LOUIS, Feb. 22, 2019 /PRNewswire/ -- Centene Corporation (NYSE: CNC) announced today that it has formed Social Health Bridge™ Trust to help community based organizations and healthcare entities work more effectively to address the social determinants of health – the non-medical barriers to health such as food insecurity, housing instability/homelessness, unemployment, and lack of access to transportation.

"When community and healthcare organizations begin to work together, they often face barriers to collaboration in the form of different terminology, dissimilar funding structures, administrative complexity, lack of payment arrangements, and capacity limitations," said Michael F.
