State Revokes, Suspends Licenses, Certifications, Registrations of Health Care Providers

OLYMPIA, Wash., April 16 -- The Washington state Department of Health issued the following news release:

The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department's Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency's website. Click on "Look up a healthcare provider license(https://fortress.wa.gov/doh/providercredentialsearch/)" in the "How Do I?" section of the Department of Health home page (doh.wa.gov). The site includes information about a health care provider's license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County

In March 2013 the Dental Commission suspended the credential of dentist James WM. Huffman (DE00005580) indefinitely. Huffman didn't comply with an agreement to participate in a substance abuse monitoring program.

Clallam County

In March 2013 the Dental Commission placed the credential of dentist Benjamin T. Bentley-Dyches (DE60195272) on probation for two years based on disciplinary action taken against him by the Colorado Dental Commission. During probation, his practice will be audited twice a year. He was fined $6,028.

In March 2013 the secretary of health indefinitely suspended the credential of radiologic technologist Jessica A. Brantly (RT00008624). While employed at a Bellingham medical center, Brantly tested positive for amphetamine. She failed to respond to charges that she didn't comply with an agreement to participate in a substance abuse monitoring program.

Grant County

In March 2013 the secretary of health suspended the credential of nursing assistant Maria DeJesus Camarillo (NA60011622) for at least five years. DeJesus Camarillo financially exploited a vulnerable adult while employed as a caregiver at a Soap Lake boarding home.

King County

In March 2013 the secretary of health suspended the credential of nursing assistant Lissette E. Karns, also known as Lissette E. Esteves, (NA00108158) for at least 10 years. Karns was convicted of two counts of third-degree theft and exploited a vulnerable adult while working in an adult family home.

In March 2013 the Nursing Commission suspended the credential of registered nurse Caroline A. Klevemann (RN00173047) for at least 24 months. Klevemann didn't comply with an agreement to participate in a substance abuse monitoring program.

In March 2013 the Secretary of Health ordered Jian Liu (MA60161983) to permanently cease-and-desist practicing massage in Washington until he obtains a valid massage practitioner credential and pays a fine of $1000.00.

In March 2013 the Nursing Commission immediately suspended the credential of registered nurse Kimberly K. Pearson (RN00077236). Her Nevada nursing credential was revoked after she tested positive for codeine and didn't participate in a required substance abuse monitoring program. In March 2013 the secretary of health denied John Mwai Thiongo's application as a registered nursing assistant after he failed to answer truthfully on the application.

In March 2013 the secretary of health indefinitely suspended the credential of respiratory care practitioner Caroline L. Zimmer (LR00002009). Zimmer didn't comply with an agreement to participate in a substance abuse monitoring program.

Lewis County

In March 2013 the Dental Commission indefinitely suspended the credential of Brady Aaron Morgan (D160219495). In October 2011 Morgan was hired as a registered dental assistant in a dental office. He told his employer that he had an Expanded Function Dental Auxiliary (EFDA) license. He was an applicant for the license but wasn't licensed. While employed, he performed dental services that were within the scope of an EFDA but outside the scope of a dental assistant.

Okanogan County

In March 2013 the Nursing Assistant Program suspended the credential of registered nursing assistant Theresa Salazar (NA00195253) for at least two years. Salazar falsified information on a patient's record while working as a nursing assistant at a retirement and assisted living residence in Omak.

Pierce County

In February 2013 the Medical Commission indefinitely suspended the credential of physician Rex M. Alvord (MD00035185). He didn't comply with an agreement to participate in a substance abuse monitoring program.

In March 2013 the secretary of health permanently revoked the credential of nursing assistant Jodi D. Becker (NA00173708), who was found guilty of third-degree criminal mistreatment and six counts of first-degree theft.

In March 2013 the secretary of health indefinitely suspended the credential of certified nursing assistant Katie Brooks Postler (NC60187832). Postler provided below standard care while working at a skilled nursing facility in Tacoma.

In March 2013 the Dental Commission indefinitely suspended the credential of dental assistant Jamie Nicole Broussard (D160201659), who didn't comply with practice conditions issued in 2011.

In March 2013 the secretary of health suspended the credential of nursing assistant Connie K. Gaylord (NA00123499) for at least 12 months. Gaylord was the owner/operator of Country Care Manor in Gig Harbor. The Department of Social and Health Services found deficiencies in care and revoked her adult family home license.

In March 2013 the Nursing Commission indefinitely suspended the credential of registered nurse Linda Ann Provencher (RN00176049). She improperly administered medication and falsified patient records while working as a registered nurse Enumclaw Regional Hospital.

In March 2013 the secretary of health indefinitely suspended the credentials of Jennifer Dawn Woolery (NA60206652 and XT60259185) registered nursing assistant and X-ray technician. Woolery didn't comply with a previous order.

Snohomish County

In March 2013 the secretary of health indefinitely suspended the credential of nursing assistant Joel S. Sijera (NA60032121), who didn't comply with conditions of a previous agreed order.

Thurston County

In March 2013 the Nursing Commission suspended the credential of licensed practical nurse Laura M. Dupont (LPN00037834) for not complying with an agreement to participate in a substance abuse monitoring program.

In March 2013 the secretary of health suspended the credential of massage practitioner Britni Ann Webber (MA60100165) at least two years. Webber didn't comply with an agreement to participate in a substance abuse monitoring program.

Walla Walla

In March 2013 the Health Care Assistant and Nursing Assistant Programs indefinitely suspended the credential of health care assistant Michael Vernon Nelson (HC6010904). He pleaded guilty to driving under the influence, harassment, second-degree criminal trespass, and disorderly conduct.

Whatcom County

In March 2013 the Nursing Commission immediately suspended the credential of registered nurse James Carey Grove (RN60184741). His California nursing credential was revoked in 2012.

Out of State

Arizona: In March 2013 the Nursing Commission indefinitely suspended the credential of Benson Chege Kiarie (RN60313559). Kiarie's Arizona nursing credential was revoked in September 2012. In January 2013 the Nursing Commission immediately suspended his Washington credential.

California: In March 2013 the secretary of health ordered Yan LiNovember 2012 an investigator found that Yan Li, co-owner of the Reflexology Shop in Silverdale, advertised "body massage" and "body acupressure" but doesn't hold a credential for massage.

Ohio: In March 2013 the Nursing Commission indefinitely suspended the credential of registered nurse Audra Elizabeth Jones (RN60165199) due to actions against her credential to practice nursing in both Ohio and Indiana.

Minnesota: In March 2013 the secretary of health suspended the credential of massage practitioner Kara Buchanan (MA00010945) for at least two years after Buchanan was convicted of first-degree rendering criminal assistance.

Indiana: In March 2013 the Medical Commission immediately suspended the credential of physician Lea Ann Marlow (MD60202294) pending further disciplinary proceedings after her credentials were suspended in both Kentucky and Indiana.
