SUBJECT: Solicitation Number 19AJ2021R0001 Life Insurance for Local Staff of the U.S. Embassy Baku, Azerbaijan Dear Prospective Offeror: The Embassy of the United States of America invites you to submit a proposal for customized Life Insurance for Local Staff of the U.S. Embassy Baku, Azerbaijan. The Embassy intends to conduct a pre-proposal conference, and all prospective offerors who have received a solicitation package are invited to attend. See Section L of the attached Request for Proposals (RFP). Please email your proposal only to [email protected] with a copy to [email protected] on or before 18:00 (6:00PM) local (Baku) time March 15, 2021. No proposal will be accepted after this time. In order for a proposal to be considered, you must also complete and submit the following: 1. SF-33 (Blocks 15A, 16, 15B, 17, 18) 2. Section B, Pricing Schedule 3. Section B, Retention Amounts in B.3 and B.7 4. Section K, Representations and Certifications; 5. Additional information as required in Section L. 6. The Offeror shall submit a completed IRS Form W-14, found at www.irs.gov/w14, to ensure compliance with FAR 52.229-11. Please put RFP number 19AJ2021R0001 in the subject line for all emails related to this RFP. When you ask questions, please do not create new emails but continue the existing correspondence. The title of the email with your proposal must state solicitation number 19AJ2021R0001. Please do not compress your quotations using ZIP, RAR or any other software. Also do not send video files, the U.S. Embassy servers will not accept these files. Please note: Proposals that contain more benefits (even if there is no increase in cost) or fewer benefits than those stated in the solicitation may be deemed technically unacceptable. The U.S. Government intends to award a contract to the responsible company submitting an acceptable offer at the lowest price. We intend to award a contract based on initial proposals, without holding discussions, although we may hold discussions with companies in the competitive range if there is a need to do so.
