This is a Subcontracting Opportunity with Little Rock Job Corps Center / Adams & Associates Inc. and not with the U.S. Department of Labor. DENTAL ASSISTANT - ID #: LRJC2014-037 Adams and Associates, Inc., contracting for the U.S. Department of Labor for the operation of the Little Rock Job Corps Center - a 237 student residential training facility located at 6900 Scott Hamilton Road, Little Rock, AR 72209, is currently soliciting written bids for on-site Dental Assistant services. The Center offers both residential and non-residential options in career skills, academic, and vocational training for young adults from 16 - 24 years old. Hourly rate is limited to a maximum of 7 hours per week. The period of performance for this subcontract will be June 1, 2014 through March 31, 2015, with one - 6 month option for the period of April 1, 2015 through September 30, 2014. The 6 month option will be awarded at the discretion of Adams and Associates, Inc. Written quotes should include cost price per hour for weekly services. All quotes must be submitted in writing to the Purchasing Department by April 30, 2014 by 3:00 p.m. Quotes received after the deadline and/or without all requested documentation will not be considered. Bid MUST be good for 90 days, after submission. No partial bids will be accepted.

Vendor is to return written bid on vendor letterhead and LRJC2014-037 at the top of each page, vendor paperwork, and all licenses for doing business in the State of Arkansas, all employee licenses required for performing work in the state of Arkansas, Certificate of Liability insurance and list of references if no past experience with Center.

CONTRACT PERIOD: This contract shall commence on June 1, 2014 and continue through March 31, 2015. If your quote deviates from specifications in any way, such exceptions must be noted on bid sheet. If unable to quote, please indicate 'No Bid' and return bid package to us.

Adams and Associates, Inc. reserves the right to reject any bids and select the bid deemed most responsible. Bidders should be aware of bid requirements for federally funded projects and act accordingly.

Bidders are placed on notice that this is a competitive bid process and cost is an important fact. Overall low bid does not constitute bid award.

Little Rock Job Corps Center is a federally funded facility and entitled to GSA pricing.

Payment terms are Net 30 days from date of invoice. Vendor is required to show number of hours billed and hourly rate on invoice, list of materials used to complete service and provide certified payroll records with invoice.

Subcontractor shall provide adequate certified personnel to perform dental services for students. Shall hold a current national DANB Certified Dental Assistant (CDA) certification, or graduate from a CODA-accredited dental assisting program to perform dental assistant services for students. Services shall include but are not limited to:

* - Provide seven (7) hours of on-site dental assistant services per week for 48 weeks per year (excludes 2 weeks for summer break and 2 weeks for Christmas holiday period). Actual hours/days of on-site service shall be jointly agreed to between the CENTER, CONTRACTOR and DENTIST(S), and shall become a part of this contract. Unexpected and infrequent changes to the on-Center schedule may be made without consequence or need for amendment to this contract, so long as total hours are met each week. Frequent or extensive changes to the agreed upon schedule will be unacceptable. On-site dental Assistant services shall not be required during weekends, during CENTER holidays, or during the break periods.

* - Assists Center dentist in providing preventative dental education. Assists Center dentist in coordination and integration of dental health program with other health components and Center activities.

* - Schedules and maintains the dental priority system.

* - Complies with OSHA requirements.

* - Maintains dental equipment and supplies to assure proper function and operation.

* - Maintains compliance with policies and procedures of infection control.

* - Performs quality assurance record audits.

* - Participates in department system for student feedback or concerns.

* - Maintains dental records.

* - Maintains dental statistics for dental productivity and monthly utilization reports.

* - Maintain the accuracy and confidentiality of all required oral health and wellness records when they are in the subcontractor's presence.

* - Provide support to the students in acquiring the oral health-related skills, knowledge, and attitudes that will make them employable.

* - Promote opportunities for the students to practice the skills that will help make them employable.

* - Comply with Job Corps infection control policies and procedures.
