This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is solicitation W91YU0-14-T-0099, issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions/clauses are those in effect through Federal Acquisition Circular 2005-72. The associated North American Industrial Classification System (NAICS) Code for this procurement is 423450 (Medical, Dental, and Hospital Equipment and Supplies Merchant Wholesalers), Size standard is 500 Employees. This RFQ is being issued as Firm Fixed Price, 100% Small-Business Set Aside. All responsible sources may submit a quotation, which will be considered by the agency. **INFORMATION AS AMENDED FOR THIS SOLICITATION DATED 21 APRIL 2014** It has been reported of electronic problems with vendor's unable to electronically respond to this solicitation. The following procedures are added for vendor response: a. An interested vendor may respond with an RFQ by email to the Contracting Officer, Point of contact as indicated below. b.
