This is a Sole Source Notice for Medical Surveillance Services for USDA APHIS NCAH in Ames, IA. The Government intends to award an IDIQ contract to Mary Greeley Medical Center on a sole source basis. Please see Justification for other than full and open competition below. 1. Requesting Activity: Occupational Health Clinic Safety and Security Unit National Centers for Animal Health 1920 Dayton Avenue Ames, IA 50010

2. Contracting Activity: USDA APHIS Contracting 250 Marquette Ave. Suite 410 Minneapolis, MN 55401

3. STATUTORY AUTHORITY: AUTHORITY PERMITTING THE USE OF OTHER THEN FULL AND OPEN COMPETITION: FAR 6.302-1 - Only one responsible source and no other supplies or services will satisfy agency requirements [41 USC 253(c) (1)]: The supplies or services required by the Agency are available from only on responsible source and no other supplies or services will satisfy agency requirements.

4. NATURE AND/OR DESCRIPTION OF THE ACTION BEING APPROVED: NCAH is requesting to acquire medical screening services from Mary Greeley Medical Center for their Medical Surveillance Program. Mary Greeley Medical Center is the only medical center located within 10 to 15 mile radius of Mary Greeley Medical Center.

According to the Occupational Safety and Health Administration (OSHA) medical surveillance is the analysis of health information to look for problems that may be occurring in the workplace that require targeted prevention. Medical surveillance may be based on a single event (e.g., an unexpected acute exposure), a point-in-time medical screening result, or the review of a patient's medical history over time.

In addition to monitoring ongoing employee health, medical screening and surveillance at the NCAH also includes pre-placement and new hire examinations. The Occupational Safety and Health Administration (OSHA) has medical screening and surveillance requirements, and the CDC Federal Select Agent Program requires medical certification examinations for employees working with Tier 1 biological agents. Employees must be able to access the contractor's laboratory services within a 10 to 15 mile radius of the National Centers for Animal Health (NCAH). The laboratory must strictly adhere to all Health Insurance Portability and Accountability Act (HIPPA) standards and requirements. Laboratory personnel must possess CLIA competency based training.

5. A DESCRIPTION OF THE SUPPLIES OR SERVICES REQUIRED TO MEET THE AGENCY'S NEEDS, INCLUDING THE ESTIMATED VALUE: An Indefinite Delivery-Indefinite Quantity contract for the collection and analysis of blood and urine samples by a CLIA (Clinical Laboratory Improvement Act) certified laboratory for approximately 350 to 450 employees. Laboratory services must be available 7 days a week, 24 hours a day.

The laboratory will receive requests to conduct analysis of blood and urine samples integral to the NCAH Medical Screening and Surveillance Program and/or from a physician overseeing the work-related medical screening and surveillance of an employee. Sample results must be provided to the NCAH Occupational Health Nurse via encrypted electronic mail or fax within 14 days of sample analysis. In identified instances the NCAH Occupational Health Clinic will request the contractor obtain patient blood samples to test for anti-rabies virus antibody levels using Rapid Fluorescent Foci Inhibition Testing. The contractor will collect these samples and send them to the laboratory under contract for this specific test.

6. A DEMONSTRATION THAT THE PROPOSED CONTRACTOR'S UNIQUE QUALIFICATIONS OR THE NATURE OF THE OF THE ACQUISITION REQUIRES USE OF THE AUTHORITY CITED:

An exposure to a potentially harmful biological or chemical stressor may occur at any time 7 days a week, 24 hours a day at the NCAH. Laboratory services must be available at all times, and employees must be able to access the contractor's laboratory services within a 10 to 15 mile radius of the NCAH. The laboratory must be able to process samples within 2 hours of collection for routine laboratory work.

7. A DESCRIPTION OF THE EFFORTS MADE TO ENSURE THAT OFFERS ARE SOLICITED FROM AS MANY POTENTIAL SOURCES AS IS PRACTICABLE, INCLUDING WHETHER A NOTICE WAS OR WILL BE PUBLICIZED IN REQUIRED BY SUBPART 5.2 AND, IF NOT, WHICH EXCEPTION UNDER 5.202 APPLIES:

A sources sought notice was posted on FBO to find other medical centers who are CLIA certified and within 15 mile radius. Only Mary Greeley replied to the sources sought.

Google Search showed that there is one other source within 15 mile radius, however they only accept federal assistance awards only.

8. A DESCRIPTION OF THE MARKET RESEARCH CONDUCTED AND THE RESULTS OR A STATEMENT OF THE REASON MARKET RESEARCH WAS NOT CONDUCTED: Market Research was conducted to locate medical centers within 15 miles radius and Mary Greeley and Story County Hospital was the only one within that distance. However, SAM shows that Story County are not registered for contracts- only federal assistance.
