Wisconsin releases enforcement actions for September

The Wisconsin Office of the Commissioner of Insurance recently released a list of administrative actions for September.

The actions are issued by OCI when licensing applicants or licensed individuals or companies have violated Wisconsin insurance statutes or regulations. In some cases, the individual or company denied the stated allegations but consented to the action taken.

Any forfeitures paid are deposited in Wisconsin’s Common School Fund administered by the Board of Commissioners of Public Lands. Earnings from the Fund are distributed to all public K-12 schools in the state and are used by school libraries to purchase materials such as books, newspapers, and computers.

Allegations and Actions Against Agents

Rene Arredondo Mena, 2900 NE 33rd Ct., Unit 202, Ft Lauderdale, FL 33306, agreed to pay a forfeiture of $500.00. This action is based on allegations of failing to respond promptly to inquiries from OCI.

William R. Burns, 2831 Exchange Ct., Ste. C, West Palm Beach, FL 33409, was ordered to pay a forfeiture of $1,000.00. This action was taken based on allegations of making a misrepresentation in the sale of an insurance product.

Diane Cano, W8207 Bridle Path, Lake Mills, WI 53551, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Alondra Crenshaw, 4389 Lailah Skye Ave., Las Vegas, NV 89141-8965, was ordered to pay a forfeiture of $1,000.00. This action was taken based on allegations of making a misrepresentation in the sale of an insurance product.

Terry Ann Dawkins, 816 Smith St., Norristown, PA 19401-4049, agreed to the voluntary surrender of her license. This action was taken based on allegations of failing to pay a previous forfeiture when due.

Tai C. Dengsavang, 5278 S. Acorn Ln., Greenfield, WI 53221, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Jimmy Grinberg, 1550 Sawgrass Corporate Pkwy., Sunrise, FL 33323-2818, was ordered to pay a forfeiture of $1,000.00. This action was taken based on allegations of making a misrepresentation in the sale of an insurance product.

Jesus Hernandez, 3138 S. 22nd St., Milwaukee, WI 53215, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Johnpierre J. Herrera, 1025 Old Country Rd., Westbury, NY 11590, had his application for an insurance intermediary license denied. This action was taken based on allegations of failing to disclose previous administrative actions and having previously surrendered his Wisconsin insurance intermediary license.

Miguel A. Ibarra, 2010 16th St., North Chicago, IL 60064, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Luis Martinez, 2445 S. 30th St., Milwaukee, WI 53215, had his application for an insurance intermediary license denied. This action was taken based on allegations of failing to disclose a criminal conviction and having a criminal conviction which may be substantially related to the circumstances of holding an insurance license.

Lennox Moses, 7930 Hampton Blvd., Apt. 720, North Lauderdale, FL 33068, agreed to voluntarily surrender his Wisconsin insurance license. This action was taken based on allegations of failing to pay a previous forfeiture when due.

Antonio Negrin, 1803 N. 11th St., Penitas, TX 78576, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely report an administrative action taken by another state.

John Offutt, 508 Solar Isle Dr., Fort Lauderdale, FL 33301-2626, agreed to the denial of his two applications for a life line of authority. This order does not alter, rescind, or replace the April 11, 2025, administrative action.

Tristin Palmer, 131 Garrett Rd., Madisonville, TN 37354-6409, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely report an administrative action taken by another state.

Luis A. Ramirez, 250 S. Grand Ave., Sun Prairie, WI 53744, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Wendy Rivera, 914 Taft St., Kaukauna, WI 54130, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Luis Silva, 143 W. Tripoli Ave., Milwaukee, WI 53207, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Matthew Skaggs, 8425 Green St., Wheelersburg, OH 45694, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely report an administrative action taken by another state.

Imon D. Williams, 4554 N. 85th St., Milwaukee, WI 53225, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Allegations and Actions Against Companies

Calco Commercial Ins Services Inc., 11339 Porter Valley Dr., Northridge, CA 91326, was ordered to pay a forfeiture of $100.00. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

Detego Health LLC, 4100 International Plz., Ste. 150, Fort Worth, TX 76109, agreed to the denial of their insurance license. This stipulation does not constitute an admission by Petitioner that any conduct cited by the Commissioner as grounds for Denial: (a) occurred or is likely to have occurred, or (b) warrants denial of the Application.

Independence Pet Insurance Company, 11333 N. Scottsdale Rd., Ste. 160, Scottsdale, AZ 85254, was ordered to pay a forfeiture of $1,000.00. This action was taken based on allegations of failing to pay a required fee to the Wisconsin Insurance Security Fund.

Keith Singer, P.A., 1515 S. Federal Hwy., Ste. 302, Boca Raton, FL 33432, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

Rateforce LLC, 3423 Piedmont Rd., Atlanta, GA 30305, had their license revoked. This action was based on allegations of defaulting on a forfeiture and multiple violations of insurance laws and regulations.
