Wisconsin releases enforcement actions for July
The Wisconsin Office of the Commissioner of Insurance recently released a list of administrative actions for July.
The actions are issued by OCI when licensing applicants or licensed individuals or companies have violated Wisconsin insurance statutes or regulations. In some cases, the individual or company denied the stated allegations but consented to the action taken.
Any forfeitures paid are deposited in Wisconsin’s Common School Fund administered by the Board of Commissioners of Public Lands. Earnings from the Fund are distributed to all public K-12 schools in the state and are used by school libraries to purchase materials such as books, newspapers, and computers.
Allegations and Actions Against Agents
Krystal Anderson, 1707 Eastwood Village Dr., Stockbridge, GA 30281, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Jack Fox, 5802 Raymond Rd., Apt. 303, Madison, WI 53711, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Juanita Funches, 467 America Junction Rd., Parrish, AL 35580, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Jennifer Jones, 9356 160th Ave., Bristol, WI 53104, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Jeffrey Kaess, 2324 21st Ave., Broadview, IL 60155, agreed to pay a forfeiture of $250.00 and maintain an active public adjuster bond while registered in Wisconsin. This action was taken based on allegations of not maintaining an active public adjuster bond in favor of the State of Wisconsin and failing to respond to inquiries by OCI.
Sindy Maldonado, 6809 60th Ave., Kenosha, WI 53142, agreed to the issuance of a one-year probationary insurance license. This action was taken based on allegations of failing to disclose an administrative action on an insurance license application and having a criminal conviction, which may be substantially related to the circumstances of holding an insurance license.
Rami Mustafa, 8874 Hogan Rd., Jacksonville, FL 32216, agreed to the revocation of his Wisconsin insurance license. This action was taken based on allegations of enrolling customers into health insurance plans without their knowledge or consent.
Kevin Pearson, 4113 N. 82nd St., Milwaukee, WI 53222, had his application for an insurance license denied. This action was taken based on allegations of owing delinquent child support.
Scott P. Pederstuen, 924 Center St., Boyceville, WI 54725, was ordered to pay a forfeiture of $500. This action was taken based on allegations of failing to timely report an administrative action taken by another state.
Chancelor Pronschinske, 812 E. Fremont St., Appleton, WI 54915, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Nathaniel T. Robinson, 3736 Valley Bluff Ln., Snellville, GA 30039-2512, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to timely report an administrative action taken by another state.
Christina M. Sherry, 471 2nd Ave. N., Onalaska, WI 54650, agreed to pay a forfeiture of $100.00. This action was taken based on allegations of failing to respond to communication from OCI.
Christopher L. Stanley, 28738 SW 134th Ct., Homestead, FL 33033, had his insurance license revoked. This action was taken based on allegations of failing to pay a previous forfeiture when due.
Kyle D. Terry, 472 Prominence Way, Hudson, WI 54016, was ordered to pay a forfeiture of $1,000. This action was taken based on allegations of misleading communications in an insurance transaction.
John R. Thomas, 2033 Goodrich Ave., Flint, MI 48503, had his insurance license revoked. This action was taken based on allegations of failing to pay a previous forfeiture when due.
Valencia P. Trice, 2818 Coff Ct., North Las Vegas, NV 89081-2443, agreed to surrender her Wisconsin insurance license. This action was taken based on allegations of failing to pay a previous forfeiture when due.
John R. Volgren, P.O. Box 188, Balsam Lake, WI 54810, agreed to pay a forfeiture of $1,000.00 and submit a 1033 felony waiver application. This action was taken based on allegations of failing to timely report an administrative action taken by another state and having a criminal conviction which may be substantially related to the business of insurance.

A. A. Murrell & Associates, LLC, 5030 King Arthur Dr. N.W., Rochester, MN 55901, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.
Erie Insurance Company, 100 Erie Insurance Pl., Erie, PA 16530, agreed to pay a forfeiture of $1,000 and reinstate the consumer's policy. This action was based on allegations of issuing an improper mid-term cancellation.
Harvey Insurance Services LLC, 922 Hurricane Shoals Rd., N.E., Lawrenceville, GA 30043, was ordered to pay a forfeiture of $1,000.00. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.
ISOA LLC, 150 W. 30th St., Ste. 1101, New York, NY 10001, was ordered to pay a forfeiture of $1,000.00. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.
Iworker Innovations LLC, 9864 E. Grand River Ave., Ste. 110-112, Brighton, MI 48116-1963, agreed to surrender its Wisconsin insurance license. This action was taken based on allegations of failing to pay a previous forfeiture when due.
JDP Products LLC, 7708 W. Ludlow Dr., Peoria, AZ 85381, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

Amazon Home Warranty, 545 Metro Place South, One Metro Place Ste. 100, Dublin, OH 43017, was ordered to pay a forfeiture of $2,000, to provide all information previously requested by OCI, and to cease and desist from selling any home warranty or service contracts in Wisconsin. This action was taken based on allegations of failing to respond promptly to inquiries from OCI and selling such contracts in Wisconsin without being properly licensed.
Complete Care Home Warranty, 1908 Coney Island Ave., Brooklyn, NY 11230, was ordered to pay a forfeiture of $4,000, to provide all information previously requested by OCI, and to cease and desist from selling any home warranty or service contracts in Wisconsin. This action was taken based on allegations of failing to respond promptly to inquiries from OCI and selling such contracts in Wisconsin without being properly licensed.
Kelly Daily, 1719 Michigan Ave., South Milwaukee, WI 53172, was ordered to pay a forfeiture of $100.00. This action was taken based on evidence of the respondent committing a fraudulent insurance act by knowingly presenting a false claim for payment of a loss.
Brandon Davison, 6638 W. Moltke Ave., Milwaukee, WI 53210, was ordered to pay a forfeiture of $200.00. This action was taken based on evidence of the respondent committing a fraudulent insurance act by knowingly presenting a false claim for payment of a loss.
