State Disciplines Health Care Providers

OLYMPIA, Wash., Jan. 7 -- The Washington state Department of Health issued the following news release:

The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department's Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency website. Click on "Look up a healthcare provider license" in the "How Do I?" section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider's license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County

In November 2012 the Chiropractic Commission charged chiropractor Gabriel Anguiano (CH00002745) with unprofessional conduct. He allegedly billed for services not provided.

In November 2012 the Counselor Program ended the probation order against certified counselor Edgar R. Gonzalez (CL60168254).

In November 2012 the Medical Commission placed the license of physician John C. Perry (MD00025747) on probation. He didn't refer a patient for proper treatment and diagnosis which resulted in treatment delays.

Chelan County

In November 2012 the Unlicensed Practice Program notified James Barrett of its intent to issue a cease-and-desist order. He allegedly advertises himself as a physical therapist and naturopathic doctor, and refers to himself as a doctor. He isn't licensed to practice health care in Washington.

Clark County

In November 2012 the Medical Commission continued a probation order against physician Patrick K. Chau (MD00030053). He failed to keep appropriate patient records, or follow proper prescribing practices.

Grant County

In November 2012 the Medical Commission reinstated the license of physician Mohammad H. Said (MD00018311) and placed his license on probation.

King County

In November 2012 the Occupational Therapy Practice Board placed conditions against the license of occupational therapist Linda S. Baker (OT00000498) with which she must comply. She worked as an occupational therapist on an expired license.

In November 2012 the Medical Commission ended an agreed order for physician Peter Brandon (MD00029025). His license is unrestricted.

In November 2012 the Chemical Dependency Professional Program charged chemical dependency professional trainee Earlesia Glenda-Ziehe Dipietro (CO60196954) with unprofessional conduct. Allegations include forming personal relationships with juvenile clients, falsifying a urinalysis report for a client, discussing information about clients with other clients and in Facebook posts, and persuading a drug-addicted patient to obtain a controlled substance for her.

In November 2012 the Chiropractic Commission charged chiropractor Vivian M. Ledesma (CH00002609) with unprofessional conduct. She allegedly billed for services not provided by using the wrong coding, didn't keep adequate treatment records, and didn't record results of treatments provided.

In November 2012 the Medical Commission ended an agreed order for physician Doane M. Rising (MD00034275). Dr. Rising's license is unrestricted.

In November 2012 the Chemical Dependency Professional Program ended the probation order against chemical dependency professional trainee Phillip B. Sims (CO60153072).

In November 2012 the Medical Commission granted a license to physician David N. Tran (MD60279954) and placed the license on probation for one year. His license to practice as a physician in the state of California was placed on probation.

In November 2012 the Respiratory Care Practitioner Program entered into an agreed order with Caroline L. Zimmer (LR00002009). She must comply with terms and conditions set against her license. She tested positive for controlled substances in a workplace drug screening and didn't comply with a substance abuse monitoring program.

Lewis County

In November 2012 the Nursing Commission charged registered nurse Susan Marie Taylor (RN00099538) with unprofessional conduct. She allegedly didn't comply with terms and conditions set against her license.

Pierce County

In October 2012 the Agency Affiliated Counselor Program denied the application of Olga S. Kimbrel (CG60312179). While practicing as a surgical technologist, she withdrew controlled substances for patients and didn't document the administration or wastage of the medication on 42 occasions. She admitted to diverting controlled substances for her own use.

In November 2012 the Nursing Commission charged licensed practical nurse Michelle R. Wenzel (LP00053632) with unprofessional conduct. She allegedly took Percocet from a patient's supply for her own use.

Skagit County

In November 2012 the Nursing Commission charged licensed practical nurse Chris R. Davidson, also known as Christine Ensley (LP00048042), with unprofessional conduct. She allegedly didn't keep adequate records for a patient, and didn't check and document the patient's blood sugar twice a day.

Snohomish County

In November 2012 the Nursing Commission charged registered nurse Clare Elizabeth Blodgett (RN00158328) with unprofessional conduct. She allegedly didn't comply with terms and conditions set against her license.

In November 2012 the Medical Commission placed the license of physician Eric R. Shibley (MD60108064) on probation for two years. The hospital where he worked ended his employment and permanently revoked his privileges.

Spokane County

In November 2012 the Unlicensed Practice Program notified Anissa D. Jenkins of its intent to issue a cease-and-desist order. She allegedly advertised massage services, including offers for full body massage. She isn't licensed to practice as a massage therapist in Washington.

In November 2012 the Agency Affiliated Counselor Program granted the application of Brandon E. Mast (CG60307229) and placed his certification on probation. He was convicted of shoplifting, two counts of theft, making or having burglary tools, and violating the Uniform Controlled Substance Act --possession of heroin, and criminal non-support.

In November 2012 the Health Care Assistant Program denied the application of health care assistant Nichole Rene McIntosh (HC60254592). She answered yes to a question about use of alcohol or drugs on the application and didn't respond to the program's offer to practice with certain conditions.

Thurston County

In November 2012 the Board of Pharmacy reinstated the license of pharmacist John W. Pemberton (PH00017890) and placed conditions against his license with which he must comply.

Out of State

Oregon: In November 2012 the Board of Pharmacy granted the application of pharmacy intern Kayllie Gabriela La Pointe (IR60288702) and placed her license on probation until October 2014. Conditions were placed against her license to practice as a pharmacy technician in Oregon.

Out of Country

Nova Scotia, Canada: In November 2012 the Medical Commission charged physician Carlo G. Carandang (MD00036496) with unprofessional conduct. The College of Physicians and Surgeons of Nova Scotia allegedly reprimanded Dr. Carandang and placed conditions against his license to practice in that jurisdiction.
