Wisconsin releases enforcement actions for August
The Wisconsin Office of the Commissioner of Insurance recently released a list of administrative actions for August.
The actions are issued by OCI when licensing applicants or licensed individuals or companies have violated Wisconsin insurance statutes or regulations. In some cases, the individual or company denied the stated allegations but consented to the action taken.
Any forfeitures paid are deposited in Wisconsin’s Common School Fund administered by the Board of Commissioners of Public Lands. Earnings from the Fund are distributed to all public K-12 schools in the state and are used by school libraries to purchase materials such as books, newspapers, and computers.
Allegations and Actions Against Agents
Alexander Cook, 5930 Coral Lake Dr., Margate, FL 33063, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to report an administrative action taken by another state and for violating a warning letter issued by OCI.
Eric Escalante, 9701 Yellow Stone Pl., Tampa, FL 33634, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to report an administrative action taken by another state and for violating a warning letter issued by OCI.
Douglas Eze, Largo Financial Services Inc., 6401 Golden Triangle Dr., Ste. 115, Greenbelt, MD 20770, agreed to voluntarily surrender his intermediary license. This action was taken based on allegations of failing to timely notify OCI of administrative actions taken by other states, failing to timely notify OCI of pending criminal matters, and having a criminal conviction substantially related to the circumstances of holding an insurance license.
Joseph Harrings, W70N447 Cedar Pointe Ave., Cedarburg, WI 53012, was ordered to pay a forfeiture of $1,000.00. This action was taken based on allegations of misleading a consumer regarding property insurance coverage and failing to respond to inquiries from OCI.
Allison Hejdak, 48 Rushmore Ln., Hartford, WI 53027, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Sharma Leblanc Saintval, 9715 W. Broward Blvd., Ste. 200, Plantation, FL 33324, was ordered to pay a forfeiture of $1,000.00. This action was taken based on an allegation of making a misrepresentation in the sale of an insurance product.
Edson Leondas, 7031 SW 26th Ct., Miramar, FL 33023, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to report administrative actions taken by other states.
Devin Lopez, 925 NW 3rd Ave., Apt. 1, Fort Lauderdale, FL 33311, had his insurance license revoked. This action was taken based on allegations of not having an insurance license in his home state, failing to report administrative actions taken by other states, and failing to participate as ordered in an administrative hearing.
Jacob L. Mathis, 11974 Lorenza Ln., Orlando, FL 32827-7800, had his Wisconsin non-resident insurance intermediary license revoked. This action was taken based on allegations of failing to notify OCI of disciplinary actions taken by other states and failing to participate in the hearing process.
Traci Onsgard, P.O. Box 62, La Crosse, WI 54602, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to notify OCI of initial pretrial hearing dates related to pending criminal charges against her and failing to respond to inquiries by OCI.
Breesean A. Pittman, 8301 Mansion Hill Ave., Apt. 3, Madison, WI 53719, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Sarah Schumaker, 9532 N. Paradise Rd., Kewaskum, WI 53040, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Andres Toro, 384 6th Ave., Salt Lake City, UT 84103, was ordered to pay a forfeiture in the amount of $500.00. This action was taken based on allegations of failing to timely report an administrative action taken by another state.

Rochelle Walgenbach, 1324 Bluebird Ct., Kaukauna, WI 54130, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Tabby Watt, 18370 Cocos Plumosa St., Jupiter, FL 33458, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely report an administrative action taken by another state.

Clear Solutions Ins LLC, 1600 E. 8th Ave., Ste. A200, Tampa, FL 33605, was ordered to pay a forfeiture of $1,000.00. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.
Jackson National Life Insurance Company, 1 Corporate Way, Lansing, MI 48951, was assessed a forfeiture in the amount of $1,000. This action was taken based on allegations of failing to pay a required fee to WISF.
Oceanview Life and Annuity Company, 1819 Wazee St., 2nd Fl., Denver, CO 80202, was assessed a forfeiture in the amount of $1,000.00. This action was taken based on allegations of failing to pay a required fee to WISF.
