Rocky Mountain Health Plans, Healthways Announce Long-Term Extension of Total Population Health Solutions

Extended Agreement Provides Continued Health and Care Support Solutions to RMHP Members; Adds Access to Well-Being Connect™

GRAND JUNCTION, Colo. & NASHVILLE, Tenn.--(BUSINESS WIRE)-- Rocky Mountain Health Plans (RMHP) and Healthways, Inc. (NASDAQ: HWAY) today agreed to a five-year extension of their partnership offering Healthways total population health management, including care support solutions to RMHP Members.

The long-term extension confirms the effectiveness of Healthways programs in helping RMHP Members understand their level of well-being, improve their health status and access individualized support to manage chronic illnesses. The extended agreement includes the expansion of services by providing Healthways Well-Being Connect™, an online behavior change portal.

New! Well-Being Connect

Healthways Well-Being Connect is an online personal behavior change portal through which RMHP Members can better manage their health. Beginning in July 2012, through Well-Being Connect, RMHP Members can:

  • Understand and explore their own well-being while connecting with peers and health professionals to pursue shared goals.
  • Utilize personal well-being plans and other online tools to create and strengthen healthy behaviors through a combination of leading consumer engagement strategies and deeply embedded behavior change science.

RMHP Commercial Members living with heart failure, coronary artery disease and diabetes will continue to have access to Healthways Care Support interventions. The interventions use evidence-based approaches and one-on-one telephonic interactions with nurses and other clinicians to empower patients to adhere to their physician’s plan of care and successfully manage their illness, improving quality of care and reducing costs. RMHP will also implement a high-risk care management registry.

Members will also continue to have access to myhealthIQ, featuring an online health risk assessment, biometric data, personalized follow-up and participation incentives.
