State Disciplines Health Care Providers

OLYMPIA, Wash., April 25 -- The Washington state Department of Health issued the following news release:

The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department's Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency website. Click on "Provider Credential Search" on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider's license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Clark County

In March 2012 the Nursing Assistant Program ended the probation order against registered nursing assistant Denis A. Nikiforyan (NA60136029).

In April 2012 the Nursing Assistant Program ended the probation order against certified nursing assistant Julia Brookes Willich (NC60133300).

Island County

In March 2012 the Massage Program charged Debra Metz (MA00005026) with unprofessional conduct. Her credential to practice as a certified nursing assistant was suspended.

Jefferson County

In March 2012 the Chiropractic Commission ended the probation order against chiropractor Keith David Meyer (CH60131354).

King County

In April 2012 the Dental Hygienist Program charged Susan Lynnette Carlson (DH00001002) with unprofessional conduct. She allegedly worked outside of her scope of practice by providing dental treatments without being adequately supervised by a dentist.

In March 2012 the Chemical Dependency Professional, Agency Affiliated, and Mental Health Counselor Programs ended the probation order against chemical dependency professional, agency-affiliated counselor, and mental health counselor associate Bobbie Denise Catton (CP60074797, CG60131664, MC60210787).

In March 2012 the Nursing Assistant Program denied the application of certified nursing assistant applicant Chelsea Elizabeth Cullom (NC60141537). She was found guilty of attempted theft and attempted trafficking stolen property.

In March 2012 the Registered Counselor and Chemical Dependency Professional Programs ended the probation order against registered counselor and chemical dependency professional Terry W. Hake (RC00018376, CP00003479).

In April 2012 the Board of Physical Therapy charged physical therapist Suzanne Marie Jennings (PT00003907) with unprofessional conduct. She allegedly failed to provide appropriate physical therapy care, failed to properly document, and/or billed for treatments not provided to patients.

In April 2012 the Chemical Dependency Professional Program granted the application of chemical dependency professional trainee Thomas Jay Osterdahl (CO60273754) and placed his certification on probation for three years. He was convicted of two counts of possession of stolen property, theft, and unlawful possession of a controlled substance -- methamphetamine.

In March 2012 the Nursing Assistant Program denied the application of certified nursing assistant applicant Grethel Tatiana Perez (NC60257270). She failed to provide proof of her social security number to the department as requested.

In March 2012 the Chemical Dependency Professional Program granted the application of chemical dependency professional trainee Janelle Alene Wallace (CO60269974) and placed her certification on probation for three years. She was convicted of obstructing a law enforcement officer, assault, possession of marijuana, and endangerment with a controlled substance.

Pierce County

In March 2012 the Nursing Commission charged registered nurse Andrea M. McGonigle (RN60001678) with unprofessional conduct. She allegedly failed to follow protocol when administering medications to patients, and failed to check a patient for an allergic reaction to a medication.

In March 2012 the Massage Program granted the application of Gillian Doreen Meyer (MA60253942) and placed conditions against her license with which she must comply. She was found guilty of unlawful possession of a controlled substance -- methamphetamine.

In March 2012 the Nursing Assistant Program denied the application of registered nursing assistant applicant Karen D. Slife (NA60214608). She was convicted of criminal solicitation of a controlled substance and of no-contact order violation. She also entered into an order of continuance for domestic violence -- assault. An Adult Protective Services Outcome Report found that she financially exploited a vulnerable adult. She failed to disclose the convictions on her application.

In April 2012 the Health Care Assistant Program granted the application of Jennifer Dawn Woolery (HC60259211) and placed her license on probation for one year. She was convicted of driving under the influence on two occasions.

Snohomish County

In March 2012 the Examining Board of Psychology released Lisa Marie Sullivan (PY00003859) from terms and conditions set against her license.

In March 2012 the Massage Program charged Anatosha M. Wagner (MA00024020) with unprofessional conduct. She allegedly billed a patient's insurance for services not provided.

Spokane County

In April 2012 the Nursing Assistant Program ended the probation order against registered and certified nursing assistant Miranda Ruth Crigger, also known as Miranda Ruth Wolfer (NA60104836, NC60128270).

In April 2012 the Chemical Dependency Professional Program granted the application of chemical dependency professional trainee Magen LeeAnn Lloyd (CO60262329) and placed her certification on probation for three years. She was found to have committed two counts of theft, criminal trespass and making a false statement. She was also convicted of possession of stolen motor vehicle and possession of a controlled substance -- methamphetamine.

In March 2012 the Medical Commission ended the probation order against physician William T. Roth (MD00035230). Dr. Roth's license is unrestricted.

In March 2012 the Medical Commission charged physician Daniel H. Stoop (MD00032086) with unprofessional conduct. The care he provided allegedly fell below the standard when managing a patient's pain treatment.

Thurston County

In March 2012 the Examining Board of Psychology placed the license of David P. Monson (PY00001768) on probation. He viewed pornographic images on his employer's equipment while at work.

Walla Walla County

In March 2012 the Board of Pharmacy charged pharmacist Nathaniel A. Baker (PH00052633) with unprofessional conduct. He allegedly diverted prescription drugs from his places of employment for his own use. He was charged in federal court for diverting drugs.

Yakima County

In March 2012 the Counselor Program charged agency-affiliated counselor Erin Teresa Michalsky (CG60127271) with unprofessional conduct. She allegedly failed to follow physician's orders to provide one-on-one observation of a patient.

In April 2012 the Nursing Commission charged licensed practical nurse Kelly I. Richardson-Arellano (LP00042925) with unprofessional conduct. She allegedly failed to comply with terms and conditions set against her license.

Out of State

Kentucky: In March 2012 the Medical Commission reinstated the license of physician David E. Collins (MD00041332). Dr. Collins' license is unrestricted in Washington.

Oregon: In March 2012 the Nursing Commission charged registered nurse Jeannine Loretta Thorpe (RN60103041) with unprofessional conduct. She allegedly tested positive for methadone in a urinalysis and failed to respond to allegations of failing to give a patient the correct pain medication.

Oregon: In March 2012 the Chemical Dependency Professional Program ended the probation order against chemical dependency professional Renee Kayhan Ulrich (CP60116496).

Pennsylvania: In March 2012 the Nursing Assistant Program ended the probation order against certified nursing assistant Joel Christopher Sancho (NC60204800).

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
