Unique Partnership with New York’s Largest Multicultural Physician-Led Network Integrates Multiple Risk-Management Programs into a Single Platform

Business Wire

SOMOS Community Care, The Garage and Optimus join forces to radically simplify data and unify care management for five different entities

ORLANDO, Fla.--(BUSINESS WIRE)-- The Garage, an advanced population health management technology company providing patient-centered digital solutions to stakeholders across the health care industry, joins SOMOS Community Care and Optimus Solutions in announcing their groundbreaking partnership. Together, the organizations will seamlessly increase and optimize care quality and operations across multiple high-risk management programs in New York City through one unified platform.

SOMOS, New York’s largest physician-led network serving Latino and Asian-American populations, and its in-house IT consulting firm, Optimus, will leverage The Garage’s population health platform, Bridge, to integrate all providers, patients and population health solutions into one knowledge engine for advanced data analytics and centralized care management. The unique structure of the collaboration allows SOMOS to house its data with Optimus, which the Garage will use to become SOMOS’s population health platform and data analysis center. The partnership will unify SOMOS’s nearly 3,000 physicians and impact a total population of over 2 million patients, primarily made up of Medicaid.

“As we continue to reinvent the health care system in New York City, it is critical to build partnerships that will bring transformative change to our community and help us deliver more effective, high quality primary and specialty care,” said Dr. Ramon Tallaj, Chairman of the Board of SOMOS Community Care. “We look forward to a long partnership with The Garage and believe the platform will help bridge the gap between our providers and our patients.”

Prior to contracting with The Garage, SOMOS and Optimus worked with several other population health management platforms, but found the solutions lacking, or discovered that they could not deliver on their promises.

“With the Garage, we have not only found a partner to drive powerful, actionable insights, but also an innovative organization that fits seamlessly within our existing structure to facilitate true integration and collaboration,” said Dr. Muhammad Haque, CEO of Optimus Health Analytics.

“Together, our organizations are committed to making a long-term investment into the future of medicine, and to bring more insights and innovation to other organizations looking to unify and streamline their population management and care coordination,” said Pranam Ben, Founder and CEO of The Garage.

Based in New York City, SOMOS is the first physician-led group in New York to be named an Innovator under Medicaid's Value-Based Payment (VBP) Roadmap. The SOMOS network serves Medicaid and Medicare patients across the Bronx, Brooklyn, Queens, and Manhattan.

About the Garage:

The Garage, based in Orlando, Fla., is more than a healthcare IT company. Deeply rooted in the healthcare industry, the company works with more than 25 ACOs and over 10,000 providers, empowering them with solutions to create a more integrated, value-centered and patient-focused care experience. Through its collaborative population health management platform, Bridge, The Garage touches more than 9 million patient lives, allowing providers to achieve the Triple Aim of lower cost, better care and improved health. About SOMOS Community Care:

SOMOS Community Care is a non-profit, physician-led network of nearly 2,000 health care providers serving over 700,000 Medicaid beneficiaries in New York City. Launched in 2015 by its Chairman Dr. Ramon Tallaj, SOMOS Community Care is the largest and only physician-led performance provider system participating in the New York State Delivery System Reform Incentive Payment Program (DSRIP). The SOMOS network includes providers delivering culturally competent care to patients in some of New York City’s most vulnerable populations, particularly Latino, Asian, African-American and immigrant communities throughout the Bronx, Brooklyn, Manhattan and Queens.

About Optimus Health Analytics:

Optimus Health Analytics is a leading technology company focused on developing and deploying population health solutions including health risk assessment and analysis. By leveraging large-scale data sets, advanced data analysis, and analytics-driven intervention systems on our cloud-based platforms, Optimus enables the transformation of care from fee-based to value-based.

Optimus Health supports the improvement of clinical, quality and financial performance across the healthcare ecosystem to managed care organizations, provider offices, and independent practice associations. We bridge the gap that exists between traditional information technology companies who lack the Healthcare expertise required to improve the health of populations while reducing the cost of delivery.
