State Revokes, Suspends Licenses, Certifications, Registrations of Health Care Providers

OLYMPIA, Wash., Dec. 18 -- The Washington state Department of Health issued the following news release:

The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department's Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency's website. Click on " Look up a healthcare provider license" in the "How Do I?" section of the Department of Health's website (www.doh.wa.gov). The site includes information about a health care provider's license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County In November 2012 the Nursing Commission suspended the credentials of licensed practical nurse and registered nurse Aaron Marshall (LP60110947). Marshall tested positive for amphetamine and methamphetamine and was found to be working while under the influence of these controlled substances.

Clallam County In November 2012 the Department of Health suspended the credential of registered nursing assistant April Price (NA60046479) after Price didn't comply with the terms of probation set against her license in November 2011.

Clark County

In November 2012 the Department of Health vacated the order of non-compliance against certified health care assistant Gerardo J. Nieves (HC60214048) after the department learned that documents requiring his response were sent to an incorrect address and that he had completed required education.

Grant County

In November 2012 the Medical Quality Assurance Commission agreed to permit physician assistant John Bearup (PA10001768) to surrender his credential following charges of inappropriate sexual contact with three patients.

King County

In November 2012 the Board of Pharmacy suspended the credentials of Chelsea Rae Gamblin (VA60206819)(VB60139953) following her arrest for driving under the influence and subsequent blood tests which were positive for Xanex and marijuana.

In November the Department of Health suspended the credential of certified nursing assistant Danielle Laurene Shear (NC10079198) for three years. Shear was convicted of first-degree theft - welfare fraud, second-degree perjury and false verification for public assistance.

Kitsap County

In November the Department of Health suspended the credential of registered nursing assistant Shaveria L. Sneed (NA00122848) for five years. In 2008 Sneed was convicted of theft, and has a previous conviction for a crime of dishonesty.

Pierce County

In November 2012 the Board of Psychology indefinitely suspended the credential of psychologist Kristi L. Mix, also known as Kristi L. Breen (PY00002298). The board determined that Mix is unable to practice with reasonable skill and safety, and set terms and conditions that must be met before she may request reinstatement.

In November 2012 the Department of Health suspended the credential of certified nursing assistant Rissa N. Tauave (NA10088297) for 42 months. Tauave was restricted from working with vulnerable adults DSHS found she physically abused a client under her care.

In November 2012 the Department of Health suspended the credential of chemical dependency professional trainee Daniel Wesley John (CO60261191) after he did not comply with the terms of an agreement to participate in a substance abuse treatment program. John tested positive for substances including morphine, Oxymorphone and marijuana.

In November 2012 the Department of Health suspended the credentials of certified and registered nursing assistant Juwana Rene Tumblin (NC10099285) (NA60204999) for ten years. She was convicted on several charges of theft and trafficking in stolen property after she stole and pawned several pieces of jewelry from patients at her place of employment.

In November the Nursing Commission suspended the credential of licensed practical nurse Mary J. Davis (LP00056245). Davis did not comply with a previous order and the probationary conditions it required. Spokane County

In November 2012 the Nursing Commission suspended the credential of registered nurse Kellie Lee Strauch (RN00117161) for failing to comply with a prior agreement to participate in a substance abuse monitoring program. She admitted to using Oxycontin and Benzodiazepines, tested positive for Tramadol, and submitted another person's urine for her own test.

In November 2012 the Department of Health suspended the credential of certified nursing assistant Melissa M. Williams (NC10101211) for five years. Williams borrowed and stole money from a patient at the facility where she was employed.

In November 2012 the Department of Health suspended the credential of certified nursing assistant Carla Joy Armstrong (NC60075236) for three years. Armstrong was restricted from working with vulnerable adults after DSHS found she neglected a vulnerable adult in her care.

In November 2012 the Department of Health issued a notice of intent to issue a cease and desist order against Larry Shepard who allegedly advertised for and provided laser tattoo removal procedures without a credential.

Thurston County

In November 2012 the Department of Health dismissed charges of unprofessional conduct against agency affiliated counselor Joshua Black (CG60152558) after a hearing.

In November 2012 the Department of Health suspended the credential of certified nursing assistant Spring M. Adeshina (NC10077833) for three years. Adeshina was convicted of fourth-degree assault - domestic violence.

In November 2012 the Department of Health suspended the credential of chemical dependency professional trainee Elizabeth R. Hadnot (CO60191914) for not complying with a previous agreement.

Out of State

Alaska

In November 2012 the Nursing Commission suspended the Washington license of registered nurse Michael Dennis Followell (RN60188057) after his credential was revoked in Alaska for failing to respond to an accusation regarding the death of a patient.

In November 2012 the Nursing Commission suspended the credential of registered nurse Kelly N. Grimm (RN00152160) after Grimm surrendered her license in Alaska for falsely asserting that she had completed continuing education requirements.

Florida

In November 2012 the Medical Commission suspended the license of physician Darrin L. Frye (MD60041498) after his license to practice medicine was suspended in Florida for prescribing steroids - including a banned substance - to patients without medical justification or proper evaluation.
