Wisconsin releases enforcement actions for October

The Wisconsin Office of the Commissioner of Insurance recently released a list of administrative actions for October.

The actions are issued by OCI when licensing applicants, or licensed individuals or companies have violated Wisconsin insurance statutes or regulations. In some cases, the individual or company denied the stated allegations but consented to the action taken.

Any forfeitures paid are deposited in Wisconsin’s Common School Fund administered by the Board of Commissioners of Public Lands. Earnings from the Fund are distributed to all public K-12 schools in the state and are used by school libraries to purchase materials such as books, newspapers, and computers.

Allegations and Actions Against Agents

Jesse M. Abrams, 5301 N. Federal Hwy., Ste. 204, Boca Raton, FL 33487-4915, had his insurance license revoked. This action was taken based on allegations of failing to pay a previous forfeiture when due.

Jorge A. Benavente, 10401 Innovation Dr., Ste. 125, Milwaukee, WI 53226, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Christian Bingo, 2788 SW 10th St., Boynton Beach, FL 33426, was ordered to pay a forfeiture of $500.00. This action was taken based on allegations of failing to report administrative actions taken by other states.

Durron Branch, 2973 N. 39th St., Milwaukee, WI 53210, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Angela Brown, 2228 W. Ruby Ave., Milwaukee, WI 53209, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Corey Bush, 6553 Madrone Ave., Kalamazoo, MI 49009, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Jessica L. Ealy, 9990 N. Scottsdale Rd., Apt. 2034, Paradise Valley, AZ 85253, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Tracy F. Gabryel, W17092 State Hwy. 64, Gilman, WI 54433, agreed to the issuance of a one-year probationary insurance license. This action was taken based on allegations of having a criminal conviction that may be substantially related to the circumstances of holding an insurance license.

Juan Gamez, 2171 S. 35th St., Milwaukee, WI 53215, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Brian J. Hale, 96 Kimberly Ct. SW, Pine Island, MN 55963, agreed to surrender his Wisconsin insurance license. This action was taken based on allegations of being unqualified as an intermediary.

Matt G. Heesacker, 3377 Wiggins Way, Green Bay, WI 54311, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Allison Hejdak, 48 Rushmore Ln., Hartford, WI 53027, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Dawn M. Hurlburt, 521 235th Ave., Somerset, WI 54025, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Michael Kirschling, 2232 E. Baldeagle Dr., Appleton, WI 54913, was ordered to pay $500.00. This action was taken based on allegations of failing to report a criminal conviction.

Liliana Lozano, 3309 S. 7th St., Milwaukee, WI 53215, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Mitsuko L. Martindale, 3550 Winbranch Dr., Apt. 2, Memphis, TN 38116-4424, had her insurance license revoked. This action was taken based on allegations of failing to pay a previous forfeiture when due.

Wendy McFarlane, 927 Orange Ave., Port Orange, FL 32129-3454, had her insurance license revoked. This action was taken based on allegations of failing to pay a previous forfeiture when due.

Cierra D. McLemore, 8083 Stonebrook Pkwy., Apt. 912, Frisco, TX 75034, had her insurance license revoked. This action was taken based on allegations of failing to report administrative actions taken by other states, failing to disclose a criminal conviction on a licensing application, having a criminal conviction that may be substantially related to the circumstances of holding an insurance license, failing to respond promptly to inquiries from OCI, and failing to appear at an administrative hearing as ordered.

Gabriela Mendoza, 442 N. Grand View Blvd., Waukesha, WI 53188, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Patrick Nugent, 105 Main St., Oconomowoc, WI 53066, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Mihail Pastrikos, 1654 Oak Spring Dr, Tarpon Springs, FL 34689, agreed to the permanent revocation of his Wisconsin insurance license. This action was taken based on allegations of misrepresentations in insurance transactions.

Michael Ries, PO Box 42, Peshtigo, WI 54157, had his application for an insurance license denied. This action was taken based on allegations of having criminal convictions that may be substantially related to insurance marketing and failing to disclose criminal charges, criminal convictions, and administrative actions.

Sue L. Robertson, 904 Old Dominion Dr., West Helena, AR 72390, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Aavon Sanders, 2822 N. 1st St., Apt. 111, Milwaukee, WI 53212, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Charles Sandoval, 905 W. Juniper Ave., Gilbert, AZ 85233, had his application for an insurance license denied. This action was taken based on allegations of providing inaccurate information and failing to report regulatory actions on a licensing application.

Karrieann Sell, 1505 Fairlawn Ave., North Fond Du Lac, WI 54937, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Chad Sokoloff, 11856 Foxbriar Lake Trl., Boynton Beach, FL 33473-7830, agreed to pay a forfeiture of $1,000.00. This action was taken based on an allegation of misrepresentation in an insurance transaction.

James Solberg, 1113 Sausalito Dr., Waunakee, WI 53597, agreed to a 60-day denial of his insurance license application. This action was taken based on allegations of failing to report an administrative action on a licensing application.

Ronald Stevenson, 3112 Clearwater Dr., Ste. B, Prescott, AZ 86305, had his application for an insurance license denied. This action was taken based on allegations of having a history of administrative actions and lacking the competence and trustworthiness required of insurance intermediaries.

Alexander R. Strate, 2443 Lismore Ln., Irving, TX 75063-8116, agreed to the permanent revocation of his Wisconsin insurance license. This action was taken based on allegations of switching a consumer's health insurance plan without their knowledge or consent.

Jasmine Tate, 3736 N. Humboldt Blvd., Apt. 10, Milwaukee, WI 53212, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Michelle D. Tate, 7003 Windy Run Ct., Spring, TX 77379-8263, had her insurance license revoked. This action was taken based on allegations of failing to pay a previous forfeiture when due.

Yvonne Trentlage-Poeschl, 2020 Marathon Ave., Apt. 6, Neenah, WI 54956, had her insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Eric S. Upchurch, 411 N. Pinckney St., Madison, WI 53703, had his application for an insurance license denied. This action was taken based on allegations of having convictions that may be substantially related to the circumstances of holding an insurance license and failing to disclose criminal convictions on an insurance license application.

Gary L. Voggesser, 411 Bleeker St., Apt. 308, Delafield, WI 53018, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Evan R. Westmore, N56W16077 Scott Ln., Menomonee Falls, WI 53051, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Terrance Williams, 8152 Blakton Rd., Madison, WI 53719, had his insurance license revoked. This action was based on allegations of failing to pay a previous forfeiture when due.

Matthew Winters, 4021 Meadowwood Dr., Eau Claire, WI 54701, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Dominic D. Wood, 502 Garfield Ave., Sparta, WI 54656, had his application for an insurance license denied. This action was taken based on allegations of failing to disclose criminal convictions on his licensing application.

Kevin J. Yonke, 205 Prospect Ave., Pewaukee, WI 53072, had his insurance license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.

Allegations and Actions Against Companies

Commercial Risk Partners LLC, DBA Greenroad Insurance, 5115 S. Lakeland Dr., Ste. 3, Lakeland, FL 33813, agreed to pay a forfeiture of $250.00 and surrendered their intermediary firm license. This action was taken based on allegations of failing to maintain a Designated Responsible Licensed Producer (DRLP) and failing to respond to inquiries by OCI.

Normandy Insurance Services LLC, 4800 N. Federal Hwy., Ste. 302A, Boca Raton, FL 33431, agreed to pay a forfeiture of $250.00 and to surrender its Wisconsin intermediary firm license. This action was taken based on allegations of failing to promptly respond to OCI inquiries and failing to maintain a Designated Licensed Responsible Producer (DRLP).

State Farm Fire and Casualty Company, One State Farm Plaza, Bloomington, IL 61710, agreed to pay a forfeiture of $500.00 and agreed to reverse a previous partial denial of coverage. These actions were taken based on allegations of misrepresenting policy terms and failing to promptly and accurately respond to the Office of the Commissioner of Insurance's information requests.

Allegations and Actions Against Other Entities

Go Lead Pro, LLC, PO BOX 30782, Phoenix, AZ 85046, was ordered to pay a forfeiture of $10,000.00 and to submit a compliance report detailing changes to direct mail and marketing practices. In lieu of submitting the ordered compliance report, Go Lead Pro, LLC, is going to cease operating in Wisconsin. This action was taken based on allegations of using public mortgage information to create and disseminate misleading insurance solicitations.
