State Disciplines Health Care Providers

OLYMPIA, Wash., March 7 -- The Washington State Department of Health issued the following news release:

The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department's Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency website. Click on "Look up a healthcare provider license (http://www.doh.wa.gov/LicensesPermitsandCertificates/ProviderCredentialSearch.aspx)" in the "How Do I?" section of the Department of Health website (http://www.doh.wa.gov/home.aspx) (doh.wa.gov). The site includes information about a health care provider's license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are encouraged to call and report their complaint.

Clallam County

In January 2014 the Nursing Assistant Program charged registered nursing assistant Michelle Madona Kinsman (NA00176365) with unprofessional conduct. Kinsman was allegedly seen smoking marijuana with a patient and talking about swapping medications. She allegedly tested positive for methadone, a drug for which she didn't have a current valid prescription.

Clark County

In January 2014 the Nursing Commission ended conditions on the credential of registered nurse Traci Anne Phillips (RN60177196).

In January 2014 the Pharmacy Commission charged pharmacy assistant and pharmacy technician Mathew James Webb (VB60261225, VA60288265) with unprofessional conduct. Charges say Webb was arrested after his employer detained him in connection with diverting controlled substances. Webb allegedly admitted diverting controlled substances from patients and from his employer for his own use.

Cowlitz County

In January 2014 the Chemical Dependency Professional Program charged chemical dependency professional and chemical dependency professional trainee Robert Scott Hauff (CP60290688, CO60146888) with unprofessional conduct. Hauff allegedly began dating, moved in with, and remains in a romantic relationship with someone he treated as a client.

King County

In January 2014 the Nursing Assistant Program charged registered nursing assistant Harmony C. Austin (NA00120570) with unprofessional conduct. The Department of Social and Health Services found she financially exploited a vulnerable adult, taking between $3,000 and $3,700 from a resident at an adult family home she managed. Austin can't be employed in caring for or having unsupervised access to vulnerable adults. She hasn't responded to a Department of Health investigator's inquiry.

In January 2014 the Pharmacy Commission charged pharmacy assistant Mario P. Beall (VB60195560) with unprofessional conduct. Beall allegedly didn't meet conditions of a 2013 stipulation requiring him to comply with a substance abuse monitoring program.

In January 2014 the Nursing Assistant Program charged registered nursing assistant and certified nursing assistant Rean Tenero Unida (NA60243196, NC60274835) with unprofessional conduct. The Department of Social and Health Services (DSHS) found she neglected a Fircrest School resident. Charges say when the resident fell and sustained a fracture, Unida helped her into her bed but didn't report the incident and later made a false report. DSHS placed Unida on its Residential Care Services Registry, prohibiting her from being employed in caring for or having unsupervised access to vulnerable adults.

In January 2014 the Chiropractic Commission modified a 2012 agreement with chiropractor Dawn M. Hunter (CH00034116), which was also modified in November 2013. The modification prohibits Hunter from providing chiropractic treatment except for completing community service requirements. Hunter has repeatedly submitted insurance claims for massages performed by people other than the practitioner whose name was on the claims.

In January 2014 the Chiropractic Commission ended conditions on the credential of chiropractor Vivian M. Ledesma (CH00002609).

In January 2014 the Veterinary Board entered an agreement with veterinarian Melissa Norton (VT00003993) that places her on probation for at least two years. She must pay a $1,000 fine, and complete continuing education in recordkeeping, veterinary law, and ethics. Norton kept inadequate records, didn't provide records to clients or investigators in a timely manner, and didn't register X-ray machines as required by law.

Pacific County

In January 2014 the Emergency Medical Services and Trauma Systems Program charged paramedic Raela D. Wetterauer, also known as Raela D. McVey (ES00117356) with unprofessional conduct. Charges say Wetterauer, a 10-year Raymond Fire Department employee, was under the influence of controlled substances while on duty. She had slurred speech, uncoordinated movement, and confusion or impaired decision making. She allegedly inserted an intravenous line into a patient after another paramedic told her he'd already installed an IV line in the patient's other arm. Wetterauer allegedly misused controlled substances for much of 2013, getting multiple prescriptions for the same or similar drugs from different physicians, didn't disclose that to either physician, and didn't follow an agreement to get controlled substance prescriptions from only one physician and pharmacy.

Pierce County

In January 2014 the Nursing Commission entered an agreement with registered nurse Karen Ann Price (RN60181825) that requires her to undergo evaluation for a substance abuse monitoring program. She was convicted of driving while intoxicated in 2013.

In January 2014 the Home Care Aide Program ended conditions on the credential of home care aide Jazmine Nicole Stanley (HM60394748).

In January 2014 the Nursing Assistant and Counselor programs charged registered nursing assistant and agency affiliated counselor William Robert Fleming (NA00105194, CG60173109) with unprofessional conduct. Fleming was convicted of forgery and third-degree theft in 2013.

Skagit County

In January 2014 the Nursing Commission entered an agreement with registered nurse and advanced registered nurse practitioner Robyn Margot Choffel (RN00103955, AP30003913) requiring her to pay a $5,000 fine and complete two programs in professional ethics. While working for the Stillaguamish Tribe, Choffel falsified her time sheets and was overpaid approximately $5,929.87.

Snohomish County

In January 2014 the Medical Commission ended probation for physician Eric R. Shibley (MD60108064).

In January 2014 the Medical Commission charged physician Roger B. Olsson (MD00015303) with unprofessional conduct, alleging that his treatment of 14 chronic-pain patients violated the standard of care. Olsson allegedly focused on prescribing controlled substances without appropriate attention to the underlying cause of pain. Charges say he didn't ensure patients took controlled substances as prescribed even when he had evidence of possible diversion or misuse.

Spokane County

In January 2014 the Home Care Aide Program charged home care aide Samuel W. Wyant (HM60311473) with unprofessional conduct. On his application, Wyant said he'd never surrendered a health care credential in connection with, or to avoid, state action. Charges say he surrendered his Oregon nursing assistant credential in 2009 after he didn't complete a required drug screening ordered by that state when he was placed on probation in 2008.

In January 2014 the Nursing Assistant Program charged registered nursing assistant Marjorie A. Flatt (NA60206939) with unprofessional conduct. The Department of Social and Health Services (DSHS) found she mentally abused a vulnerable adult in a supported living setting. By placing Flatt on its Residential Care Services Registry, DSHS will not allow her to be employed in caring for or having unsupervised access to vulnerable adults.

In January 2014 the Nursing Commission entered an agreement with licensed practical nurse Jody Marie Dunham (LP00034245) requiring her to undergo evaluation for a substance abuse monitoring program. After the care center where she worked investigated a complaint about narcotics that went missing during Dunham's shifts, she admitted to a Department of Health investigator that she'd taken patients' narcotic medications for her personal use.

Yakima County

In January 2014 the Nursing Assistant Program charged registered nursing assistant Shawn R. Wear (NA00184246) with unprofessional conduct. The Department of Social and Health Services (DSHS) found Wear neglected vulnerable adults while he was working for a community living organization. DSHS will not allow him to be employed in caring for or having unsupervised access to vulnerable adults.

Out of State

California: In January 2014 the Dental Commission ended conditions on the credential of dentist and dentist general anesthesia permit holder Sohrab Moshiri (DE00005739, GA10000161).

Massachusetts: In January 2014 the Pharmacy Commission charged nonresident pharmacy Ameridose LLC (FO60080065, FO60223552) with being out of compliance with requirements for nonresident pharmacies. Charges say Ameridose has stopped operating its Massachusetts pharmacies.
