State Disciplines Health Care Providers

OLYMPIA, Wash., Jan. 14 -- The Washington state Department of Health issued the following news release:

The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department's Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency website. Click on "Look up a healthcare provider license" in the "How Do I?" section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider's license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Chelan County

In November 2012 the Chiropractic Commission modified the 2010 agreed order against chiropractor Jason Schroeder (CH00003389) and placed him on monitoring for at least three years.

Clark County

In December 2012 the Nursing Assistant Program entered into an agreed order with certified nursing assistant Angela P. Winther (NC10073267) and placed her certification on probation for two years. She was convicted of possession of a controlled substance with intent to deliver -- methamphetamine and possession of a controlled substance with intent to deliver -- marijuana. She also tested positive for methamphetamine and marijuana following a workplace drug test.

Cowlitz County

In December 2012 the Emergency Medical Services and Trauma Systems Program charged paramedic Stephen W. Pitts (ES00113043) with unprofessional conduct. He allegedly submitted knowledge and skill evaluations for his required training sessions that he didn't attend.

Franklin County

In December 2012Board of Pharmacy charged pharmacy assistant Francisco J. Leyva-Gonzalez (VB60090180) with unprofessional conduct. Charges say he admitted to diverting controlled substances from his workplace.

Grant County

In December 2012 the Nursing Assistant Program denied the reactivation application of certified nursing assistant Jessica L. Grib (NC10073201). She was convicted of possessing stolen property.

King County

In December 2012 the Agency Affiliated Counselor granted the application of Joseph Andrew Black (CG60311330) and placed his registration on probation. He was convicted of vehicular assault, violation of the Uniform Controlled Substance Act -- delivery of cocaine, and five counts of violation of the Uniform Controlled Substance Act -- possession of cocaine.

In December 2012 the Dental Commission charged dentist Gary Duane Bogachus (DE00004187) with unprofessional conduct. Charges say he provided care that fell below the standard by not diagnosing tooth decay on a patient.

In December 2012 the Nursing Commission entered into an agreed order with registered nurse Zenaida Robancho Caoagdan (RN00100411) and placed her license on probation for one year. She worked as a registered nurse while her license was expired.

In November 2012 the Nursing Assistant Program ended the order of probation against registered and certified nursing assistant Amy Elizabeth Peterson (NA60166582, NC60199177).

In November 2012 the Health Care Assistant Program denied the application of Linda Aracely Tamat (HC60274873). She was convicted of burglary and didn't respond to the departments offer to keep her license with conditions.

Kitsap County

In November 2012 the Medical Commission charged physician Narinder M. Duggal (MD00036603) with unprofessional conduct. Allegations include not performing adequate testing for diagnosis, not obtaining records of prior treatments, prescribing medications without medical justification for medication type or dosage amounts, and making inappropriate sexual advances toward a patient.

Okanogan County

In October 2012 the Nursing Assistant Program denied the application of registered nursing assistant applicant Lisa R. Rieb (NA60304536). She answered yes to a question on the application related to use of chemical substances and didn't respond when offered a registration with conditions.

Pierce County

In December 2012 the Nursing Commission charged registered nurse Carol Jean Davis (RN00109898) with being unable to practice with reasonable skill and safety by reason of a mental or physical condition.

In December 2012 the Nursing Commission charged licensed practical nurse Stephanie L. Henzel (LP00036991) with unprofessional conduct. She was convicted of obtaining a controlled substance by fraud or forged prescription, theft, identity theft, and attempt to obtain methadone by means of a false and forged prescription.

In November 2012 the Examining Board of Psychology charged Sunil K. Kakar (PY00003098) with unprofessional conduct. Charges say he misused drugs and alcohol.

In November 2012 the Unlicensed Practice Program notified Yanfang Kong of its intent to issue a cease-and-desist order. He allegedly provided a massage to a client and isn't licensed as a massage therapist in Washington.

In November 2012 the Nursing Assistant Program charged registered and certified nursing assistant Elizabeth Taeao Leaeno-Walter (NA60088665, NC60183051) with unprofessional conduct. Allegations include transferring a patient without using a two-person transfer as required in the patient's care plan - resulting in the patient's injury; using inappropriate, profane and abusive language; and striking a patient.

In November 2012 the Nursing Assistant Program released certified nursing assistant Emmanuel L. Lukudu (NC10071376) from terms and conditions set against his certification.

Snohomish County

In December 2012 the Social Worker Program granted the application of independent clinical social worker associate Guy Brent Ashmore (SC60305097) and placed his license on probation. His registration to practice as a registered counselor was suspended in 2006.

In November 2012 the Nursing Commission charged registered nurse Gabrielle Eleanor Camarot (RN00110401) with unprofessional conduct. Charges say she didn't administer or document wastage of medications, created a medication or documentation error when administering medication, and didn't comply with a substance abuse monitoring program.

Spokane County

In November 2012 the Nursing Commission amended the statement of charges against registered nurse and advanced registered nurse practitioner John V. Billings (RN00127908, AP30004240). His treatment, diagnosis, documentation, and prescribing practices allegedly fell below the standard when caring for patients.

In December 2012 the Nursing Commission entered into an agreed order with registered nurse Kaarene D. Parker (RN00130561) and placed her license on probation for six months. She worked as a registered nurse while her license was expired.

In November 2012 the Nursing Assistant Program charged registered nursing assistant Angela Hope McClain (NA00170193) with unprofessional conduct. Charges say she didn't give a patient's medication to the patient, later giving the patient the wrong medication in a bottle with a computer-generated label she made herself.

In November 2012 the Chiropractic Commission charged chiropractor Stephen Frank Renner (CH00001206) with unprofessional conduct. Charges say he didn't comply with terms and conditions set against his license.

Thurston County

In November 2012 the Chiropractic Commission charged chiropractor Matthew W. Martinez (CH60191433) with unprofessional conduct. Allegations include billing for services not provided and representing himself as a licensed massage practitioner when he doesn't hold an active massage practitioner credential in Washington.

Whatcom County

In December 2012 the Chemical Dependency Professional Program granted the application of chemical dependency professional trainee Kelly Ryan Frost (CO60312416) and placed conditions against her certification. She was convicted of assault -- domestic violence, and malicious mischief.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
