Wisconsin releases enforcement actions for October

The Wisconsin Office of the Commissioner of Insurance recently released a list of administrative actions for October.

The actions are issued by OCI when licensing applicants or licensed individuals or companies have violated Wisconsin insurance statutes or regulations. In some cases, the individual or company denied the stated allegations but consented to the action taken.

Any forfeitures paid are deposited in Wisconsin’s Common School Fund administered by the Board of Commissioners of Public Lands. Earnings from the Fund are distributed to all public K-12 schools in the state and are used by school libraries to purchase materials such as books, newspapers, and computers.

Allegations and Actions Against Agents

Jonathan Ashmawi, 700 W. Hillsboro Blvd., Ste. 201, Deerfield Beach, FL 33441-1612, had his insurance license suspended for six months and was ordered to pay a forfeiture of $2,500.00. This action was taken based on repeated violations of insurance laws.

John Braem, 2618 W. Sunnyview Cir., Appleton, WI 54914, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Kimberley Campagna, 11200 Biscayne Blvd., Miami, FL 33181, was ordered to pay a forfeiture of $1,000.00. This action was taken based on an allegation of making a misrepresentation in the sale of an insurance product.

Timothy B. Cooper, 3424 Stony Creek Rd., Urbana, OH 43078, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to report an administrative action taken by another state and for violating a warning letter issued by OCI.

Jeffrey Cozzi, 3573 Deer Creek Palladian Cir., Deerfield Beach, FL 33442, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to report an administrative action taken by another state.

Manuel Escudero, 802 NW 87th Ave., Apt. 418, Miami, FL 33172, had his insurance license revoked. This action was taken based on allegations of failing to notify OCI of administrative actions taken by other states, not having an insurance license in his home state, and failing to respond to inquiries from OCI.

Amcora N. Hensley, 12603 Telge Rd., Apt. J-9, Cypress, TX 77429, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to disclose administrative actions taken by other states and violating a warning letter issued by OCI.

Ridley Jackson, P.O. Box 5051, Fort Lauderdale, FL 33310, agreed to surrender his insurance intermediary license. This action was taken based on allegations of failing to timely disclose administrative actions taken by other states, making misstatements on an application, and having a probationary license in his home state.

Tyler Jakoblich, W263N2020 E Fieldhack Dr., Unit 207, Pewaukee, WI 53072, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Robert Kirk, 701 Cork Xing, Cottage Grove, WI 53527, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Sara Makinen, 1427 Park Ave., Racine, WI 53403-2717, agreed to a three-year suspension of her Wisconsin insurance license. This action was taken based on allegations of authorizing the use of her NPN by other insurance agents to enroll customers in health insurance plans.

Justin M. Martin, W209N11340 Reger Ct., Germantown, WI 53022, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Christian Nwuju, 2575 Irving Ct., Apt. B, Omaha, NE 68131-1677, had her insurance license revoked. This action was taken based on allegations of failing to pay a previous forfeiture when due and other previous violations.

Joseph Ojukwu, 20110 Broad Run Ln., Richmond, TX 77407, had his application for an insurance intermediary license denied. This action was taken based on allegations of failing to disclose previous license denials and having criminal convictions that may be substantially related to insurance marketing.

Jorge M. Ordonez, 6911 Ian Ct., # 21, New Port Richey, FL 34653, had his application for an insurance intermediary license denied. This action was taken based on allegations of having administrative actions taken by other states, having insurance producer licenses suspended and revoked in other states, and improperly misappropriating monies received in the course of doing insurance business, and having been found to have committed fraud.

Dante Paone, 3450 Buschwood Park Dr., Ste. 200, Tampa, FL 33618, was ordered to pay a forfeiture of $1,500.00. This action was taken based on allegations of failing to disclose administrative actions in other states, failing to respond promptly to inquiries from OCI, and violating a warning letter previously issued by OCI.

Christopher I. Robards, 306 Bemner St., Starbuck, MN 56381, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Laura J. Sedivy, 133473 Soda Creek Rd., Marathon, WI 54448, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Shannon Shaw, 1400 Sunset Ave., Ste. 4, Waukegan, IL 60087, had her application for an insurance license denied. This action was taken based on allegations of having criminal convictions, which may be substantially related to the circumstances of holding an insurance license and providing incomplete documentation in her license application.

Miguel Solis, 2248 Sheridan Rd., Ste. #1, Zion, IL 60099, agreed to pay a forfeiture of $500.00 and to maintain an active public adjuster bond while registered in Wisconsin. This action was taken based on allegations of failing to maintain an active public adjuster bond in favor of the State of Wisconsin and failing to respond to inquiries from OCI.

Yevgeniya Stradling, 906 S. Henry Ln., Gilbert, AZ 85296, had her application for an insurance intermediary license denied for 60 days. This action was taken based on allegations of being permanently barred by the Financial Industry Regulatory Authority, having a restricted insurance license in California, having had a license application previously denied in Wisconsin, and misappropriation of consumer funds.

Allegations and Actions Against Companies

Advanced Health Insurance Agency LLC, 1770 NW 127th Way, Coral Springs, FL 33071, was denied an intermediary firm insurance license. This action was taken based on allegations of having administrative actions in other states.

Coalition Ins Solutions Inc., 548 Market St., #94729, San Francisco, CA 94104, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to report an administrative action taken by another state and violating a warning letter issued by OCI.

J D Allman Inc., 1730 I St., Ste. 240, Sacramento, CA 95811, was ordered to pay a forfeiture of $1,000.00. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

Allegations and Actions Against Other Entities

Areli Diaz Salgado, 504 E. Fulton Cir., Edgerton, WI 53534, was ordered to pay a forfeiture of $100.00. This action was taken based on evidence of the respondent committing a fraudulent insurance act by knowingly presenting a false claim for payment of a loss.

Cindy Kroening, 2210 Pond View Pl., Mosinee, WI 54455, was ordered to pay a forfeiture of $100.00. This action was taken based on evidence of the respondent committing a fraudulent insurance act by knowingly presenting a false claim for payment of a loss.

Joshua Lesley, 3500 S. 92nd St., Ste. 2C, Milwaukee, WI 53228, was ordered to pay a forfeiture of $6,000.00. This action was taken based on allegations of violating Wis. Stat. by engaging in fraud in the conduct of business, and by engaging in an impermissible conflict of interest.

Joshua P. Molle, W147 Franklin St., Freemont, WI 54940, was ordered to pay a forfeiture of $100.00. This action was taken based on evidence of the respondent committing a fraudulent insurance act by knowingly presenting a false claim for payment of a loss.

Tanya Ortiz, 206 Spring St., Somerset, WI 54025, was ordered to pay a forfeiture of $100.00. This action was taken based on evidence of the respondent committing a fraudulent insurance act by knowingly presenting a false claim for payment of a loss.
