside 100% Small Business under North American Industrial Classification Standard (NAICS) code: 562998; Small Business Size Standard: $7.0 million.

PROJECT TITLE: GREASE TRAP PUMPING

PROJECT LOCATION: Thomas E. Creek VA Health Care System

6010 West Amarillo Boulevard

PROJECT DESCRIPTION: Service to perform annual maintenance pumping/cleaning of Dietetics 7,200 gallon and Canteen 1,000 gallon grease traps.

BACKGROUND: The Thomas E. Creek VA Health Care System located in Amarillo, Texas, is seeking service to perform maintenance of two grease traps, consisting of pumping and cleaning. There is approximately 5,400 gallons of grease in the Dietetics grease trap, which has a 7,200 gallon capacity. There is approximately 750 gallons of grease in the Canteen grease trap, which has a 1,000 gallon capacity. The service will include pressure washing the walls and floors of grease/solids, transporting the waste to the disposal site, any landfills fees, and if possible, recycle the grease (brown grease).

REQUIREMENTS: The Contractor must be registered with the Texas Commission on Environmental Quality (TCEQ) to pump and transport grease trap waste. Transporters of brown grease must be registered under Texas Administration Code (TAC) Title 30, Section 312. The Contractor shall provide a copy of the Safety Plan according to the Environmental Protection Agency (EPA) requirements, including the Standard Operating Procedures (SOP), the safety requirements and personal protective equipment. The Safety Plan must be submitted with the proposal for review and approval. Proposals without Safety Plans will not be technically acceptable.

LABOR STANDARDS: As incorporated in FAR 22.10, Contractors are required to comply with the Service Contract Act of 1965. The location of the wage determination tables for each location covered under the terms of the contract can be found by going to http://www.wdol.gov/sca.aspx and following the directions below for each city.

For Amarillo, select Texas as the state, Potter as the county, then click "continue". When asked if these services were previously performed under an SCA-covered contract, click "no". Select "no" when asked if the services are Non-Standard Services. The Wage Determination requested will appear in the box below.

The Wage Determination for Amarillo, Texas is: Wage Determination Number: 2005-2517, revised 09/24/2009, revision number 12.

SUBMISSION TO THE VA: The Contractor shall submit the following items with the proposal:

1. Insurance Verification

2. Proof of TCEQ registration under TAC Title 30, Section 312

3. Safety Plan

4. Cost Break-Down

Failure to provide any of these items will result in the offer being deemed technically unacceptable.

PERIOD OF PERFORMANCE: The period of performance for the base year will be from date of award to June 30, 2011. The services shall be performed and completed within 15 days of the award date and will last no longer than two working days. Each option year will be renewed at the government's discretion in accordance with Federal Acquisition Regulations (FAR) clause 52.217-9.

SUMMARY OF WORK:

ACCESS TO WORK SITE: The Contractor shall contact the Contracting Officers Technical Representative (COTR) for access hours. Normal work hours for the Amarillo VA are 8:00am to 4:30pm, some work may be required outside of the normal working hours and the Contractor is required to notify the COTR in advance to schedule after-hours work.

CONTRACTOR FURNISHED MATERIEL: The Contractor shall be required to furnish all tools and equipment necessary for the pumping/cleaning.

SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES: The general outline of work to be performed includes, but is not limited to the following items:

1. Equipment: The Contractor shall be required to provide all of the necessary equipment to do all of the services required in the Statement of Work.

2. Service Information: The Contractor shall confirm these descriptions of services with the COTR. The COTR will be able to confirm or make adjustments to any of the following descriptions of services.

Service to include cleaning: pressure washing walls and floors of grease/solids, transporting to disposal site, and landfill fees. The Contractor must be registered with TCEQ to pump and transport grease trap waste. If possible, recycle grease (brown grease). Transporters of brown grease must be registered under 30 TAC, Section 312.

The service shall also include a manifest that states the following information:

General Information:

" Facility Name, address, and phone number

" Type of waste (e.g., grease trap)

" Trap or Tank capacity and type and amount of waste collected

" Date of service

" Signature verifying the information and approving the disposal site to be used.

Transporter Information:

" Transporter's name, address, and phone number

" Transporter's TCEQ registration number

" Transporter's city or counter permit number, if required

" Gallons transported

" Driver's name and signature

Disposal Site Information:

" Disposal site's name and permit number or registration number

" Disposal site's address

" Date and amount of waste received; and

" Site representative's name and signature

5. Schedule: All work is to be accomplished within two calendar days during the normal business hours unless the COTR is notified in advance.

EXECUTION: All work shall be performed in a safe and efficient manner.

Site Clean-up: The Contractor shall clean job site and dispose of all debris appropriately off grounds. Cleaning shall be to the satisfaction of the Contracting Officer (CO) or the COTR. Any areas of work or adjacent areas disturbed shall be restored to original conditions with all associated costs.

Final Inspection: A pre-final inspection of the project shall be performed to compile a punch list, if necessary, to help expedite efficient project completion. The pre-final inspection shall be conducted by the Contractor, the COTR, the Administrative Contracting Officer (ACO) and the CO if available. Any items found to be deficient shall be addressed and completed to the satisfaction of the CO.
