Effective March 31, 2022, the Idaho Department of Insurance revoked the insurance producer license of Kyle Blaine Swallow for violations of the Idaho Insurance Code.
Swallow, a licensed insurance agent from Meridian, Idaho, admitted to eight violations of completing and submitting forged life insurance applications to gain a commission – including one instance of doing so with a deceased individual’s identity. Swallow received approximately $5,962.68 in commissions on the false applications. This is in violation of Idaho Code § 41-293(1)(c).
“The Department does not tolerate misconduct by licensed agents,” said Director Dean Cameron. “Insurance agents hold a position of trust in Idaho and are required to abide by Idaho Insurance Code. We expect them to conduct themselves with integrity and ethically.”
Since his Idaho resident producer’s license in 2019, Swallow sold insurance policies with Fidelity & Guaranty Life Insurance Company, Gerber Life Insurance Company, the Independent Order of Foresters, Kansas City Life Insurance Company, and United of Omaha Life Insurance Company.
In addition to Swallow’s insurance producer license being revoked, an administrative penalty of $8,000 was imposed on him by the Idaho Department of Insurance. This order can be read in its entirety on the Department website.
