This is a Sources Sought only. The Department of Veterans Affairs (VA) is performing market research to determine if there are any vendors with economic status of Service Disabled Veteran Owned, Veteran Owned, Women Owned, and other Small Businesses that can provide Hospice Care Services in the Central California area (15 miles from VA Central California Health Care System located at 2615 E. Clinton Avenue, Fresno, CA 93703). The projected period of performance is November 01, 2017 through October 31, 2020. The socio-economic status of the responders will determine the type of set-aside, if any. Summary of Work

The VA Central California Health Care System has a requirement for a contractor to provide two (2) male beds for Hospice Care Services to eligible beneficiaries.

Please be aware all Federal, State and local labor laws will be applicable to this procurement including, federal minimum wage as provided in Section 6(a)(1) of the Fair Labor Standards Act and the McNamara-O Hara Service Contract Act. If you have specific questions regarding labor laws you should contact your local Federal Department of Labor representative.

There is no solicitation document as this time. The NAICS is 623110 with Small Business size of $27.5 million. NOTE: Please ensure that SAM indicates this NAICS code if you are interested in the requirement.

Interested firms will be Medicaid/Medicare qualified as applicable and licensed as a Nursing Home in the State of California. Only firms interested should send their information and/or capability statement and any questions by email to Nandini Johnson Nandini[email protected] no later than 3:00 PM Pacific Standard Time, August 14, 2017. Information should include:

REQUESTED INFORMATION:

(1) STATEMENT OF CABABILITY: Submit a brief description, five (5) pages or less, that demonstrates how your firm can provide the requested services at time of award. Include your company s experience in performing these services to the VA, other Government (Federal or State) agency, or for a private insurance company.

(2) Include the DUNS number of your firm.

(3) BUSINESS SIZE AND SOCIO-ECONOMIC STATUS:

(a) Indicate whether your business is large or small as defined by the NAICS

(b) If small, indicate if your firm qualifies as a small, emerging business, or small disadvantaged business

(c) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act

(d) Indicate if your firm is a certified Hub-zone firm

(e) Indicate if your firm is a woman-owned or operated business

(f) Indicate if your firm is a certified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB)

(g) State whether your firm is registered with the System for Award Management (SAM) at https://www.sam.gov/portal/public/SAM/ and/or the VetBiz Registry at http://vip.vetbiz.gov/. If not, please

(h) NOTE that any future solicitation can only be awarded to a company that is registered in SAM and to receive award based on VOSB or SDVOSB status you must be registered in VetBiz Registry.
