This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6 - streamlined procedures for commercial items, in conjunction with FAR Part 13 - Simplified Acquisition Procedures, specifically FAR Subpart 13.5 -
Solicitation VA262-14-Q-0800 is issued as a Request for Proposal (RFP) and constitutes the only solicitation, a written solicitation will not be issued. The Government intends to award a single Firm Fixed Price award with a base year and up to four (4) option years.

The Government is seeking a turn-key preventive maintenance service contract for Government-Owned B. Braun Dialog + and Dialog + Evolution Dialysis machines in accordance with the B. Braun OEM specifications and all terms, conditions, special provisions, statement of work, and schedules herein. There are a total of 22 Dialog + units located at VA

Provisions 52.212-1, Instructions to Offerors - Commercial and 52.212-2, Evaluation - Commercial Items are applicable to this acquisition. Furthermore, Provision 52.212-2 is utilizing the Lowest Price Technically Acceptable (LPTA) on a Pass/Fail evaluation process and award will be made to the offeror who conforms to the solicitation and represents best value to the Government.
Submitting Quote: Offeror(s) shall submit their quote on company letterhead and shall include the completed pricing forms, along with name, address, telephone number of the offeror, firm's DUNS#, total price and a completed copy of Clause 52.212-3 Offerors Representations and Certifications- Commercial Items or vendor's ORCA document.
