This is an INTENT TO SOLE SOURCE NOTICE only. The Department of Veteran Affairs, Network Contracting Office (NCO) 16, intends to issue a sole-source purchase order to Olympus America Inc., 2400 Ringwood Avenue, San Jose, CA 95131. This sole-source procurement is in accordance with FAR 6.302.1 (only one responsible source and no other supplies or services will satisfy agency requirements) for the repair of the Olympus Endoscopes model GIF-H180J for the Gulf Coast and Pensacola Veterans Medical Centers. Services will be from 04/18/2014 - 05/30/2014 under NAICS 811219.

This procurement involves the Repair of the Olympus Endoscopes that operates IAW specifications of the Original Equipment Manufacturer (OEM).

" Contractor shall provide service to repair the following equipment:

Olympus GIF-H180J Endoscope, serial# 2104963 & 2105045

" Below are a list of all services needed:

Send in repair (parts/labor) to OEM factory.

" Contractor shall supply only OEM manufactured or approved new parts. No refurbished, used or re-stocked parts will be sent.

" Point of contact is E. J. Lipscomb and can be reached at 850-912-2238 and

Point of contact is Doug McCool and can be reached at 228-523-5569.

" Contractor shall return equipment to the Biomed Shop, 790 Veterans Way, Pensacola, FL 32507. Room 28152-1-JAC.

" Contractor shall provide equipment specific vendor trained individuals to service the equipment. Contractor shall provide all documentation/certificates showing completion of equipment specific training from the vendor by the individual conducting the repair. --

" Shall be produced, reviewed, and approved by the COR before award

" Any contractor (individual completing repair) other than the OEM manufacturer must have completed no less than four (4) service repairs on equipment (stated in the SOW) within the last calendar year. For each instance they shall provide the following: equipment name, equipment model, date, repair description.

" The contractor shall provide a service ticket for the .maintenance service at the completion of the work. This ticket must show the service performed, parts used, and signature of employee completing work.

" The contractor shall ship all parts and provide service in the most expeditious manner available.

" Any part or service not covered by this service contract will not be completed without first procuring a formal purchase order from the VAMC, Engineering Service Biloxi, MS 39531.

" The contractor shall obtain all necessary licenses and/or permits required to perform this work. Contractor shall take all precautions onecessary to protect persons and property from damage during performance of this contract. Contractor shall be responsible for any injury to himself or his employees, or others, as well as for any damage to personal property that occurs during the performance of this contract that is caused by him/her or his/her employee's fault or negligence. The contractor shall maintain personal liability and property damage insurance prescribed by the laws of the State of Mississippi.

" Contractor shall provide services during normal work hours of 8:00 a.m. to 4:30

p.m. Monday through Friday, except Federal Holidays. Federal Holidays: New Year's Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, Christmas Day, and any other day specifically declared a by the President of the United States to be a federal holiday.

" The contractor, their personnel, and their subcontractors shall be subject to the Federal laws, regulations, standards, and VA Directives and Handbooks regarding information and information system security as delineated in this contract.

" The C&A requirements do not apply and a Security Accreditation Package is not required.

" No sensitive information should be accessed, to ensure this, a Clinical

Engineering Tech will escort the contractor the entire time they are on-site and have all studies currently stored on the unit deleted before contractor arrives.

This notice is neither a request for quotes nor a solicitation of offers; however, all responsible sources may submit a capability statement, proposal, or quotation, which shall be considered by the agency. No solicitation package is available. A determination not to compete is solely within the discretion of the Government. Any response to this notice must show clear and convincing evidence that competition would be advantageous to the Government. Interested parties must respond to this notice no later than 8:00 am on April 15, 2014 to [email protected]. Responding as an interested party under the www.fbo.gov website doesn't constitute convincing evidence of capabilities. All interested parties are reminded to be registered with Central Contractor Registration (CCR) now SAM at in order to be eligible for award of Government contracts. No telephone calls will be accepted.
