This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number W9114F-15-A-0005 is issued as a request for quotation (RFQ). Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-50, effective 16 March 2011. Not a set-aside. North American Industry Classification System (NAICS) code for this solicitation is 541380. The Europe Regional Contracting Office has a requirement to establish a Blanket Purchase Agreement (BPA) for proficiency testing materials and services to evaluate the competence of the Laboratory Sciences operation in accordance with the requirements of ISO/IEC 17025:2005 and the accrediting body for Laboratory Sciences, American Association for Laboratory Accreditation. The BPA prices shall include commercial door-to-door shipping to Public Health Command Region - Europe (PHCR-E), Germany. Para (b) Replace subparagraph (4) with the following: (a) If a Quoter believes that the requirements in these instructions contains an error, omission, or are otherwise unsound, the Quoter shall immediately notify the Contracting Officer (KO) in writing with supporting rationale. All quoters shall submit written narratives on company letterhead that will demonstrate how their quote(s) meet the salient characteristics below: Factor I - Technical. 1. The Quoter holds an accreditation to ISO / IEC 17043: 2010 as a proficiency testing provider. 2. The Quoter must be provide a minimum of two (2) test rounds per year for each of the testing areas under the PHCR-E's Scope of Accreditation for environmental fields of testing (Exhibit 1). 3. The Quoter is capable of providing results within 2 days of receipt of data. 4. The Quoter is capable of providing an on-line method of reporting of proficiency results. 5. The Quoter is capable of providing direct reporting the PHCR-E's test result data to its accrediting body, American Association for Laboratory Accreditation. 6. The Quoter is capable of providing PHCR-E access to proficiency test result database for a minimum of five years to allow for laboratory review of past reports. Factor II - Price. The Government will select the quote that is based upon the lowest evaluated price, technically acceptable quote (LPTA). All required items shall have been priced by the Quoter. If the Quoter failed to price all required items, the quote will not be considered for award. Para (m) All Quoters will submit a written narrative on company letterhead detailing how they meet Technical Factors 1 through 6 and will provide a copy of their ISO/IEC 17043: 2010 accreditation. Quoters shall quote all items in Technical Exhibit 1 and submit the quote in Excel spreadsheet. Quoters who fail to provide this information will not be eligible for a BPA award. Para (n) ELECTRONIC OFFERS. Quoters may submit electronic quotes in response to this RFQ. The quote must arrive by the time specified in the RFQ to the following email address: [email protected] All quotes are due by 16:00 CET on Friday, 20 February 2015. Quoters may call Elena Hughes at 011-49-6371-9464-5434 to verify receipt of the quote. If the Quoter chooses to email the quote, the Government will not be responsible for any failure of transmission or receipt of the quote, or any failure of the Quoter to verify receipt of the emailed quote. If receipt of the quote submitted by email is not verified within 24 hours of submission, the Quoter shall resubmit its quote by fax to 011-49-6371-86-8070. Para (o) QUESTIONS. Prospective vendors are strongly encouraged to submit any questions regarding this RFQ no later than five (5) days before closing date of quote submission by e-mail to [email protected] or by fax (011) 49-6371-86-8070. If the Quoter chooses to email the questions, the Government will not be responsible for any failure of transmission or receipt of the questions, or any failure of the Quoter to verify receipt of the emailed inquiry. If receipt of questions submitted by email is not verified within 24 hours of submission, the Quoter shall resubmit their inquiry by fax. All questions received will be answered officially through an amendment for distribution to all prospective vendors. ACQUISITION PROCEDURE This is a commercial item under FAR Part 12 and the acquisition procedures to be used for this purchase will be in accordance with FAR Part 13. SPECIAL NOTE: All vendors must be registered in the System for Award Management (SAM) prior to award, and lack of registration shall make an offer ineligible for award. Vendors may obtain information on registration and annual confirmation requirements by calling 1-866-606-8220 or via Internet at http://www.sam.gov. ADDENDUM TO FAR 52.212-2 The Government will award a Blanket Purchase Agreement (BPA) resulting from this solicitation to the responsible Quoter whose quote conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Award will be made on the basis of the low price technically acceptable quote (LPTA). To be considered for award quotes must meet the technical evaluation factors described above. EVALUATION STANDARDS. The Quoter's technical quote will be evaluated using the following standards for a final rating: ACCEPTABLE: A quote that meets the Government's requirements identified in the solicitation. UNACCEPTABLE: A quote that fails to meet the Government's requirements identified in the solicitation. SPECIAL NOTE: All Quoters must be registered in the System for Award Management (SAM) prior to award, and lack of registration shall make an offer ineligible for award. Quoters may obtain information on registration and annual confirmation requirements by calling 1-888-227-2423 or via Internet at https://www.sam.gov/. All quotes are due by February 20, 2015 before 16:00 Central European Time. Quotes may be sent via email to [email protected], or faxed to 011-49-6371-86-8070.
