State Revokes, Suspends Licenses, Certifications, Registrations of Health Care Providers

OLYMPIA, Wash., May 28 -- The Washington State Department of Health issued the following news release:

The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department's Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency's website. Click on "Look up a healthcare provider license (https://fortress.wa.gov/doh/providercredentialsearch/)" in the "How Do I?" section of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider's license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County

In December 2013, the secretary of health indefinitely suspended the credential of home care aid Corey Gene Miller (HM60314129), who didn't comply with conditions of an agreement.

Clallam County

In February 2014 the secretary of health suspended the credential of registered nursing assistant Rhonda May Hampton (NA60217204) for at least two years. On or about November 7, 2013, the Department of Social and Health Services found that Hampton had physically abused a vulnerable adult and placed Hampton on the Residential Care Services Registry, which restricts her ability to practice as a nursing assistant.

Clark County

In February 2014 the Chiropractic Commission permanently revoked the credential of chiropractor Kelly C. Smith (CH00002760). Smith's license was suspended for one year on or about December 8, 2012 yet he continued to provide chiropractic care to patients at his clinic asking which were billed to insurance companies under an employee chiropractor's national provider identifier and had an employee falsify treatment notes that they provided chiropractic care and not Kelly.

Grant County

In December 2013 the Nursing Commission indefinitely suspended the credential of registered nurse Kathleen K. Mugnos (RN00131012). The suspension is a modification of a July 2013 order that required Mugnos to participate in a substance abuse monitoring program. Before petitioning for reinstatement, she must prove that she has been clean and sober for at least 24 consecutive months.

Grays Harbor

In December 2013 the Nursing Commission lifted the suspension on the credential of registered nurse Gerald Bruce Ensley (RN00044064).

King County

In February 2014 the secretary of health indefinitely suspended the credential of emergency medical technician Todd Foreman (ES00106166) for failing to provide proof of successful completion of an ethics course in order to comply with a 2013 stipulation.

In February 2014 the secretary of health suspended the credential of certified nursing assistant Malcom Jackson (NC.10019396) for at least two years. Jackson didn't respond to a January 2012 statement of charges alleging that while employed as a certified nursing assistant at the geriatric psychiatry unit at Highline Medical Center in Tukwila he grabbed a 90-year-old dementia patient by the wrists during a behavioral disturbance causing her wrists to bleed.

In December 2013 the Pharmacy Commission indefinitely suspended the credential of pharmacy technician Lindsay Kristina Mossburg (VA60328665) after she didn't comply with conditions of an agreement.

Kitsap County

In January 2014 physician Narinder M. Duggal (MD00036603) surrendered his physician license and agreed not to resume the practice of medicine in the state of Washington, including any temporary, emergency, or volunteer practice. The agreement between Duggal and the Medical Commission to surrender his license was a result of several instances of him committing unprofessional conduct and violating the standard of care for patients.

Lewis County

In February 2014 the secretary of health indefinitely suspended the credential of nursing assistant Kimberly Rose Woo (NA00169356) after she didn't comply with terms and conditions in a December 2012 stipulation.

Lincoln County

In February 2014 the Board of Physical Therapy indefinitely suspended the credential of physical therapist Benji Ricco Fazio (P160234074). Fazio didn't comply with terms and conditions set forth in a February 2013 stipulation requiring him to enroll and participate in the Washington Health Professional Services monitoring program.

Pierce County

In January 2014 the Nursing Commission indefinitely suspended the credential of registered nurse Charity A. Ridgley (RN00176856), who didn't comply with a required substance abuse monitoring program.

Snohomish County

In February 2014 the secretary of health suspended the credential of certified nursing assistant Peter W. Eng (NC60116761) for at least three years. Eng admitted to taking the wallet of a resident in his care at the Aegis Senior Inn of Edmonds and using that resident's credit card to make purchases for himself in the amount of approximately $492. The Department of Social and Health Services placed Eng on the Residential Care Services Registry and he is prohibited from being employed in the care of or having unsupervised access to vulnerable adults.

In February 2014 the secretary of health suspended the credential of registered nursing assistant Audrey P. Erickson (NA00093399) for at least two years. Erickson, who was the owner and nursing assistant at the Rosewood Adult Family Home in Rosewood, was found by the Department of Social and Health Services to have neglected a vulnerable adult after she didn't report that another nursing assistant working at the facility was convicted of diverting a resident's controlled substances for her personal use. Erickson admitted to both not reporting the theft arrest and for keeping the nursing assistant on as an employee even after she learned of the theft.

In February 2014 the secretary of health immediately suspended the credential of certified nursing assistant Animesh Sharma (NC60293272) pending further disciplinary proceedings. On January 15, 2014 the Department of Social and Health Services issued a Notice of Final Finding on Sharma for abuse of a vulnerable adult. As a result, Sharma is prohibited from caring for or having unsupervised access to vulnerable adults.

Spokane County

In January 2014 the secretary of health suspended the credential of registered nursing assistant Vanessa M. Thrall (NA60258055) after the Department of Social and Health Services found

that she mentally and physically abused a vulnerable adult while employed as a nursing assistant at Start in Spokane.

Thurston County

In February 2014 the secretary of health immediately suspended the credential of registered nursing assistant Darlene S. Teo (NA60283468) pending further disciplinary proceedings after the Department of Social and Health Services issued a notice of final finding on her for abuse and financial exploitation of a vulnerable adult. As a result of these findings, Teo is prohibited from being employed in the care of and having unsupervised access to vulnerable adults.

Out of State

Idaho: In December 2013 the Nursing Commission suspended the credential of Natalie A. Fister (LP00053334) for at least two years after Fister reported to the commission that she'd received three convictions in Idaho for driving while intoxicated. Before she can ask for reinstatement, Fister must provide evidence of being clean and sober for at least two years.

Minnesota: In December 2013 the Nursing Commission immediately suspended the credential of Collette Marie Michaud (RN.60355332) pending further disciplinary proceedings. Michaud's nursing credential in Minnesota was suspended in June of 2013 due to her failure to comply with a 2010 stipulation.

Oregon: In February 2014, the Nursing Commission indefinitely suspended the credential of registered nurse Laura Faye Dunn (RN60027850) after the Oregon State Board of Nursing revoked her license for diverting oxycodone; a conviction of perjury; possession of methamphetamine, marijuana and heroin; endangering the life of a minor and failing to respond to Oregon's notice of proposed revocation of registered nurse license.
