The transaction is expected to close in the first quarter of 2018. Genova Health will manage two distinct divisions: care management and home care. Under the terms of the agreement, Accountable Aging and ProperCare will combine and offer care management services under the Accountable Aging brand. Genova Health's flagship home care company Family Tree In-Home Care will continue to offer home care services under the Family Tree brand. The entire team at both Accountable Aging and ProperCare, including management, will continue with the combined company.
