State Revokes, Suspends Licenses, Certifications, Registrations of Health Care Providers

OLYMPIA, Wash., March 19 -- The Washington State Department of Health issued the following news release:

The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department's Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency's website. Click on "Look up a healthcare provider license (https://fortress.wa.gov/doh/providercredentialsearch/)" in the "How Do I?" section of the Department of Health home page (http://www.doh.wa.gov/Newsroom/2014NewsReleases/www.doh.wa.gov) (doh.wa.gov). The site includes information about a health care provider's license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County

In January 2014 the Dental Quality Assurance Commission indefinitely suspended the credential of dental assistant Jessica Ann Peterson (D160191460). She was convicted of second-degree assault.

Chelan County

In October 2013 the secretary of health suspended the credentials of registered and certified nursing assistant Shannon Rene Abrams (NC60277046 and NA60239360) for five years. Abrams made a $900 purchase at thugfashion.com and multiple purchases at Fred Meyer with a patient's credit card while working at a long-term care facility.

Clark County

In November 2013 the secretary of health permanently revoked the credential of registered nursing assistant Sergey V. Gensitskiy (NA00197463). He was convicted of three counts of first-degree child molestation, one count of second-degree child molestation, two counts of third-degree child molestation, and four counts of second-degree incest.

In October 2013 the secretary of health suspended the credential of registered nursing assistant Sara L. Carman (NA60269002) for three years. Carman withdrew more oxycodone than was necessary for patients while working at an assisted living facility. She did not properly document the withdrawal, and either took the drugs herself or was over-administering patients' medication.

In November 2013 the secretary of health suspended the credentials of agency affiliated counselor and chemical dependency professional trainee Charles R. Spivey (CG60249861 and CO60293929) for five years. Spivey had a sexual relationship with a patient.

In January 2014 the Nursing Care Quality Assurance Commission immediately suspended the credential of registered nurse Brenda Lee Hanson (RN00129197) pending further action. Hanson surrendered her license to the Oregon State Board of Nursing after she admitted to taking Dilaudid from her employer for personal use.

Cowlitz County

In January 2014 the secretary of health indefinitely suspended the credential of certified nursing assistant Eric Alexander Nathan (NC60195123). He overdosed on oxycodone and methadone during a night shift at an adult family home while he was supposed to be tending to five patients.

Douglas County

In January 2014 the secretary of health suspended the credential of registered nursing assistant David J. Lopez (NA60337509) for five years. Lopez mentally abused a vulnerable adult.

King County

In November 2013 the secretary of health suspended the credential of registered nursing assistant Ernesto P. Dilla (NA00183524) for two years. Dilla neglected a vulnerable adult while he worked at an adult family home.

In January 2014 the Veterinary Board of Governors accepted the surrendered credential of veterinarian Cathy A. Johnson-Delaney (VT00002784). Employees witnessed Johnson-Delaney inhaling the anesthetic sevoflurane in her clinic's operating room.

In January 2014 the secretary of health suspended the credential of registered nursing assistant Marianne Zweegman (NA00130895) for two years. Zweegman neglected vulnerable adults while working at an adult family home.

In January 2014 the secretary of health issued a permanent cease and desist order to Haoran Mi, who provided a massage to a client without a credential to practice as a massage practitioner.

Mason County

In October 2013 the secretary of health suspended the credentials of certified nursing assistant and emergency medical technician Crystal A. Stanley (NC10084571 and ES01176331) until further notice. Stanley was charged with second-degree theft for stealing $2,500 from a firefighter association while serving as its treasurer.

Pierce County

In November 2013 the Medical Quality Assurance Commission immediately suspended the credential of physician and surgeon Gregory R. Brammer (MD00037500) pending further disciplinary hearings. Brammer's license was suspended in Texas due to a 2012 harassment conviction. His wife allegedly filed a police report claiming he punched, strangled, and slapped her.

In November 2013 the Nursing Care Quality Assurance Commission suspended the credential of licensed practical nurse Drew Anne Hamrick (LP00044526) for 10 years. Hamrick was convicted of four counts of witness tampering and one count of unlawful imprisonment relating to a child abuse investigation.

In November 2013 the Veterinary Board of Governors revoked the credential of veterinarian William J. Briskey (VT00001336). He provided subpar care to numerous animals that put them at considerable risk for harm and resulted in the death of one dog.

In January 2014 the Medical Quality Assurance Commission withdrew charges against the credential of Paula L. Schulze (MD00021853).

In December 2013 the Department of Health denied a certified nursing assistant application for Janet Lynne Cites (NC60425132) based on a prior second-degree theft conviction.

In January 2014 the secretary of health suspended the credential of registered nursing assistant Amanda Jill Thiel (NA60320585) for five years. Thiel was convicted of unlawful possession of a controlled substance - heroine.

In January 2014 the Department of Health suspended the credentials of certified and registered nursing assistant Terri L. Jadin (NC60276755 and NA60099691) pending further action. Jadin allegedly stole a patient's computer and other household items while she worked at an assisted living facility.

Snohomish County

In January 2014 the Nursing Care Quality Assurance Commission indefinitely suspended the credential of registered nurse James Randall-Graydon Nelson (RN60105802). He failed the terms of his drug monitoring program. Nelson tested positive for benzodiazepines, missed scheduled check-ins, and didn't submit urine samples. He was also arrested for domestic violence and claimed that he didn't remember the event because he was using benzodiazepines.

Spokane County

In January 2014 the Pharmacy Quality Assurance Commission indefinitely suspended the credential of pharmacy assistant Casey V. McGaughy (VB00071773). McGaughy diverted almost 1,300 pills while employed at a Shopko Pharmacy in Spokane.

Thurston County

In January 2014 the Department of Health granted Taylor Miryn Pulsifer (NC60331766) her credential to practice as a certified nursing assistant with conditions and stipulations.

In January 2014 the secretary of health suspended the credential of registered nursing assistant Riley David Branon (NA60315555) for two years. Branon left a wheelchair-bound patient with developmental disabilities alone in their residence and wrote "resigning" next to his signature in the staff log.

Walla Walla County

In January 2014 the Medical Quality Assurance Commission withdrew the charges against Kenneth E. Moore (PA10003031).

Whatcom County

In January 2014 the secretary of health suspended the credential of registered nursing assistant Amy K. Hamner (NA00130741) for two years. Hamner neglected a vulnerable adult while working as a caregiver for Volunteers of America.

Yakima County

In December 2013 the secretary of health suspended the credential of certified nursing assistant Maria Lourdes Martinez (NC10035858) for five years. Martinez was convicted of second-degree identity theft. She stole a patient's credit card and made unauthorized purchases while she was working at an in-home health care provider.

Out of State

Alabama: In January 2014 the Nursing Care Quality Assurance Commission immediately suspended the credential of registered nurse Terrelynn Huston (RN60265983) pending further action. The California Board of Registered Nursing revoked Huston's credential after she obtained or possessed controlled substances without a prescription.

California: In November 2013 the Nursing Care Quality Assurance Commission indefinitely suspended the credential of registered nurse Stephen M. Kelly (RN00161556). His credential was suspended by the Ohio Board of Nursing for not complying with a consent order on charges of unprofessional conduct. His license was then suspended by the Virginia Board of Nursing.

Oregon: In November 2013 the Board of Naturopathy accepted the surrendered credential of naturopathic physician Chrysalis Yashpal Jayne (NT00000694). He was convicted of first-degree criminal mistreatment. A person under Jayne's care stopped breathing and died hours after taking Ibogaine.
