Affinia Health Network Expands Partnership with Wellcentive to Support PHM Strategy Across Care Delivery Settings

ATLANTA, April 25, 2016 /PRNewswire/ -- Wellcentive, a leader in value-based care solutions, today announced the expansion of its population health management platform and services within the growing Affinia Health Network (AHN).

AHN, a subsidiary of Mercy Health and a member of Trinity Health and the Trinity Health Accountable Care Organization (ACO), has committed to be among the first Next Generation ACOs within the new Centers for Medicare & Medicaid Services (CMS) model. AHN's clinically integrated network (CIN), including its Grand Rapids, Michigan region providers, will expand its rollout of Wellcentive's platform to solidify data aggregation and value-based care strategies throughout its network of primary and specialty providers delivering care in both practice and hospital settings, and coordinate its participation growth within alternative payment models.

Next Generation allows organizations to participate in incentive-based payment approaches that reward accountability for outcomes, risk, and cost management of its population.
