Through the collaboration, clinicians at the participating ACOs will be able to track prescription activity and usage for the SilverScript beneficiaries they treat, including the use of generic versus brand prescriptions and the identification of any medication adherence issues or gaps in care. Based on this information, we will work together to develop and implement appropriate interventions.
As a Medicare Part D PDP sponsor, SilverScript implements a Medication Therapy Management (MTM) program that includes Comprehensive Medication Reviews and other adherence and gaps in care programs that enhance the coordination of pharmacy care for beneficiaries. Through these programs, SilverScript beneficiaries are counseled individually about maintenance medications, patient concerns and any potential risks. These programs are designed to identify concerns around adherence to maintenance medications and possible risks, such as drug-to-drug interactions, duplicate therapies or high risk medications.
