This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Quotation (RFQ). Submit only written quotations for RFQ FA4613-14-R-0010 this solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-63. This acquisition is 100% set-aside for Total Small Business. The associated NAICS code is 562910 with a size standard of 500 employees. The line item description is as follows: CLIN 0001- Asbestos Abatement for Building 325

The provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items, applies to this solicitation and the offeror must include a completed copy of this provision with their proposal. FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this solicitation. Specifically, the following DFARS clauses cited are applicable to this solicitation: 252.204-7004, Alternate A, 252.232-7003 Electronic Submission of Payment Requests and Receiving Reports, 5352.201-9101 Ombudsman, 5352.242-9000, Contractor Access to Air Force Installation. EVALUATION OF QUOTATIONS/OFFERS: Quotes/offers will be evaluated IAW 13.106-2. This will be a lowest price technically acceptable award. Technical acceptability - at a minimum - will consist of meeting all characteristics, including those above

Quotes must be signed, dated and submitted by , 1:00 pm, Mountain Standard Time (MST) to the 90th Contracting Squadron/LGCB, Attn: Kristin Doty, Contracting Specialist, Commercial Phone 307-773-6649, Fax 307-773-3973, E-mail mailto:[email protected]. Quotes submitted via e-mail are preferred but not required.

LATE OFFERS: Offerors are reminded that e-mail transmission of quote/offer prior to the closing date and time will not constitute a timely submission of quote/offer unless it is received by the government prior to the closing date and time. Quotations/offers or modification of quotes/offers received at the address specified for the receipt of offers after the exact time specified for receipt of offers WILL NOT be considered (ref. FAR 52.212-1(f)).

All contractors must be registered in the System for Award Management (SAM) ref. FAR 52.204-99 database prior to any contract award.

Please submit the following information with each quote: Cage Code, DUNS number, Tax Identification Number, Prompt Payment Terms, Delivery Time, Date offer expires, warranty, Line item unit price, and Total cost. SITE VISIT INFORMATION FOR ASBESTOS ABATEMENT BLDG 325

DATE: April 10, 2014 at 2:00 p.m.

**Please note: Anyone who wants to participate in the walkthrough will be required to provide current asbestos certification, current fit test, and current medical surveillance documentation to enter the building. Also, Personal Protective Equipment (respirator, Tyvek suit, etc.) per WY DEQ will be required. All documentation will be reviewed prior to entry and those not current will not be allowed to enter the building.**

CONTRACTORS WITHOUT A CURRENT PASS:

F. E. Warren Air Force Base requires all visitors to stop at the Visitor Building located outside of Gate 1. Please do not attempt to enter the security gate without visiting the Visitor Building for a temporary pass.

A member of the Contracting Team will meet you inside the Visitor Building to sponsor you in order for you to get your temporary pass. Every person coming onto the base WILL be required to have

A valid photo ID

A valid copy of your vehicle registration if you are driving your vehicle on the base.*

A copy of your valid vehicle insurance card if you are driving your vehicle on the base.*

*Please be advised, your vehicle is subject to random searches as any vehicle coming onto the base. PLEASE GIVE YOURSELF ENOUGH TIME TO RECEIVE YOUR PASS. IT IS A FIRST COME, FIRST SERVE LINE WHICH MAY REQUIRE EXTRA TIME. THE CONTRACTING TEAM MEMBER WILL BE THERE AT APPROXIMATELY 1:30 P.M. TO START SPONSORING CONTRACTORS. CONTRACTOR TEAM MEMBER WILL LEAVE THE VISITOR BUILDING AT APPROXIMATELY 1:45 P.M. After receiving your temporary pass, you may follow the Contracting Team Member to the 90th Contracting Squadron, Building 208 at 7505 Barnes Loop. We will all meet together then travel to the site.

Please meet at the 90th Contracting Squadron, Building 208 at 7505 Barnes Loop by 2:00 p.m. on Thursday, April 10th. We will be traveling to the site after all have arrived.
