The Fargo Veterans Affairs Health Care System plans to issue a Request for Proposal (RFP) in accordance with Public Law (Pub. L.) 104-262, The Veterans Health Care Eligibility Reform Act of 1996, and Title 38 United States Code (USC) 8151 Enhanced Health Care Resources for professional autopsy examination services conducted by pathologists certified and/or license in anatomic pathology. Services will be conducted on an as needed basis within the lawful provision required for autopsy services, additionally; the contractor shall provide a suitable facility for the performance of the services described within the Performance Work Statement (PWS). All services are to be provided consistent with the community standard of care, Veterans Affairs clinical guidelines, and pertinent Veterans Affairs performance measures to monitor timeliness of services and thoroughness to ensure good post mortem care is completed within prescribed terms for this service. The Fargo Veterans Affairs (VA) Health Care System will be responsible for the transportation to ensure (maintain) the disposition of body - chain of receipts.

The North American Industry Classification System Code (NAICS) is 621399 and the small business size standard is $7.0 million (USD). The solicitation will be unrestricted, identified with solicitation number for this Request for Proposal (RFP) is VA-263-12-R-0032, and will be issued on or about November 20, 2011. This proposed contract will be a firm fix-price per procedure. Potential offerors are responsible for downloading all documents, including amendments, from this website without further notification from the Contracting Officer. The term of the contract will be one (1) base year plus four (4), one (1) year options, at the discretion of the Government.

Offerors are responsible for submitting offers, and any modifications, revisions, or withdrawals, so as to reach the Network-23 Contracting Officer (NCO-23) listed on the SF 1449, block 9 by the time specified in the solicitation. If no time is specified in the solicitation, the time for receipt is 4:30 P.M., Central Time (CT), for the designated Government officer on the date the offers or revisions are due.

1. CENTRAL CONTRACTOR REGISTRATION (CCR):

All interested parties should insure they are registered in Central Contractor Registration (CCR), the primary registration database for the United States Federal Government. For registration information, please access:

To be considered for award, the successful offeror must be registered in Central Contractor Registration (CCR) and provide the following information

a. A company profile, including name of business, DUNS number, and other pertinent information

b. A description of capabilities.

c. Past performance data for similar size facilities, multiple locations which demonstrate your capability to satisfactorily execute a contract to provide safe and timely laboratory courier services as described in this notice. Data shall include customer name, address, contract number, and point of contact information (name, phone number, and e-mail address.)

d. The business size and business socioeconomic category; such as Service-Disabled Veteran Owned Small Business, Veteran-Owned Small Business, Hub Zone Small Business, etc--- Please note, all SDVOSB's and VOSB's must be verified by the office of Small Disadvantaged Business Utilization's Center for Veterans Enterprise (CVE) prior to receiving any award.

2. SOLICITATION NOTES:
