Wisconsin releases enforcement actions for March

The Wisconsin Office of the Commissioner of Insurance recently released a list of administrative actions for March.

The actions are issued by OCI when licensing applicants or licensed individuals or companies have violated Wisconsin insurance statutes or regulations. In some cases, the individual or company denied the stated allegations but consented to the action taken.

Any forfeitures paid are deposited in Wisconsin’s Common School Fund administered by the Board of Commissioners of Public Lands. Earnings from the Fund are distributed to all public K-12 schools in the state and are used by school libraries to purchase materials such as books, newspapers, and computers.

Allegations and Actions Against Agents

Justin E. Ander, 2652 Yalonda Ct., Beavercreek, OH 45434-6750, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to report an administrative action taken by another state.

Christian Bingo, 3970 Laurel Wood Ln., Del Ray Beach, FL 33445, had his license revoked. This action was taken based on allegations of failing to pay a previous forfeiture when due, and subsequently, failing to respond, appear, and update his address.

Grace Blue, 29400 Towne Center Cir., #05, Warren, MI 48093, had her insurance intermediary license revoked and was ordered to pay a forfeiture of $5,000.00. These actions were taken based on allegations of making misrepresentations relating to the insurance business and failing to respond to inquiries from OCI.

Michael P. Brown, 14155 W. Mountain View Blvd., Apt. 1058, Surprise, AZ 85374-4832, agreed to the suspension of his Wisconsin insurance license. This action was taken based on allegations of switching consumers' health insurance plans without their knowledge or consent.

John Carroll, 2070 N. Riverboat Rd., Apt. 414, Milwaukee, WI 53212, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Shadron Carson, 3613 N. 22nd St., Milwaukee, WI 53206, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Jonathan A. Corona Perez, 100 Shearwater Court East, Apt. 24, Jersey City, NJ 07305, had his application for an insurance license denied. This action was taken based on allegations of failing to disclose administrative actions taken by other states and having a restricted license in his home state.

Paul D. Diaz, 5034 Burnside Farm Pl., Haymarket, VA 20169-2583, agreed to pay a forfeiture of $1,000.00. This action was taken based on an allegation of misrepresentation in an insurance transaction.

Lamar Felder, 4650 Mossburg Ct., Marietta, GA 30066, had his application for an insurance license denied. This action was taken based on allegations of having a criminal conviction substantially related to the circumstances of holding an insurance license, having a restricted license in his home state, and owing child support arrears.

Brian Forbes, 1544 Mistflower Ln., Winter Garden, FL 34787, agreed to the permanent revocation of his Wisconsin insurance license. This action was taken based on allegations of fraudulent or dishonest business practices.

Michael C. Furman, 736 Cresta Cir., West Palm Beach, FL 33413, had his application for an insurance license denied. This action was taken based on allegations of an administrative action taken by the Securities and Exchange Commission and having a license application denied by another state.

John Herrera, 699 W. Mifflin St., Madison, WI 53703, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Richard L. Johnson Jr., 4618 Forest Hills Dr., Two Rivers, WI 54241, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Jenna A. Lautt, 705 14th St., Ste. #4, Jamestown, ND 58401, agreed to the permanent revocation of her insurance license. This action was taken based on allegations of submitting life insurance applications with forged customer signatures.

Brandon A. Lewandowski, 3119 16th Street Up, Cleveland, OH 44109, was ordered to pay a forfeiture of $1,000.00. This action was taken based on allegations of failing to timely disclose administrative actions taken by other states.

Luke A. McNamara, 1114 Susan Ln. N., Roberts, WI 54023, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

John M. Nassif, 1151 Bob White Trl., Chuluota, FL 32766, had his application for an insurance license denied. This action was taken based on allegations of having criminal convictions that may be substantially related to insurance marketing and making misleading statements on his license application.

Mary S. Peterson, 512 4th St. N., #3, Hudson, WI 54016, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Nyreneisabella Rodriguez, 8525 Hidden River Pkwy., Unit 203, Tampa, FL 33637-1151, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely report an administrative action taken by another state.

Stephen Schramke, 1580 N. Logan St., Ste. 660, Denver, CO 80203-1994, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely report an administrative action taken by another state.

Thomas L. Self, 2308 61st St., Downers Grove, IL 60516, agreed to surrender his public adjuster registration and to pay a forfeiture of $1,000.00. This action was taken based on allegations of failing to maintain a public adjuster bond and failing to respond to inquiries by OCI.

Bradley R. Smith, 10855 S. Wedington Blacktop Rd., Lincoln, AR 72744-8532, agreed to pay a forfeiture of $1,000.00. This action was taken based on an allegation of authorizing the use of his NPN for health insurance enrollments by other agents.

Chrissa Smith, 8106 N. Thomas Meyers Dr., Apt. F, Kansas City, MO 64118, had her application for an insurance intermediary license denied. This action was taken based on allegations of having a criminal history that could be substantially related to insurance business conduct and failing to report a criminal charge.

Trevor A. Smith, 730 Redwing Place Dr., Houston, TX 77009, had his insurance license revoked. This action was taken based on allegations of failing to respond to inquiries by OCI and failing to participate as ordered in an administrative hearing.

Michelle Tegelman, 700 S. Main St., Necedah, WI 54646, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

John Thomas, 2033 Goodrich Ave., Flint, MI 48503, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely report an administrative action taken by another state.

Monique R. Turner, 15700 W. Capitol Dr., Ste. D, Milwaukee, WI 53005, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Thomas Valiare, 120 W. 12th St., Ste. 1700, Kansas City, MO 64105-1923, had his application for an insurance license denied. This action was taken based on allegations of owing child support arrears and having a conviction substantially related to the business of insurance.

Michael C. Walker, 12656 Southern Highlands Pkwy., Unit 1040, Las Vegas, NV 89141-3325, agreed to the suspension of his Wisconsin insurance license. This action was taken based on allegations of misrepresentation and sales of unsuitable health insurance.

Allegations and Actions Against Companies

AA Ins Group LLC, P.O. Box 8325, Chicago, IL 60608, was ordered to pay a forfeiture of $1,000.00. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

AGINS LLC - 000, P.O. Box 817, Perry, KS 66073, agreed to surrender their intermediary firm license and to pay a forfeiture of $500.00. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

Blackrock Insurance Group Inc., 1604 NW Cataluna Cir., Port St Lucie, FL 34986, agreed to pay a forfeiture of $500.00 and surrendered their intermediary firm license. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

First Continental Life & Accident Insurance Company, 101 Parklane Blvd., Ste. 301, Sugar Land, TX 77478, following a hearing, was ordered to pay: (1) $100,170 to OCI for violating agent appointment requirements; and (2) $16,467.92 to a Wisconsin member for failing to provide a certificate of coverage, and through its agent, providing false and misleading information.

Fortune Management, Inc., P.O. Box 75009, Seattle, WA 98175, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

Global Secutive LLC, 335 16th Ave. NE, St. Petersburg, FL 33704, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

Iworker Innovations LLC, 9864 E. Grand River Ave., Ste. 110-112, Brighton, MI 48116, was ordered to pay a forfeiture of $1,000.00. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

Koshick & Associates LLC, 13500 Watertown Plank Rd., Ste. 105, Elm Grove, WI 53122, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

Oak Ridge Financial Services Group, Inc., 701 Xenia Ave. S., Ste. 100, Golden Valley, MN 55416, agreed to pay a forfeiture of $1,500.00. This action was taken based on allegations of having a history of administrative actions and failing to report administrative actions.

Saetre Insurance Solutions LLC, 12574 Flagler Center Blvd., Ste. 101, Jacksonville, FL 32258, agreed to surrender their intermediary firm license and pay a forfeiture of $500.00. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

Saginaw Underwriters Inc., PO Box 1928, Saginaw, MI 48605, agreed to surrender their intermediary firm license and to pay a forfeiture of $500.00. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

TDC Life Inc., 1440 Arrowhead Dr., Maumee, OH 43537, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

TVT Elkhorn Travel, 1219 W. Main St., Lake Geneva, WI 53147-1712, agreed to surrender its license and pay a forfeiture of $100.00. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

Zachariah J Inc DBA Hammel & Company Advisory Group, 2060 N. Humboldt Ave., Ste. 275, Milwaukee, WI 53212, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

Allegations and Actions Against Other Entities

Sawan Kumar, N173W21450 Northwest Psge, Jackson, WI 53037, was ordered to pay a forfeiture of $33,000.00. This action was taken based on misrepresentations amounting to fraudulent insurance acts violating Wis. Stat 601.41(12).
