Empire BlueCross BlueShield HealthPlus' managed long-term care plan offers benefits such as podiatry, dentistry, vision and hearing benefits, nursing care, a 24/7 NurseLine, care management, adult day care, a personal emergency response system, nonemergency transportation and meals and nutrition support.

Ratings are based on a comparison of plan rates to statewide averages. They include data assessments and member satisfaction surveys.
