Statement of Work/Statement of Objective1. Background. Veterans Health Administration (VHA) established an Employee Health Promotion/Disease and Impairment Prevention (EHPDIP) program in Fiscal Year (FY) 2008 as a Veteran Affairs Central Office (VACO) field-based office located in Minneapolis. This program is part of, and funded by, the Occupational Health Strategic Healthcare Group (OHSHG), Public Health (10P3), VACO. EHPDIP is tasked with planning, developing and implementing a comprehensive workplace program for employee health promotion. EHPDIP develops tools and resources for field use, provides guidance to the field, trains VHA employee health promotion coaches, and evaluates all program components for feasibility and efficacy. It has been well documented that comprehensive, strategically designed investment in employees' social, mental, and physical health through employee health promotion programs provide a significant return on investment. Traditionally ROI has been seen as a major predictor of wellness program success. Data required for ROI predictive models has included insurance data such as risk, utilization, and claims. This data is not available to VHA due to the way Federal Employee Health Benefit constructs its database across all Federal agencies without dividing by individual agency. Due to this inability to access individual agency data from the program it is important to develop a model based on statistically significant and peer reviewed default values. Without the ability to accurately predict default values based on comparable peer reviewed studies it is impossible to access the effectiveness of a VHA wellness program with data that is pulled from a multitude of federal agencies.

The EHPDIP program requires an independent automatic financial and health benefit tool that will determine the benefits of its health promotion program. This tool shall provide a snapshot of current aggregate employee health status, the most significant modifiable health risks, and the ability to provide estimates of cost savings and cost avoidance across multiple scenarios. This information will be used to analyze the cost drivers and adjust EHPDIP program strategies to address the specific needs of the employee population.

2. Scope. The contractor shall deliver to the Government (VA Medical Center):

a. Comprehensive, customizable, database assessment tool that will provide financial and health benefits for the program under multiple user defined scenarios

b. Financial benefits to be calculated based on the following inputs:

" Health risks (i.e. from a health risk assessment)

" Demographic characteristics (including age, sex, geographic location, and race,).

" Cost of risk reduction programs

c. Outputs will include, at a minimum

" Cost saving

" Cost avoidance

" Identification of modifiable health risk that has the most impact

d. Train-the-trainer orientations to educate and support the use of this tool. Approximately 8-10 hours of training for 3 people over 2 sessions.

e. Customization to include default values for unavailable information that are peer reviewed and statistically valid

f. Data analysis and technical support. Report generation will be completed by the VA.

g. The contractor shall deliver the customizable modeling tool to measure the cost effectiveness of the EHPDIP program through a variety of user specified scenarios

h. The vendor will provide a tool that is customizable and user defined

3. Objectives.

a. The EHPDIP program wishes to conduct internal, self-evaluations of VHA health promotion programs across the country in order to validate effective programs, improve programs that are less than optimal and provide an efficient means of selecting programs for continued funding.

b. VHA will assist with the collection of available data and the review of default data when there is no available data that is required for statistical validity.

c. VHA will use its findings to improve or create new programs, efforts, initiatives, and offerings to its employees for the purpose of educating and improving their overall well-being. This supports the Presidential Memorandum-Enhancing Work-Life Program including Employee Health Promotion.

d. Telephone meetings will be held as needed by the contractor or Government.

2. Delivery.

a. Contractor shall provide EHPDIP with a financial and health benefit model that is customizable to program requirements at the VHA user level.

b. All technical, programming and design work will be completed by October 1st 2015.

c. Delivery location: Access to the tool will be granted to the Director, EHPDIP and other designees as determined by the VHA.

d. Receipt documentation: Confirmation of delivery notice will be provided to vendor upon receipt by VHA.

e. Design, implement and analyze baseline and post test measures for pre-post comparisons.

3. Government Furnished Property. The vendor will utilize their own computers and software to develop the ROI model.

4. Security.

a. If proposing an online application versus a Microsoft Excel (or similar) based program, vendor must adhere to VHA security policy regarding online applications.

b. Vendor must adhere to the highest standards of confidentiality of data, protection and safety of physical equipment, servers, hosting facilities and all other such systems.

5. Place of Performance.

a. Product will be delivered either electronically or via CD/DVD to the VHA EHPDIP program.

b. There will be training for EHPDIP on the use of this tool either face to face, online or through conference calls. Approximately 8-10 hours of training for 3 people over 2 sessions.
