“Rocky Mountain Health Plans and
New! Well-Being Connect
Healthways Well-Being Connect is an online personal behavior change portal through which RMHP Members can better manage their health. Beginning in
- Understand and explore their own well-being while connecting with peers and health professionals to pursue shared goals.
- Utilize personal well-being plans and other online tools to create and strengthen healthy behaviors through a combination of leading consumer engagement strategies and deeply embedded behavior change science.
RMHP Commercial Members living with heart failure, coronary artery disease and diabetes will continue to have access to Healthways Care Support interventions. The interventions use evidence-based approaches and one-on-one telephonic interactions with nurses and other clinicians to empower patients to adhere to their physician’s plan of care and successfully manage their illness, improving quality of care and reducing costs. RMHP will also implement a high-risk care management registry.
Members will also continue to have access to myhealthIQ, featuring an online health risk assessment, biometric data, personalized follow-up and participation incentives.
