Wisconsin releases enforcement actions for February
The Wisconsin Office of the Commissioner of Insurance recently released a list of administrative actions for February.
The actions are issued by OCI when licensing applicants, or licensed individuals or companies have violated Wisconsin insurance statutes or regulations. In some cases, the individual or company denied the stated allegations but consented to the action taken.
Any forfeitures paid are deposited in Wisconsin’s Common School Fund administered by the Board of Commissioners of Public Lands. Earnings from the Fund are distributed to all public K-12 schools in the state and are used by school libraries to purchase materials such as books, newspapers, and computers.
Allegations and Actions Against Agents
Michael J. Archimede, S41W26893 Oak Grove Ln., Waukesha, WI 53189-6536, agreed to the permanent revocation of his Wisconsin insurance license. This action was taken based on allegations of borrowing money from a customer.
Beau A. Breske, 1678 Hunter Rd., Waukesha, WI 53189, had his application for an insurance license denied and his hearing request dismissed. These actions were taken based on allegations of failing to timely disclose criminal charges; failing to disclose a criminal conviction on a licensing application; having a criminal conviction that may be substantially related to insurance marketing type conduct; violating the terms of a previous warning letter and failing to appear at an administrative hearing as ordered.
David E. Cortez, 12326 Hawthorne Hill Cir., Humble, TX 77346-4562, had his non-resident license revoked. This action was taken based on allegations of failing to pay a previous forfeiture when due.
Sierra Currie, 4011 W Capitol Dr., Ste. 201, Milwaukee, WI 53216-2557, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Daren Davis, 19730 NW 34th Ave., Miami Gardens, FL 33056, had his application for an insurance license denied. This action was taken based on allegations of failing to disclose administrative actions on a licensing application.

BH TPA Services LLC, 2817 W End Ave., Ste. 126, Nashville, TN 37203-1453, had its license revoked. This action was taken based on allegations of failing to pay a previous forfeiture when due.
Mt. Morris Mutual Insurance Company, N1211 County Road B, Coloma, WI 54930, agreed to pay a forfeiture of $250.00 and agreed to offer to reinstate a customer's personal auto policy. This action was taken based on allegations of failing to provide proper notice of nonrenewal.
Warrantech Automotive, Inc., 2200 Hwy 121, Ste. 100, Bedford, TX 76021, had its application to renew its intermediary firm license denied. This action was taken based on allegations of failing to comply with an order from the Commissioner directing it to change its firm name to avoid confusion with an insurer already in business.
