This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the Government to award a contract. Requests for a solicitation will not receive a response. Responses to this Sources Sought must be in writing. The purpose of this Sources Sought Announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified businesses interested and capable of providing the services described below. Documentation of expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition. Responses to this notice must include the following:

a.) Company Name

b.) Address

c.) Point of contact

d.) Phone, fax, and email address

e.) DUNS number

f.) Cage Code

g.) Tax ID Number

h.) Type of business (small, large, SDVOSB, etc).

i.) Does your company possess a General Service Administration (GSA) Contract for this service If so, please list the contract number and expiration date.

Services to be Provided:

1. Contract Title. GE AMX4 plus Portable X-ray Battery Replacement Procurement

2. Background. Biomedical department does not have the specialized test equipment or technicians trained to perform service on our GE AMX 4 portable x-ray unit. A qualified service technician is required to maintain calibration after battery replacement in order to certify that this radiological device is kept operational IAW manufacturer's specifications and OSHA/FDA regulations.

3. Scope. Contractor shall furnish all supervision, labor, travel, parts, test equipment and tools to replace batteries and verify calibration for Saginaw VAMC's portable x-ray system (MFG: GE, Mdl: AMX4 plus, Site ID: 0002593661, EE# 18921). Listed equipment is currently located the Aleda E Lutz VA Medical Center, Radiology Department, room 1-2142, 1500 Weiss Street, Saginaw, Michigan 40602. Service will be provided between 8:00am to 5:00pm, Monday through Friday, excluding Federal holidays.

4. Specific Tasks.

a) Contractor shall perform all manufacturer specified maintenance in regards to battery replacement and calibration verification as published in the Manufacturer's Service Manual.

b) Contractor shall furnish all supervision, labor, travel, equipment, parts, and supplies to accomplish replacing the batteries, any repairs as necessary and ensuring system is properly calibrated.

c) OEM parts shall be used for all maintenance and repair services.

d) Contractor shall provide a copy of the Service Report, either by email or by fax, no later than 5 work days after the completion of the scheduled maintenance or repair service.

e) The Service Report shall contain the following information:

1) Contract purchase order number

2) Date service initiated

3) Date completed

4) Equipment make and unique identification number/code

5) Description of original complaint

6) Problems found

7) Part/quantities installed

8) All work performed fully described

5. Performance Monitoring Performance will be monitored by observation of work. Work will be observed by the COTR, or designee, during performance of maintenance service. Equipment operators will be contacted to report on contractor performance when COTR, or designee, is not available.

6. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI).

No Government supplied Equipment or Information is required or anticipated.

7. Other Pertinent Information or Special Considerations. Only Manufacturer Trained Service Engineers/Technicians or Manufacturer Authorized Service Engineers/Technicians will perform services and repairs.

a) Identification of Possible Follow-on Work. No follow-on work is anticipated.

b) Identification of Potential Conflicts of Interest (COI). No known COI.

c) Identification of Non-Disclosure Requirements. Contractor will not have access to Sensitive information.

d) Packaging, Packing and Shipping Instructions. N/A

e) Inspection and Acceptance Criteria. COTR, or designee, will verify that the unit is working as per Manufacturer's specifications after completion of service or repair.

8. Risk Control. The Contractor shall obtain all necessary licenses and/or permits required to perform this work. Contractor shall take all precautions necessary to protect persons and property from injury or damage during the performance of this contract. Contractor shall be responsible for any injury to any contractor representative, or others, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by the contractor's fault or negligence.

The Contractor will not discuss any patient issues that were observed during the course of the work. The Contractor meets the definition of Business Associate. Therefore, a Business Associate Agreement is required to comply with both the Health Insurance Portability and Accountability Act (HIPAA) Privacy and Security regulations.

9. Place of Performance. All services will be performed at the Aleda E. Lutz VAMC, 1500 Weiss Street, Saginaw, MI 48602

10. Period of Performance Base Year. 30 Days ARO

11. Delivery Schedule. Service Reports will be emailed or faxed to the COTR by the 5th workday after completion of work.
