State Revokes, Suspends Licenses, Certifications, Registrations of Health Care Providers

OLYMPIA, Wash., Oct. 30 -- The Washington state Department of Health issued the following news release:

The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department's Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency's website. Click on "Look up a healthcare provider license" in the "How Do I?" section of the Department of Health home page (doh.wa.gov). The site includes information about a health care provider's license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County

In August 2012 the Nursing Commission indefinitely suspended registered nurse Tamira J. Junt (RN00174176). Junt provided substandard care. On one occasion, she set up a PCA pain pump improperly and the patient wasn't getting any pain medication following knee surgery. She also didn't monitor glucose for a diabetic patient, who had a severe insulin reaction.

In August 2012 the Nursing Commission suspended the credential of registered nurse Sara L. Wardian (RN60015759) for at least 24 months. Wardian diverted numerous controlled substances for her own use from the facility where she worked, and tested positive for at least five controlled substances during a drug screen.

In February 2012 the Counselor Program suspended the credential of agency affiliated counselor Jonathan P. Smith (CG60161775) for at least 30 months. His credential expired but was eligible for renewal. He was convicted of unlawful display of a weapon, assault, attempting to elude police, and malicious mischief.

Chelan County

In September 2012 the Secretary of Health suspended the credential of registered nursing assistant Miriam Elizabeth Jimenez (NA60164300) for at least five years. In February 2012, she was convicted of second-degree theft and second-degree trafficking stolen property for pawning a wedding ring that belonged to a dementia patient.

Cowlitz County

In September 2012 the Board of Pharmacy indefinitely suspended the credential of pharmacy technician Frances B. Swett (VA00046198). She was unable to retain information over short periods of time, requiring multiple interventions from other pharmacy personnel while filling prescriptions.

Grant County

In September 2012 the Medical Commission indefinitely suspended the license of physician Mohammad H. Said (MC00018311) until he successfully completes an ethics course. He will then be placed on probation for five years with conditions that he attend a yearly compliance hearing, and be restricted from prescribing controlled substances. Said repeatedly didn't comply with practice restrictions by prescribing controlled substances and authorizing use of medical marijuana.

King County

In September 2012 the Secretary of Health indefinitely suspended the credentials of registered nursing assistant and surgical technologist Angela Maria Singleton (NA60155771, ST60142076). Singleton took multiple blank prescription forms from her employer without permission, forged prescriptions for controlled substances, and filled them for herself on several occasions.

In October 2012 the Department of Health indefinitely suspended the credential of chemical dependency professional Jerry Carroll (CDP.CP.00005166) for not complying with an agreed order to participate in a substance abuse monitoring program.

In September 2012 the Department of Health dismissed a cease-and-desist notice alleging that Daniel Park (DENR.DN.00000312) was practicing as a denturist without a credential. His credential expired in 2009; he applied for renewal in July 2012.

Lewis County

In September 2012 the Unlicensed Practice Program ordered that Toby P. Swanger cease-and-desist the practice of medicine and pay a $100 fine. In November 2011 Swanger performed a test on a client that purports to monitor electrical changes in the skin. He then prescribed essential oils and nutritional supplements.

In August 2012 the Nursing Commission suspended the credential of registered nurse Michelle I. Eskridge (RN00153974) for 60 months and fined her $5,000. She didn't document administration of pain medication and didn't follow the medication orders for a patient at a skilled nursing facility where she was employed.

In August 2012 the Nursing Commission suspended the credential of licensed practical nurse Jill Holguin (LP60122511) for at least 36 months and ordered her to complete 38 hours of continuing education and pay a $1,000 fine. Holguin didn't properly document administering or disposing controlled substances on multiple occasions.

Pierce County

In August 2012 licensed practical nurse Ronni Marie Benson (LP00052126) agreed to surrender her license to the Nursing Commission. She cannot practice as a licensed practical nurse in Washington. Benson didn't comply with a previous stipulation that she participate in a substance abuse monitoring program.

In August 2012 the Nursing Assistant Program suspended the credential of certified nursing assistant Teosia Paaga (NC10068759) for at least five years. Paaga took $30 from a resident at the assisted living facility where she worked.

Spokane County

In September 2012 the Nursing Assistant Program indefinitely suspended the credential of registered nursing assistant Mary Lois Belton (NA60119200). Belton tested positive for methamphetamine and marijuana during a workplace drug screen.

In September 2012 the Nursing Assistant Program indefinitely suspended registered nursing assistant Timothy Joshua Martinez (NA00202075). In January 2012 Martinez entered a long-term care facility with a rifle, locked an employee and resident in a room, and demanded all the medication in the facility.

In September 2012 the Nursing Assistant Program indefinitely suspended the credential of registered nursing assistant Jacob F. Colliton (NA00141987). Colliton sought emergency care after taking the controlled substance Ecstasy. During the hospital visit, he said he had been snorting and smoking the narcotic OxyContin for several months. He also didn't cooperate with an investigation.

Stevens County

In September 2012 the Secretary of Health indefinitely suspended the credential of physician Daniel W. Moullet (MD00034152), and ordered him to pay a $1,000 fine. He was also ordered to submit to a physician's evaluation to determine if he can practice medicine without posing unreasonable risk of harm. If the evaluator recommends it, Moullet would be required to pass a professional ethics program before he can seek reinstating his credential. He had a sexual relationship with a patient, who he later hired as one of his employees.

Walla Walla County

In September 2012 the Respiratory Care Practitioner Program indefinitely suspended the credential Carolyn R. Parris (LR00003991) for not complying with a previous stipulation that she participate in a substance abuse monitoring program.

Whatcom County

In September 2012 the Nursing Assistant Program indefinitely suspended the credential of registered nursing assistant Margaret C. Roeckl (NA10038202) due to a physical or mental health condition that prevents her from being able to practice with reasonable skill and safety.

Out of State

Michigan: In September 2012 the Nursing Commission indefinitely suspended the credential of registered nurse Christina Hartsell (RN60049870). Hartsell withdrew controlled substances for four patients, and then either didn't administer them, document wastage of them, or diverted the medication for her own use. She also tested positive for marijuana use during a workplace drug screen.

Nevada: In August 2012 the Nursing Commission suspended the credential of registered nurse Michele A. Langholz (RN60115585) for at least 24 months. Langholz diverted fentanyl from at least one patient, and tested positive for fentanyl use during a drug screen.
