State Disciplines Health Care Providers

OLYMPIA, Wash., Dec. 21 -- The Washington state Department of Health issued the following news release:

The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department's Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency website. Click on "Look up a healthcare provider license" in the "How Do I?" section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider's license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Clark County

In November 2012 the Chemical Dependency Professional Program granted the application of chemical dependency professional trainee Scott Alan Kleinschmidt (CO60306513) and placed his license on probation. He was convicted of attempted burglary, two counts of unlawful possession of heroin, and residential burglary.

Grant County

In November 2012 the Nursing Assistant Program charged certified nursing assistant Jessica L. Grib (NC10073201) with unprofessional conduct. She was convicted of second degree possession of stolen property.

In November 2012 the Board of Osteopathic Medicine and Surgery amended charges against osteopathic physician Allen Quinn (OP00001193). Allegations include not meeting the standard of care in managing chronic pain patients, not retaining patient treatment records, and treating family members and himself.

Grays Harbor County

In November 2012 the Nursing Commission charged registered nurse and advanced registered nurse practitioner Kathryn A. Lewis (RN00077331, AP30004525, AP60003639) with unprofessional conduct. The care she provided during a home birth allegedly fell below the standard when assessing the patient's due date, not performing prenatal testing, inadequate monitoring of the fetal heartbeat during labor, not being prepared for newborn resuscitation, and not contacting emergency services in a timely manner.

Jefferson County

In November 2012 the Nursing Assistant and Emergency Medical Services and Trauma Systems Programs ended the probation order against registered nursing assistant and emergency medical technician Jeffrey Bruce Fairbanks (NA60164724, ES60142479).

King County

In October 2012 the Chemical Dependency Professional Program granted the application of chemical dependency professional trainee Matthew Bennet Ellefson (CO60300965) and placed his certification on probation. He was convicted of four counts of theft, three counts of possessing stolen property, burglary, forgery, and unlawful possession of a stolen vehicle.

In November 2012 the Chiropractic Commission released chiropractor Dawn M. Hunter (CH00034116) from terms and conditions set against her license in 2007. She must continue to comply with the 2012 order against her license.

In November 2012 the Nursing Assistant Program ended the probation order against certified nursing assistant Amenan Natacha Kouakou (NC60260508).

In November 2012 the Dental Commission charged dentist Cynthia Leu-Yeh (DE00007993) with unprofessional conduct. The dental treatment, and prescription and billing practices she provided to patients allegedly fell below the standard.

In November 2012 the Nursing Assistant Program amended charges against registered and certified nursing assistant Teresa Margaret Trudeau (NA60052315, NC60134420). She violated a protection order -- vulnerable adult on multiple occasions and is in contempt of court.

In November 2012 the Nursing Assistant Program charged registered and certified nursing assistant Karen Sue Williams (NA00194766, NC60146385) with unprofessional conduct. She allegedly moved into a client's home and borrowed $50 from the client which she never repaid.

In November 2012 the Surgical Technologist Program charged Ken R. Welling (ST00000019) with unprofessional conduct. He was convicted of seven counts of theft.

Kitsap County

In November 2012 the Chiropractic Commission charged chiropractor Blake J. Wagner (CH00003095) with unprofessional conduct. He allegedly asked a patient to loan him $600 on one or more occasions.

Lewis County

In November 2012 the Nursing Assistant Program charged certified nursing assistant Brandee L. Leal-Lopez (NC10064144) with unprofessional conduct. She allegedly entered her neighbor's home without permission, broke into a safe, stole a pack of cigarettes, and ingested Vyvanse, a controlled substance from the neighbor's supply.

Pacific County

In November 2012 the Midwifery Program placed the license of Tamra K. Roloff (MW00000283) on probation for four years. The midwifery treatment she provided fell below the standard in charting, assessing maternal vital signs, not transferring the patient to a hospital, not addressing the failure of labor to progress, not administering magnesium sulfate when the patient suffered her first and second seizure, not monitoring fetal heart tones, and not providing records to the hospital in a timely way. The care she provided fell below the standard and resulted in injury and created unreasonable risk of harm.

Pierce County

In November 2012 the Emergency Medical Services and Trauma Systems Program charged paramedic Gareth William Gabb (ES60112453) with unprofessional conduct. He allegedly didn't comply with terms and conditions set against his certification.

In November 2012 the Nursing Commission charged registered nurse Laura Caldwell Karlin (RN00149864) with unprofessional conduct. She allegedly practiced as a registered nurse without a valid credential.

In November 2012 the Board of Pharmacy ended the probation order against pharmacy assistant Molly A. Komiti (VB60114509).

In November 2012 the Nursing Commission charged licensed practical nurse Joanne H. White (LP00045333) with unprofessional conduct. She allegedly didn't comply with terms and conditions set against her license.

Snohomish County

In November 2012 the Medical Commission placed the license of physician Ella Y. Bostanjian (MD60039274) on probation. She didn't comply with prior conditions set against her license.

In November 2012 the Massage Practitioner Program amended charges against Amenia Renfro-Mandregan (MA00025325). She allegedly failed to comply with terms and conditions set against her license.

Spokane County

In November 2012 the Board of Physical Therapy placed the license of Jeffrey Randall Waters (PT00005489) on probation for two years. He charted and billed for treatments he didn't provide.

Thurston County

In November 2012 the Unlicensed Practice Program notified Tricha Baker of its intent to issue a cease-and-desist order. She allegedly performed veterinary technician duties but doesn't hold a license to practice as a veterinary technician.

In November 2012 the Chiropractic Commission ended the probation order against chiropractor Marvin Kunikiyo (CH00002656).

In November 2012 the Chemical Dependency Professional Program denied the application of chemical dependency professional trainee applicant Kimberly Arlandis Patterson (CO60302151). She was convicted of tampering with a witness -- domestic violence, two counts of violation of no contact -- domestic violence, violation of post conviction no contact order -- domestic violence, and two counts of driving while license suspended.

Yakima County

In November 2012 the Nursing Assistant Program charged registered nursing assistant Dakota Ryan Jensen (NA60237709) with unprofessional conduct. She allegedly stole Oxycodone, a controlled substance, from a patient.

Out of State

Texas: In November 2012 the Nursing Commission charged registered nurse Karen Y. Schumann (RN60156559) with unprofessional conduct. Her license to practice as a registered nurse in Montana was suspended.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
