State Disciplines Health Care Providers

OLYMPIA, Wash., Dec. 6 -- The Washington state Department of Health issued the following news release:

The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department's Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency website. Click on "Look up a healthcare provider license" (https://fortress.wa.gov/doh/providercredentialsearch/) in the "How Do I?" section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider's license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Clallam County

In November 2012 the Dental Hygienist Program charged Judy C. Carroll (DH00004654) with unprofessional conduct. She allegedly provided treatment outside of her scope of practice, put patients at risk of harm by treating them with an unproven technique, and didn't schedule follow-up appointments after using the technique.

Cowlitz County

In November 2012 the Agency Affiliated Counselor Program ended the probation order for Raymond James Sellek (CG60179291).

Franklin County

In November 2012 the Nursing Commission charged registered nurse Kimberly M. Perry (RN00142012) with unprofessional conduct. She allegedly didn't comply with terms and conditions set against her license.

King County

In November 2012 the Agency Affiliated Counselor Program granted the application of Teresa Lynn Anderson-Harper (CG60301635) and placed her registration on probation for three years. She was found guilty of attempting to elude a police vehicle, two counts of domestic violence assault, conspiracy to deliver a controlled substance, theft, willfully violating community custody conditions, attempted violation of the Uniform Controlled Substances Act -- possession of cocaine, and possession of stolen property.

In November 2012 the Nursing Commission charged registered nurse Debra Estenson Cadiente (RN00075160) with unprofessional conduct. She allegedly worked as a registered nurse with an expired credential.

In October 2012 the Dental Commission placed the license of dentist Benjamin Dorantes (DE00008750) on probation for two years. He prescribed drugs to a family member and friend for non-dental conditions.

In November 2012 the Nursing Commission charged registered nurse Estera Gradinaru (RN00174148) with unprofessional conduct. She allegedly diverted liquid morphine from a patient for her own use and didn't comply with terms and conditions set against her license.

In November 2012 the Chemical Dependency Professional Program charged chemical dependency professional Lelan C. Jamison (CP00003694) with unprofessional conduct. He allegedly made inappropriate sexual comments to a patient and touched the patient in a sexual way.

In October 2012 the Board of Pharmacy reinstated the license of pharmacist Patrick G. Oyco (PH00021211) and placed his license on probation for one year.

Kitsap County

In November 2012 the Health Care Assistant Program ended the order of probation against Sandra Denise Stuckey (HC60202869).

Okanogan County

In November 2012 the Medical Commission charged physician William L. Dienst (MD00025927) with unprofessional conduct. Allegations include not meeting the standard of care when examining two patients, and smoking marijuana. He entered a plea agreement for charges of possession of marijuana and drug paraphernalia.

Pierce County

In November 2012 the Nursing Assistant Program charged registered nursing assistant Betty Ireigbe (NA60076979) with unprofessional conduct. In October 2012, the Department of Social and Health Services issued a notice of findings that Ireigbe abused and neglected a vulnerable adult while a licensed provider of an adult family home.

In November 2012 the Nursing Commission charged registered nurse Carol Ann Miller (RN00127854) with unprofessional conduct. She allegedly tested positive for drugs during a workplace drug test and didn't schedule a substance abuse evaluation.

In November 2012 the Nursing Assistant Program charged registered nursing assistant Tanya Sturm-Triano (NA60191028) with unprofessional conduct. She allegedly used cocaine and took about 200 hydrocodone tablets from residents for her own use.

In October 2012 the Dental Commission entered an agreed order with dentist April L. Ziegele (DE00007932) and placed conditions against her license. She didn't keep adequate treatment records, provided dental treatment that fell below the standard of care, and prescribed drugs to family members for non-dental purposes.

Skagit County

In October 2012 the Nursing Assistant Program charged certified nursing assistant Barbara R. Schwetz (NC10092448) with unprofessional conduct. She was convicted of second-degree theft, and two counts of identity theft.

Snohomish County

In November 2012 the Nursing Assistant Program charged certified nursing assistant Jongkunda Ad Ceesay (NC10075035) with unprofessional conduct. In October 2012, the Department of Social and Health Services issued a notice of findings that Ceesay abused a vulnerable adult while employed as a nursing assistant.

In November 2012 the Dental Commission charged dentist Robert Futhey Clark (DE00002494) with unprofessional conduct. He allegedly wrote a prescription for a controlled substance for a patient but the patient's records show no basis for the controlled substance.

Spokane County

In October 2012 the Emergency Medical Services and Trauma Systems Program charged emergency medical technician Jennifer S. Na (ES60219564) with unprofessional conduct. She was criminally charged with five counts of theft for stealing from her workplace.

In October 2012 the Emergency Medical Services and Trauma Systems Program charged emergency medical technician Trevor L. Veillette (ES01175229) with unprofessional conduct. He was convicted of third-degree child assault with sexual motivation and second-degree child assault.

Thurston County

In November 2012 the Medical Commission charged physician Chris N. Griffith (MD00022754) with unprofessional conduct. He allegedly left a sponge in a patient during a surgery because he didn't determine a correct sponge count at the time of the closure of the first incision. He also didn't chart a sponge count in his post-operative note.

In November 2012 the Nursing Assistant Program charged certified and registered nursing assistant Joshua Michael Lee Rancour (NC60259875, NA60245221) with unprofessional conduct. He allegedly had sex with a patient.

In October 2012 the Nursing Commission charged licensed practical nurse Beatrice Elizabeth Safford (LP00044634) with unprofessional conduct. She allegedly didn't comply with terms and conditions set against her license.

Whatcom County

In November 2012 the Chemical Dependency Professional Program granted the application of chemical dependency professional trainee Frank G. Laliberte (CO60304263) and placed his certification on probation for at least three years. In 2008, his credential to practice as a registered counselor was suspended for three years.

Yakima CountyIn November 2012 the Chemical Dependency Professional Program granted the application of chemical dependency professional trainee Manuel Armando Din, Jr. (CO60284889) and placed his certification on probation for three years. In 2010, he was convicted of two counts of manufacturing or possessing a dangerous weapon.

Out of State

Oregon: In November 2012 the Nursing Commission amended the statement of charges against registered nurse Suzanne G. Franklin (RN00142851). She voluntarily surrendered her license to practice as a registered nurse in Oregon.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
