State Disciplines Health Care Providers

OLYMPIA, Wash., June 27 -- The Washington State Department of Health issued the following news release:

The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department's Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency website. Click on "Look up a healthcare provider license" in the "How Do I?" section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider's license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Clark County

In May 2013 the Nursing Assistant Program charged certified nursing assistant Alisha Lahela Miner (NC60300557) with unprofessional conduct. Miner allegedly stole about $300 from a resident at a facility where she worked. The Department of Social and Health Services in a preliminary finding determined she financially exploited a vulnerable adult.

Cowlitz County

In May 2013 the Nursing Assistant Program charged registered nursing assistant Lesa A. Newt (NA00149395) with unprofessional conduct. The Department of Social and Health Services found she financially exploited a vulnerable adult at an assisted living facility where she worked. She is not allowed to be employed in a job with access to vulnerable adults.

In May 2013 the Nursing Assistant Program ended probation for registered nursing assistant Brandi LeeAnn Wasson (NA60208954).

Douglas County

In May 2013 the Nursing Commission charged registered nurse Stephanie Michelle Scott (RN00172953) with unprofessional conduct. A hospital where Scott worked discharged her when she allegedly refused to take a drug test after being questioned about missing narcotics. A review later showed 88 medications that were unaccounted for had been dispensed to Scott.

Grays Harbor County

In May 2013 the Department of Health granted a home care aide credential to Angel Dove Chilcott (HM60288795), who was convicted of possession of methamphetamine in 2009.

King County

In May 2013 the Unlicensed Practice Program notified Wenjing Feng of its intent to issue a cease-and-desist order. Feng, who has no credential, allegedly massaged a client's feet, calves, and lower thighs.

In May 2013 the Nursing Assistant Program charged certified nursing assistant Fantanesh H. Terara (NC10087040) with unprofessional conduct. Terara allegedly didn't comply with a previous stipulation.

In May 2013 the Nursing Assistant Program conditionally granted a certified nursing assistant credential to Michelle Ellen Barnett (NC60344736). Barnett conditionally received a health care assistant credential in 2012.

In May 2013 the Nursing Assistant Program charged registered nursing assistant Andreas Rayford (NA00149531) with unprofessional conduct. Rayford allegedly left a vulnerable adult in a company van when he went home, while working for a supported living agency. The Department of Social and Health Services will not allow Rayford to be employed in a job with access to vulnerable adults.

In May 2013 the Hearing and Speech Board suspended the hearing instrument fitter and dispenser credential of Cheryl A. Conway (HA00000779) for 10 years. Conway falsified records and kept incomplete records. She didn't give her employer money it was due for equipment and services.

Pierce County

In May 2013 the Pharmacy Board charged pharmacy assistant and pharmacy technician Christa Nelson (VB60153239, VA60245078) with unprofessional conduct. Nelson was charged with one count of residential burglary, three counts of second-degree identity theft, and one count of unlawful possession of a payment instrument. Nelson allegedly admitted entering an elderly neighbor's home after an ambulance took the neighbor to a hospital; she allegedly also admitted taking checks, forging, and cashing them for a total of $1,020.

In May 2013 the Medical Commission charged physician David A. Davidson (MD00035868) with unprofessional conduct. In 2012 Davidson was charged with two counts of first-degree possession of depictions of minors engaged in sexually explicit conduct. He pleaded guilty to one count of second-degree computer trespass.

In May 2013 the Nursing Assistant Program charged certified nursing assistant Earnestine Neal (NC10022228) with unprofessional conduct. Neal allegedly didn't provide care to a patient at a skilled nursing facility where she worked. The patient was found soaked in urine after being unattended for seven hours. Neal also allegedly falsified records to indicate she had provided care for the patient.

In May 2013 the Nursing Assistant Program charged certified nursing assistant Chelsia C. Morrison (NC10095743) with unprofessional conduct. In 2013 Morrison was convicted of fourth-degree assault.

In May 2013 the Nursing Commission charged registered nurse Sarah M. Bain (RN00161217) with unprofessional conduct. Bain allegedly didn't comply with a previous agreement.

In May 2013 the Nursing Assistant Program released certified nursing assistant Alena D. Alves (NC10045511) from conditions of a previous order. She may practice without restrictions.

Snohomish County

In May 2013 the Nursing Assistant Program charged registered nursing assistant Tijan Cham (NA60059721) with unprofessional conduct. Cham worked in a facility with three vulnerable adults who required 24-hour support. He allegedly left them alone when he went to a store to buy headache medicine, and his absence was extended when his vehicle got a flat tire. The Department of Social and Health Services will not allow him to be employed in a job with access to vulnerable adults.

In May 2013 the Nursing Assistant Program charged registered nursing assistant Ebrima Dibba (NA00173030) with unprofessional conduct. Dibba allegedly yelled and repeatedly swore at a patient in an assisted living facility where he worked. The Department of Social and Health Services will not allow Dibba to be employed in a job with access to vulnerable adults.

In May 2013 the Nursing Assistant Program charged certified nursing assistant Serena Marie Olson (NC10022849) with unprofessional conduct. In 2013 Olson was allegedly in a patient's home when the patient went to a hospital. When the patient returned home, more than 20 valium tablets were missing. Olson allegedly tested positive for methamphetamine, amphetamine, and marijuana. In 2012, after an arrest in connection with shoplifting, Olson was convicted of third-degree theft. Olson also allegedly didn't report the conviction to the Department of Health as law requires.

Thurston County

In May 2013 the Nursing Assistant Program charged registered nursing assistant Tracy Marie Hill (NA60252834) with unprofessional conduct. In 2013 Hill's health care assistant credential (HC00149317) was indefinitely suspended.

Whatcom County

In May 2013 the Nursing Assistant Program charged registered nursing assistant Greg Alan Horch (NA60127685) with unprofessional conduct. Horch appeared to be impaired at work. He allegedly admitted having a substance abuse problem and that he'd diverted hydrocodone from a patient. The Department of Social and Health Services will not allow him to be employed in a job with access to vulnerable adults.

In May 2013 the Dental Hygienist Program charged dental hygienist Anita Lee Rodriguez (DH00001530) with unprofessional conduct. Rodriguez allegedly performed services outside the scope of her credential.

Whitman County

In May 2013 the Nursing Commission charged registered nurse Eldon B. Hodges (RN00136232) with unprofessional conduct. Hodges allegedly violated a substance abuse monitoring contract by testing positive for morphine. He'd agreed to the contract after admitting to diverting drugs for his own use from the hospital where he worked.

Yakima County

In May 2013 the secretary of health granted a denturist credential to Isrrael Felix Cardenas (DN60047254) and placed him on probation for at least two years. In 2008 Cardenas was convicted of communication with a minor for immoral purposes.

In May 2013 the Nursing Assistant Program charged registered nursing assistant Mayra C. Sanchez (NA00183257) with unprofessional conduct. In 2011 Sanchez was convicted of first-degree criminal trespass and making a false or misleading statement to a public servant. In response to a police officer's inquiry, Sanchez took $50 from her pocket, gave it to the officer, and allegedly said she had found it in the room of a patient at an assisted living facility where she worked. She later admitted that she had stolen the money. The Department of Social and Health Services will not allow her to be employed in a job with access to vulnerable adults.

In May 2013 the Nursing Assistant Program charged registered nursing assistant Karan L. Jensen (NA00123341) with unprofessional conduct. Jensen allegedly didn't comply with a previous stipulation.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
