Provide weekly service for fresh bread items in our Job Corps cafeteria: * 1. Include product delivery of:

* 20 oz white wheat sandwich bread;

* 20 oz wheat sandwich bread;

* 32 ct dinner rolls;

* 16 ct hotdog buns;

* 12 ct hamburger buns;

* 12 ct Texas Toast;

* 20 oz white sandwich bread;

* 6 pack hoagie rolls

* 2. Include Purchase Order on each invoice and remove empty trays every week by vendor.

W-9; Vendor Questionnaire along with bid (attached) If you are a HubZone or disadvantaged (8a) business; please include a copy of your certification.

Adams and Associates, Inc. contracting for the U.S. Department of Labor for the operation of the Gadsden Job Corps Center; a 225 student residential training facility located at 600 Valley Street, Gadsden, AL Vendor is to return written bid, vendor paper and all licenses affiliated with doing business in the State of Alabama, including all employee licenses required, Certificate of Liability Insurance , Workers Comp (1,000,000), Comprehensive Automobile Liability (1,000,000/combined single limit) and a list of references if no previous experience with this Center. Period of contract: January 1, 2014 - March 31, 2014. If your quote deviates from specifications in any way, such specifications must be noted on bid sheet. Adams and Associates, Inc. reserves the right to reject any bids and select the bid deemed most responsible. Bidders should be aware of bid requirements for federally funded projects and act accordingly. Bidders are placed on notice that this as a competitive bid process and cost is an important factor. Overall low bid does not constitute bid award. Gadsden Job Corps Center is a federally funded facility and is entitled to GSA pricing. Payment terms are net 30 days from date of invoice.
