The purpose of this Sources Sought is to conduct market research to identify responsible firms capable of providing community nursing home (CNH) services within the VA Northern California Health Care System s (VANCHCS) catchment area. Nursing home care shall meet the total medical, nursing, and psychosocial needs of VA beneficiaries and must:Have current Center for Medicare and Medicaid Services certification.

Be licensed by the State of California

The VANCHCS serves the following California Counties: Alameda, Amador, Butte, Colusa, Contra Costa, El Dorado, Glenn, Placer, Sacramento, Shasta, Siskiyou, Solano, Sutter, Tehama, Trinity, Yolo, and Yuba.

The proposed NAICS Code is 623110, Nursing Care Facilities (Skilled Nursing Facilities) the corresponding small business size is $27.5 Million.

This is a Sources Sought Notice. It is solely for information and planning purposes only and does not constitute a solicitation. A request for quotes/proposal may be issued at a later date.

Interested firms shall respond to this Sources Sought no later than 4:00pm (Pacific), Friday, November 10, 2017. Send all replies to Tracy Worrell at [email protected]

Responses shall include:

Business Name and Address

Point of contact Name, Phone Number, and E-mail Address

DUNS and NAICS code

Business Size (small or large) and Business type (Veteran owned, HUB Zone, Woman-owned, etc.)

Pamphlet with information about nursing home (location, type of care provided, etc.). The pamphlet shall not exceed 2MB or 5 pages.

Answers to the following:

Is your establishment licensed by the state

Is your establishment certified by the Center for Medicare and Medicaid Services

What is the firm s level of experience with case-mix reimbursement under a Federal rate structure (e.g., Resource Utilization Group)

Does the establishment meet all state requirements for level of medical liability insurance

Is the establishment compliant with the requirements of 42 CFR 483.35

Has a VA CNH Review team has ever conducted an assessment of the establishment

Has a VA CNH Review team ever made a recommendation to exclude or terminate the establishment from participation in the CNH Program

All businesses capable businesses are encouraged to reply as this information may be used to determine potential set-asides for the above noted service.
