COMBINED SYNOPSIS/SOLICITATION1. This is a Combined Synopsis/Solicitation for Commercial Items prepared in accordance with the format in FAR Subpart 12.6 - Streamlined Procedures for Evaluation and Solicitation for commercial items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number VA119-16-R-0008 is being issued as a Request for Proposal (RFP). This notice and the incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 05-73 dated
2. The associated North American Industrial Classification System (NAICS) code for this procurement is 524128. This procurement action is unrestricted.

4. Description of Services: Veterinary-care benefits are required for two groups of dogs. . (Group 1) consists of veteran-owned dogs, also referred to as "Service Dogs." These Service Dogs shall be defined as Guide or Service Dogs prescribed for a disabled Veteran pursuant to 38 Code of Federal Regulations (CFR) 17.148. Service Dogs are privately owned by veterans suffering from visual, hearing and/or substantial mobility impairments. Pursuant to 38 CFR 17.148, the contractor shall provide veterinary health insurance coverage, which includes both a comprehensive wellness and health plan as well as sick/injured insurance coverage for all Dogs screened and approved by
5. The government contemplates an Award of a Firm Fixed Price Indefinite Delivery Indefinite Quantity (IDIQ) contract in accordance with FAR Part 16.504(a) resulting from this solicitation. The anticipated period of performance includes a one (1) year base period and four (4) one year option periods.
6. Offerors must submit representations and certification in accordance with provision at FAR 52.212-3 Offeror Representations and Certifications-Commercial Item.
