State Revokes, Suspends Licenses, Certifications, Registrations of Health Care Providers

OLYMPIA, Wash., July 28 -- The Washington State Department of Health issued the following news release:

The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department's Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency's website. Click on "Look up a healthcare provider license (https://fortress.wa.gov/doh/providercredentialsearch/)" in the "How Do I?" section of the Department of Health home page (http://www.doh.wa.gov/) (doh.wa.gov). The site includes information about a health care provider's license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County

In February 2014 the Nursing Commission indefinitely suspended the credential of registered nurse Tamira J. Junt (RN00174176). She didn't comply with the terms and conditions of a 2013 agreed order that required her to file quarterly performance evaluations or unemployment declarations as well as proof of completion of 60 hours of continuing education that was preapproved by the commission.

Cowlitz County

In March 2014 the Nursing Commission suspended the credential of licensed practical nurse Misty Dawn Janson (LP60146118) for at least two years. Before petitioning for reinstatement, Janson must provide evidence that she has been clean and sober for two years. Janson admitted to the Department of Health that she'd taken clonazepam and morphine and smoked marijuana on the evening of November 3, 2011 even though she did not have prescriptions for the clonazepam or morphine and didn't have a medical marijuana authorization.

In February 2014 the Nursing Commission indefinitely suspended the credential of registered nurse Katherine L. Krause (RN00102955) after she didn't comply with the terms of a 2012 agreement that required her to submit quarterly employer reports or declarations. She also didn't complete required course work or pay the $1,000 in required cost recovery fees.

In February 2014 the secretary of health suspended the credential of mental health counselor Michael F. Vaughn (LH00005701) for at least one year after he became involved in a personal relationship with one of his clients.

King County

In March 2014 the Nursing Commission put the credential of licensed practical nurse Jackie Elizabeth Breiling (LP60276827) on probation for at least two years. While employed at Peoples Retirement Community, Breiling made several medication errors and also tested positive for codeine, morphine, hydrocodone, hydromorphone, and oxycodone. Breiling must enter into a Washington Health Professional Services monitoring program.

In March 2014, the secretary of health ordered Cherie Ann Byrd to cease and desist from conduct constituting practice of massage/massage therapy in the state of Washington unless she first gets a massage license. Byrd must pay a civil fine of $1,000 by September 2014. Byrd was a licensed massage therapist from April 1980 until her credential expired in 1984. On or about March 2013 she engaged in the practice of massage and advertised her massage practice from April through November 2013.

In March 2014, the Nursing Commission indefinitely suspended the credential of registered nurse Galia Pauline Gray (RN00104988) after she didn't comply with a required monitoring program.

In March 2014 the Nursing Commission immediately suspended the credential of registered nurse James William Green (RN60228074) after he was prohibited from practicing nursing by the state of North Dakota due to unprofessional conduct.

In March 2014, the secretary of health withdrew without prejudice the cease and desist order against Ellie McCormick.

Pierce County

In March 2014Nursing Commission suspended the credential of registered nurse Lindsey M. Alvarez (RN60154086) for at least two years. Alvarexdidn't comply with a contract with a substance monitoring program. She was required to enter the program after admitting to diverting narcotics that she sold or traded for alcohol.

In February 2014 the Nursing Commission indefinitely suspended the credential of registered nurse Carole I. Edenfield (RN00137966) after she admitted to her employer that she had diverted a schedule II controlled substance over the course of her employment.

In February 2014 the Nursing Commission suspended the credential of registered nurse Rachel Nezat (RN00174765) for at least two years. Nevat entered Swedish Medical Center in April 2013 even though she was not employed at Swedish at the time and had no patient there. While at the facility, she entered into the rooms of two patients and tampered with their patient-controlled analgesia machines. She was convicted of two counts of property destruction, which is a gross misdemeanor, and of acquiring a controlled substance by deception. Before her license can be reinstated, Nezat must provide evidence of being clean and sober for at least two years.

In February 2014 the Nursing Commission indefinitely suspended the credential of registered nurse Kimberley K. Pearson (RN00077236) after she didn't comply with terms of a 2013 agreed order.

Snohomish County

In February 2014 the Nursing Commission suspended the credential of registered nurse Caryn M. Carter (RN00167165) for not complying with a chemical dependency monitoring contract. Before asking for reinstatement Carter must show that she has been clean and sober for at least two years.

In March 2014, the secretary of health indefinitely suspended the credential of certified nursing assistant Animesh Sharma (NC60293272) after the Department of Social and Health Services found Sharma had abused a vulnerable adult.

Spokane County

In March 2014 the secretary of health suspended the credential of certified medical assistant Margretta Anne Brown (CM60372740) for at least two years. Brown, representing herself as another health care provider, called in and submitted prescriptions for hydrocodone in her husband's name and admitted to picking them up for her personal use.

In March 2014 the Nursing Commission suspended indefinitely the credential of registered nurse Catharine Jessica Farmer (RN00136035) after she failed to comply with the terms and conditions of a 2012 agreed order by not entering into the Washington Health Professional Services monitoring program.

In February 2014 the secretary of health suspended the credential of registered nursing assistant Marjorie A. Flatt (NA60206939) for at least two years. The Department of Social and Health Services found that Flatt had mentally abused a vulnerable adult over a two-year period while employed by Dream Works Residential, Inc. She is prohibited from being employed in the care of or having unsupervised access to vulnerable adults.

In January 2014 the secretary of health suspended the credential of nursing assistant Kara M. Langford (NC10094425) for diversion of fentanyl patches from patients at her place of employment, and for failing to respond to department inquiries.

In March 2014 the secretary of health suspended the credential of nursing assistant Desiree J. Seymore (NA60246126) for at least three years. Seymore took a controlled substance from a resident at the Beacon House Adult Family Home in Spokane while she was an employee there. Seymore admitted to a Department of Health investigator that she had an addiction to narcotics. Before Seymore can ask for her credential to be reinstated she must provide evidence of being clean and sober for at least two years.

In March 2014 the Nursing Commission suspended indefinitely the credential of registered nurse Karen H. Williams (RN00103810). In May 2013, several coworkers reported that Williams appeared to be impaired. Williams was asked to take a drug test, which came back positive for morphine and cannabinoids.

Yakima County

In March 2014 the secretary of health suspended indefinitely the credential of chemical dependency professional trainee Manuel Armando Din, Jr. (CO60284889) after he failed to comply with the requirements of an agreed order.

In February 2014 the Nursing Commission indefinitely suspended the credential of licensed practical nurse Jaimie J. Melsen (LP00059258). Melsen didn't comply with terms and conditions in a July 2013 Stipulation to Informal Disposition requiring her to enter the Washington Health Professional Services monitoring program.
