State Disciplines Health Care Providers

OLYMPIA, Wash., Aug. 19 -- The Washington State Department of Health issued the following news release:

The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department's Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency website. Click on "Look up a healthcare provider license" in the "How Do I?" section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider's license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Clallam County

In July 2013 the Chemical Dependency Professional and Agency Affiliated Counselor programs ended probation for chemical dependency professional trainee, chemical dependency professional, and agency affiliated counselor Stephanie Marie Diltz, also known as Stephanie Marie Trumbull, (CO60123260, CP60253597, CG60302475).

Clark County

In July 2013 the Chemical Dependency Professional and Agency Affiliated Counselor programs charged chemical dependency professional trainee and agency affiliated counselor Charles R. Spivey (CO60293929, CG60249861) with unprofessional conduct. Spivey engaged in sexual contact with a client.

King County

In July 2013 the Nursing Commission entered an agreement with registered nurse Debra Estenson Cadiente (RN00075160) that says she must complete continuing education and pay a $250 fine. For about nine months from the time Cadiente's credential expired until she renewed it, she used the designation "RN" in e-mail correspondence.

In June 2013 the Home Care Aide Program denied a home care aid credential to Oleg Linskiy (HM60306898). In 2006 Linskiy was convicted in California of two counts of theft of a vehicle, three counts of theft, and three counts of possession of stolen property. In 2008 Linskiy was convicted in California of second-degree commercial burglary.

In July 2013 the Nursing Commission charged registered nurse Wade A. Thompson (RN00131565) with unprofessional conduct. Thompson allegedly didn't comply with a previous stipulation.

In July 2013 the Nursing Assistant Program charged registered nursing assistant Marilu N. Fojas (NA00174381) with unprofessional conduct. Fojas owned an adult family home where six vulnerable adults lived. An employee there allegedly fondled a resident's breast. Fojas allegedly assured a police officer the employee would not return to work until he passed a polygraph, but she allegedly allowed him to return before he passed such an exam.

In June 2013 the Pharmacy Board conditionally granted a pharmacy technician credential to Ceciley Christine Beyers (VA60335894), whose pharmacy assistant credential was placed on probation earlier in 2013.

In July 2013 the Nursing Commission charged registered nurse and advanced registered nurse practitioner Joel G. Berman (RN00072108, AP30000296-PSYC) with unprofessional conduct. Berman allegedly issued medical marijuana authorizations to two patients, a 6-year-old and a 4-year-old, to treat what he diagnosed as a wasting condition. Berman allegedly didn't conduct complete physical exams of the children, didn't adequately review or consider their medical histories, and didn't adequately document either his exams or other treatment measures.

Lewis County

In July 2013 the Nursing Assistant Program charged registered nursing assistant Alayna M. Charvet (NA60110174) with unprofessional conduct. Charvet allegedly didn't comply with previous agreements and contracts.

Pierce County

In July 2013 the Nursing Commission charged licensed practical nurse Christine A. Graden (LP00042136) with unprofessional conduct. Graden allegedly took money, personal property, and identifying information from co-workers, and looked up a co-worker's health care information. She allegedly used the data to apply for consumer credit in the co-workers' names and allegedly tried to buy goods with credit granted in the co-workers' names. Graden pleaded guilty to five counts of second-degree identity theft and was sentenced to a year in community custody.

In July 2013 the Agency Affiliated Counselor Program entered an agreement with agency affiliated counselor Lisa L. Waugh (CG60153669) that reinstated her credential and placed it on probation for two years. Waugh's credential was suspended in 2012 after she developed personal relationships with clients.

In July 2013 the Chemical Dependency Professional and Certified Counselor programs ended probation for chemical dependency professional and certified counselor Rolando Erick Vasquez (CP00006436, CL60152035).

In July 2013 the Nursing Assistant Program charged registered nursing assistant Danelle Jo Boon (NA60039609) with unprofessional conduct. While working at an adult family home, Boon allegedly wrote checks totaling at least $550 on a patient's account. The checks allegedly were for her personal use and were written without the patient's permission.

Skagit County

In July 2013 the Nursing Commission entered an agreement with licensed practical nurse Chris R. Davidson, also known as Christine Ensley (LP00048042) that placed her credential on probation for at least one-and-a-half years. Under the agreement, she must also pay a $1,000 fine. Davidson didn't check a patient's blood sugar as ordered, didn't make sure the patient's service plan was up-to-date or complete, and didn't keep adequate records for the patient.

Snohomish County

In July 2013 the Nursing Commission entered an agreement with licensed practical nurse Veronica Q. Apolista (LP00049779) that says she must pay a $2,000 fine and complete course work in ethics, legal issues and liability, Washington's Uniform Disciplinary Act, time management, and disciplinary action. Apolista practiced for more than a year with an expired credential and falsified documentation about the renewal date.

In July 2013 the Nursing Commission charged licensed practical nurse Linda J. Kasner (LP00047051) with unprofessional conduct. Kasner allegedly didn't comply with a previous agreement.

In July 2013 the Dental Commission charged dental assistant Cynthia Janean Stokes (D160221561) with unprofessional conduct. Stokes allegedly admitted stealing two prescription slips from the dental office where she worked and giving them to another person.

In July 2013 the Nursing Assistant Program entered an agreement with registered nursing assistant Fatoumata Dibba Badjan (NA60032432) that placed her credential on probation for at least one-and-a-half years. Badjan didn't follow a support plan for a client under her care by not making sure the client wore a GPS device at all times. The client, who had a history of wandering away, did wander away from the supported living home. The Department of Social and Health Services won't allow Badjan to be employed in caring for vulnerable adults.

Spokane County

In July 2013 the Nursing Assistant Program charged registered nursing assistant Cynthia R. Sommer (NA00198424) with unprofessional conduct. While working in an adult family home, Sommer allegedly took or allowed someone to take a resident's television set.

In July 2013 the Nursing Commission entered an agreement with licensed practical nurse and registered nurse Jessica Grace Hanna (LP60185480, RN60232768) that placed her credentials on probation for at least two years. Hanna told her employer she'd passed a national licensing exam before she actually passed it. She worked as a registered nurse during that time. When her employer discovered the deception, she was terminated.

Yakima County

In July 2013 the Nursing Commission ended probation for registered nurse Heather L. Morse (RN00145303).

In July 2013 the Nursing Commission ended probation for registered nurse Beth Merle Weiss (RN60219485).

Out of State

Arizona: In July 2013 the Nursing Commission charged registered nurse Diane G. Blake (RN00143044) with unprofessional conduct. Blake allegedly didn't comply with a previous stipulation.

Idaho: In July 2013 the Dental Commission charged dentist and dentist general anesthesia permit holder John W. Morrison (DE00011204, GA60004326) with unprofessional conduct. Morrison allegedly didn't comply with a previous stipulation.

Minnesota: In July 2013 the Nursing Commission charged registered nurse and advanced registered nurse practitioner-certified registered nurse anesthetist Theresa M. Duffy (RN60313543, AP60315329) with unprofessional conduct. A Minnesota order placed conditions on Duffy's license in that state after she allegedly didn't adequately anesthetize a surgical patient.

Oregon: In July 2013 the Naturopathy Board amended a statement of charges against naturopathic physician Chrysalis Yashpal Jayne (NT00000694) to reflect a conviction of first-degree criminal mistreatment in 2013.
