SOMOS, New York’s largest physician-led network serving Latino and Asian-American populations, and its in-house IT consulting firm, Optimus, will leverage The Garage’s population health platform, Bridge, to integrate all providers, patients and population health solutions into one knowledge engine for advanced data analytics and centralized care management. The unique structure of the collaboration allows SOMOS to house its data with Optimus, which the Garage will use to become SOMOS’s population health platform and data analysis center. The partnership will unify SOMOS’s nearly 3,000 physicians and impact a total population of over 2 million patients, primarily made up of Medicaid.
